Care Homes for Adults (18-65 years) Page 3 of 9 What we found: The reason for the visit was to undertake a pharmacist inspection of the service. The visit lasted 2.5 hours and involved looking at arrangements within the service that support the safe handling and administration of medicines. We looked at the medication policy, medication records, storage, security and stock control. We gave verbal feedback to two directors of the provider company. Medicines storage in the service is generally secure and well managed although the fridge used to store medicines requiring refrigeration was very dirty and contained food products alongside medicines. Temperature monitoring of the fridge was inadequate to ensure that medicines are stored within the temperature range recommended by the manufacturer. The current Medication Administration record (MAR) charts were looked at. These charts are prepared as required by staff within the service and are designed to support the recording of all essential information on medication requirements for each service user. The front sheet of the record is used to record a summary of all the medication a person receives and subsequent sheets are printed off to record the administration of individual doses of medication to each service user. For one service user the summary of medication had not been updated to match the changes in medication recorded on the administration records. When the record sheet for a service user is full a new sheet is printed but there is no process in place to confirm that the printed entries are correct and that the document has been checked by a second person to confirm accuracy. Medication administration procedures within the service are well managed and involve two staff throughout the administration process. There were no significant gaps in the administration records, or any discrepancies between the quantities of medicines received, the number of doses recorded and the quantities of medicine remaining. This indicates that people are getting their medication as prescribed. Care plans for service users provide detailed guidance on how carers should meet the health care needs of service users including how to manage seizures, for example. There is no controlled drug cupboard in the service but there are currently no service users receiving controlled drugs requiring secure storage. The ordering and disposal of medication is well controlled , with no evidence of surplus medication in the home. All staff handling medication have received appropriate training and there is a programme of refresher medication training in place throughout the year to update staff.
