Dothan House is a registered care home for 19 people aged 65 and over. The home is situated in a residential area of Gidea Park and there is parking at the rear of the property. The home is a large two storey detached house with extensions. There is a passenger lift to the first floor. There is a combined lounge/dining area on the ground floor and another smaller lounge on the first floor. There are 17 single bedrooms and one double bedroom. Nine of the bedrooms have en-suite facilities. There is shower room and a bathroom. Personal care is provided on a 24- hour basis, with health care needs being provided by health professionals such as GPs and community nurses. The charge per week for each person is between 430 & 440 pounds. This information was provided by the proprietor at the time of the last key inspection. What the care home does well A relative said, "I visit three times each week. The manager and staff are excellent. They are patient and there are lots of activities. It`s a real family place". A resident said, "The deputy/acting manager is good and has done a lot for us since she has been here. We do a lot more here and we go out more as well". Staff were observed to smile and talk to the residents as equals. Staff are receiving the training and the support they need to carry out their duties and to provide a good service to the people who live at Dothan House. Residents are seen and treated as individuals and receive personalised care as far as possible. What the care home could do better: The proprietor is still the registered manager for the service and does visit regularly. However the deputy is running the home on a day to day basis and is seen as the manager by staff, relatives and residents. If this arrangement is to continue an application must be made for the deputy manager to be registered with the Commission as the manager of the service. Although the Commission is notified of most of the things that happen at the home it is important that they are notified of any event in the care home which adversely affects the well-being and safety of any of the people using the service. This is to comply with regulations and also to enable the Commission to monitor the service. Information about the service provided is contained in the service users guide Care Homes for Older People Page 3 of 10 What we found: Two unannounced visits were made to Dothan House. The first was on 24th June at 10 pm and this visit lasted for approximately one and a half hours. During this time we spoke to some residents, to staff on duty and to the deputy/acting manager. We also visited the lounge and one of the bedrooms and checked some records. A second visit took place on 17th July 2009. On this occasion two inspectors arrived at the home at 9.30 am and stayed for approximately 4 hours and 30 minutes. On this occasion we again spoke to the deputy/acting manager, residents, staff and a relative of someone living at Dothan. We also toured the building and checked various records. 24th June 2009 There were two staff on duty and three residents were still up and sitting in the lounge. The rest of the residents had already gone to bed. The deputy/acting manager arrived at the home about 40 minutes after the inspector. We had the opportunity to have a more lengthy discussion with one of the staff. She was able to confirm that she has had the necessary training to carry out her duties and to clearly tell us about the tasks that night staff are required to do. She was also able to describe what would happen in the event of an emergency and confirmed that the deputy/acting manager was always available for support. One resident said that as it was only 10 it was too early for him to go to bed. However a short while later he decided that he was ready to go to bed and staff supported him to do this. Another person had been to bed for a while and had got up again. Staff made this person comfortable in an armchair and covered her with a blanket. She was offered drinks but declined to have any. She was obviously very comfortable and relaxed sitting in the chair. Staff were able to explain how they supported the third person and why it was best for him to go to bed later than other people. At the time of this visit there were sufficient staff on duty to meet the needs of residents and residents appeared to be appropriately cared for. Staff on duty were aware of residents needs and how to meet them. There were no requirements or recommendations as a result of this visit. 17th July 2009. There were 18 residents at the home with four care staff, a cook, domestic and handyman on duty. The deputy/acting manager arrived at the home shortly after the inspectors. From discussions with staff and observations on the day it was apparent that there was sufficient staff on duty to meet peoples needs. We had the opportunity to talk individually with two of the staff team. Both people were able to confirm that they had had the necessary training to carry out their duties. One person had not yet had POVA (Protection of Vulnerable Adults)training but was aware of safeguarding issues and was due to do the training in the near future. Both people were clear that they had never seen any of the residents treated badly or heard shouting and also clear that they would report this to the manager, or to social services or the Commission if they did. Both people said that they were happy working at Dothan House and that there was a good and supportive team. They also said that they get good Care Homes for Older People Page 4 of 10 support from the deputy/acting manager and that she often checks with them if everything is okay. From looking at records, talking to staff and to residents it was evident that activities are regularly taking place both in the home and in the community. On the day before the inspection some of the residents had been in the garden planting potatoes and painting a garden bench. On another occasion previously they had been making cakes and decorating them and had a competition to see which was the best decorated cake. People had also been on a trip to the London Eye and to Southend. Some residents go to church regularly and another likes to go to the pub. One resident said that the deputy/acting manager is good and had done a lot for them. She talked about the trips and the activities and said she enjoyed these. We looked at a sample of residents care plans and found these to be comprehensive. Each person also had a night care plan. Daily logs are kept and these were detailed. Additional monitoring is carried out when necessary. For example someones food or fluid intake might be recorded. From discussions with the deputy/acting manager and with staff it was evident that they know the residents well and work hard to ensure that they are well cared for and as healthy as possible. Records are also kept of any injuries, accidents or near misses. These records were comprehensive and gave a clear picture. However not all of these had been reported to the Commission as required and the deputy/acting manager was advised of this. We checked menus and these were varied and nutritious. Some people require fortified food and when necessary the potatoes are mashed with cream rather than milk. Staff spoken to confirmed that there was always plenty of food for people and there were adequate stocks in the kitchen, store cupboard and freezer. Additionally the delivery of food was expected later that day. At the time of this visit the home appeared to be clean and hygienic and there were no unpleasant smells.
