What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last key inspection. This included: The annual quality assurance assessment (AQAA) that was sent to us by the The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. Information we have about how the service has managed any complaints. What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement. The previous key inspection. Relevant information from other organisations. What other people have told us about the service. What has this told us about the service? The Boynes returned the annual quality assurance assessment (AQAA) when we asked for it. This was brief in parts, but overall all the sections were completed and the information gave us a reasonable picture of the current situation at the home. The last inspection made a requirement about the need to improve the moving and handling risk assessment documentation, and there were ten recommendations made. The AQAA does not state if this improvement has taken place and does not give any information to show they have considered and implemented any of the recommendations. The AQAA and the last key inspection report showed that the service obtain comprehensive information about peoples needs and expectations before they agree to admit. This helps the service decide if the potential admission is likely to be a success for everyone. The AQAA tells us, all prospective residents have a comprehensive pre-admission assessment usually with relatives or a social worker if there are no relatives. On admission all residents have a comprehensive nursing assessment and risk assessment to form a base line for their care. The AQAA told us how the service is operating and improvements they have made and wish to make. We read the AQAA and our judgement is that the service is still providing a two star good service. The key unannounced inspection, which was carried out on the 02/03/2009, formed a judgement that the service was providing a two star good service. To ensure equality and diversity is addressed, we saw at the last key unannounced inspection that assessments of peoples individual physical and social care needs are undertaken on admission to the home, and care plans are derived from this information. These are individual plans written with the involvement of people and their representatives about what people can do for themselves and in what areas they Annual Service Review Page 4 of 7 require support. We also saw that care reviews are undertaken with people living at the home, their families and the home staff, in order to assess whether their care needs are still being met there. This confirms a person centred approach to care. The AQAA contained numerical data which told us they provide a service for older people of both sexes, who have a range of needs such as general frailty due to old age, mental health needs and dementia, physical and sensory disabilities. The service currently employs a multi-cultural team of male and female care staff. We received seven surveys from people who use the service and six of these people had been assisted to complete the forms by their relative or the relative had completed them on their behalf. This is what the people who use the service or their relatives said about the service and its staff when we asked them what the service does well Staff are kind and competent. My Mother is always clean and tidy. The Boynes gives excellent food. It is always lovely and warm and homely. There is always someone to talk to if there is anything you need. The Boynes is a friendly and welcoming nursing home. The nurses and carers are always helpful and do their best to please. The nursing care is excellent and there is always a nurse on hand. Food - there is plenty of choice and it is nicely presented. Hot dishes are always very hot. Activities, many and varied are provided for the residents, but there is no pressure to take part. Special treats are on hand on important occasions, Easter, Christmas, New Year etc. Residents privacy is respected and we are not bothered with unnecessary rules and regulations. Visiting staff who join residents in their rooms on certain days provide stimulation and interest. I go to the home every single day and I only have to ask for anything for her and they do everything in their power to help. Welcomes personal belongings and will put up pictures and photos for residents to make their rooms cosy. Staff explain to the resident what theyre doing while attending to personal needs. Staff take a pride in helping my mother to dress prettily and add the important touches like jewellery. If a particular meal/drink is not to the residents liking staff fetch an alternative. Staff initiate conversation with residents. They listen to the things I would like such as a daily shower and do their utmost to meet my individual needs. They make me feel safe. They are conscious of what I do and do not like to eat and make specific adjustments to ensure I can eat what I like. They will even pop to the shops to purchase it. This is what the people who use the service or their relatives said about the service and its staff when we asked them what the service could do better: The food at The Boynes is excellent. It is a pity that the residents do not get their meals at the same time in the dining room, it seems to get served willy nilly and some have finished their meal before other have had theirs. Owing to pressure of work, carers are not always able to cope with bed making until late morning (12 oclock). Similarly trays and dirty cups and saucers are not always removed for several hours, or even until the next day. Usually a carer comes round to give details of the next days menu but this is not always done. Respond to residents call buttons more quickly. People who use the service and their relatives also told us, the management and nursing staff are always there to give help and support whatever the problem they always give their time and act quickly. The care is very good. Staff smile. The Boynes has a happy atmosphere. Foreign staff have excellent English and chat easily with my mother. Visitors are made to feel very welcome. As a relative staff are always available to discuss my mothers health and needs. Staff really do care, more than just a job. I think you would go a long way to find anywhere better. They really did their
Annual Service Review Page 5 of 7 best at Christmas and New Year and I had a wonderful time. If they didnt look after me so well my illness would be much worse. Information in the last key inspection report dated 02/03/09 also tells us that the people using the service were pleased with the standard of care they receive. The AQAA told us the service has received four complaints in the last 12 months, and none of these complaints were upheld. We have received a complaint about the service since the last inspection, and we passed this to the owner to investigate. This complaint was upheld. There have been five referrals to the local multi-agency safeguarding team regarding allegations of neglect of care of five identified people who use the service. The outcomes provided by Worcestershire County Council are two were not proven, three were inconclusive. There have been no changes to the management of the home since the last inspection. The AQAA tells us about the policies and procedures the service has in place and when they were last reviewed. Some of the policies in place have not been reviewed for over two years. These policies need to be reviewed as a matter of urgency to ensure the guidance available for staff reflects current legislation and practice. What are we going to do as a result of this annual service review? There will be no change to the inspection plan and will do a key inspection by 01/03/2012. However, we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
