5 5Crosby Lodge is located in a quiet residential area of Charminster, Bournemouth, within easy reach of local shops, churches, library and doctor`s surgeries etc. Buses run nearby to other areas of Bournemouth and beyond. The town centre and seafront are less than two miles away. On street parking in the vicinity of the Crosby Lodge is available for visitors. The home is registered to accommodate up to 26 older people with mental disorders and/or dementia. (Variations to registration have been granted for the admission of five service users under the age of 65 years.) Crosby Lodge comprises two separate houses situated next door to each other. Both have ramped access to the front door and a stair lift is available at 2A to assist access between floors. Accommodation at 2A includes a lounge and small separate dining room, four double and six single bedrooms and No 2 includes a lounge and separate dining room, two double and eight single bedrooms. There are no en-suite facilities, but sufficient communal bathrooms and WC`s are available. The home has a well maintained garden, which is planted to provide year round interest and is mostly accessible to wheelchairs. An attractive park lies on the opposite side of the road, with a bowling green, tennis courts etc, and seating. Crosby Lodge provides 24-hour personal care, all meals, laundry and domestic services. Small pets may be permitted, by agreement.

What the care home does well The home keeps clear records of all medication changes made by the GP or consultant and any tests or reglar appointments which people need. The manager responded positively to advice from the Primary Care Trust Pharmacist and put the recommendations into practice. What the care home could do better: The method of administering medicines must be changed to ensure that it is safe and staff are individually accountable for their actions.The competency of care staff to administer medicines safely and follow the home`s procedures must be checked regularly. Care Homes for Older People Page 3 of 9 What we found: This inspection was carried out by the pharmacist inspector to look at all aspects of medication handling in the home. The home had recently been visited by the pharmacist from the Primary Care Trust (PCT) who had given them advice about safe storage, record keeping and staff training which the manager had already started to put into place. Staff had recently made a mistake with someones medicines and this had prompted the visit. All staff were having training in medication handling and the manager had agreed to use information from the PCT to help her to test the competency of her staff to carry out the task. The current policy did not include a procedure which stated how staff should do the various tasks involved and the PCT were helping to produce this. The home had suitable storage for all medicines except conrolled drugs. The medication administration charts were correctly completed and new audit forms had been started to allow all mediines to be accounted for. Photographs were available to help staff to identify the correct person for the medicine. The care plans of the people we looked at had clear, up to date information about medication changes or advice from their doctor. The way that medicines are administered in the home is not consistent with current good practice guidelines. This was the case in both areas of the home. Due to the restrictions of space and the diverse ability of the people in the home, two staff must be available when medicines are given as is currently the case, but the member of staff who signs for the medication must administer it themselves and take responsibility for the whole process. The manager agreed to review their practice and institute a safe method of administration. She had already obtained a coloured tabbard which the member of staff could wear to indicate to other people that they should not be disturbed whilst concentrating on the medication. During the inspection we also discussed the care staffs ability to communicate with the people in the home as many of them do not have English as a first language. The manager said that she was aware of problems with certain members of staff but was takng steps to rectify this. Staff without a sufficient knowledge of English may not be able to complete the medication training or pass a competency test within the home.
