What the care home does well The service offers a well-planned and personal level of support to the people living there, ensuring that they receive information about the home and events and activities that are available. Procedures are in place to ensure that issues are dealt with quickly, and everyone living in the home is aware of how to make any concerns known. The service offers a comfortable home to the people living there, and the ongoing building works, when complete, will provide improved facilities. What has improved since the last inspection? Since the last inspection, improvements to the homes procedures have addressed the requirements that were made, and together with the above mentioned improvements to the buildings will give people in the home better facilities, many of these already being in use. What the care home could do better: The manager of the service is not yet registered with the commission, and an application is expected shortly, and they need to ensure that their individual plans are fully utilised, and contain as much detail as possible. This means the people who use this service experience good quality outcomes. This was an unannounced inspection, and looked at the key standards for older people. During the visit, the manager was available to assist with information, and the proprietors were also there for part of our visit. Prior to the inspection, an Annual Quality Assurance Assessment was submitted (AQAA), and information and comments form this, and from people living in the home has been used in this report. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 3, 5 and 6 Quality in this outcome area is good This judgement has been made using available evidence including a visit to this service. Detailed information is gathered before anyone moves into the home to ensure that people looking to move into Driftwood can be confident their identified needs will be met. EVIDENCE: Prior to anyone moving into the home, a detailed assessment of need is undertaken. The home has a mixture of privately funded and local authority funded residents, but would ensure that irrespective of the funding arrangements, a pre admission assessment is carried out. From this they are able to ensure that a person’s needs can be met by the home, and that any necessary equipment, aids or adaptations are available. Driftwood House DS0000027358.V367878.R01.S.doc Version 5.2 Page 9 Wherever possible, they meet with the person hoping to move into the home, but occasionally this is not possible in which case they meet with relatives. They also try to arrange for the person moving to the home to visit before they decide if they wish to move in. During our visit, we looked at some of the records held on people living in the home, and the pre admission documents were seen on the files we examined. Once it has been established that the home is able to meet the needs identified in the assessment, and that the person involved is happy with their choice of home, arrangements are made for them to move in, contacts completed and an initial care plan prepared. It was recommended that the home confirm all the details in writing before the person moves in. The service does not offer intermediate care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 7, 8, 9 and 10 Quality in this outcome area is good This judgement has been made using available evidence including a visit to this service. Individual plans provide information to care staff on how to meet needs, although more detail would be a benefit. Procedures in place ensure medication is administered safely at all times. EVIDENCE: Once it has been established that the home is able to meet the identified needs of the individual, an initial care plan is written in readiness for the person to move into the home. This provides basic details to enable the home to provide the required support as soon as the person arrives, and is reviewed and developed further as they get to know the person better. A new format of care plan was introduced last year, and during our visit we looked at the care plans for three of the people living in the home. The plans provided the basic information required, but need to have more detail, using the format they already have, and need to record medical appointments such as chiropody visits. Driftwood House DS0000027358.V367878.R01.S.doc Version 5.2 Page 11 Last years inspection report noted some difficulties with the local GP practice, but following meetings with the surgery Practice manager, these issues have been resolved, and the relationship between them is good. Medication is administered using the Boots monitored dosage system, and training is provided to all staff involved in the administration of medicines. Since the last visit, a new stock control system has been introduced, which has addressed the concerns that were raised. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 12, 13, 14 and 15 Quality in this outcome area is good This judgement has been made using available evidence including a visit to this service. A range of activities and events are available to people living in the home, often including family and friends, and regular residents meetings allow people living in the home the opportunity to discuss aspects of the home, including menus. EVIDENCE: Although the home does not have a dedicated activity worker, care staff help in providing activities for the people living in the home. A list of activities is available on the notice board, and the manager confirmed that wherever possible, they encourage people living in the home to get out and about, and take part in the events that they arrange. These include regular entertainment, church services, arts and crafts and a library. Following building works in the home which are shortly to be complete, the home will be having a summer fete, which is being opened by the home’s oldest resident who is 104, and will include families and local dignitaries, including their local MP. Residents meetings are held every three months or so, and very well attended, at the last meeting in April, all 19 people in the home at the time attended, and the meeting gives them chance to discuss events and activities, planned changes to the home, menus and any other concerns. People living in the home are aware of how to make concerns or allegations known, and confident that these will be listened to. EVIDENCE: Procedures are in place to deal with any concerns or complaints raised with the home. A copy of their procedure is included within the service user guide, and given to relatives. The commission have not received any concerns in the past year. Adult safeguarding procedures are in place and staff receive training as part of initial induction training as well as regular training updates. There have been no safeguarding issues raised in the last year. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 19 and 26 Quality in this outcome area is good This judgement has been made using available evidence including a visit to this service. The alterations to the premises, which are almost complete, have improved facilities in the home for the people living there. EVIDENCE: Building works that were being done at the last inspection are almost complete, which will add additional rooms, and improve facilities. It is anticipated that these works will be completed shortly, and a big opening event is planned. In the meantime the home is managing to maintain cleanliness within the home, and minimising disruption to people living in the home. On the day of our visit, the home was clean, and no unpleasant odours were noted. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 27, 28, 29 and 30 Quality in this outcome area is good This judgement has been made using available evidence including a visit to this service. Recruitment procedures ensure that required information is obtained on all employees to ensure the safety of the people they will be supporting, and a range of training provided to staff. EVIDENCE: The service operates with a senior and three care staff in the mornings, which reduces to two care staff during the afternoon and early evening. The staffing levels will be looked at again when the service completes the building works and the new rooms come into use. In addition to care staff, the service has domestic and kitchen staff, and the new manager no longer takes a hands on care role. Recruitment procedures are in place to ensure that adequate checks are made, and evidence of these checks was seen on the files examined during the inspection. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. Health and safety procedures ensure that staff and residents are not placed at risk, and regular discussion with people living in the home maintains their place at the centre of everything that is happening in the home. EVIDENCE: Although the services manager has already completed her Registered manager’s award, no application for registration has yet been submitted. The home’s proprietors remain in day-to-day control of the home, but an application will be submitted shortly. Driftwood House DS0000027358.V367878.R01.S.doc Version 5.2 Page 18 The service puts the needs of those living in the home at the forefront of all that they do, and residents that were seen during the inspection appeared very satisfied with the support they receive. People we spoke to during our visit were more than happy with the support and service they receive, with comments made such as ‘they look after me very well’ ‘the staff are wonderful’ The service avoids dealing with resident’s financial matters wherever possible, and ensure that any financial transactions are recorded and signed for by two people.
