Ealing House is a care home providing personal care and accommodation for 17 older people some of whom have dementia. It is owned and managed by Mrs Sally Watson. Ealing House is located in the centre of the village of Martham close to shops, pub and local amenities. It is a short car ride to the coastal town of Great Yarmouth. The home consists of a detached two-storey house, with a variety of communal space. The building is situated within its own grounds, with an enclosed garden at the rear. Annual Service Review Page 3 of 7 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received, after we asked for, since the last key inspection. This included: - The annual quality assurance assessment (AQAA) that was sent to us by the service. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. - What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement. - The previous key inspection and the results. - Communication we had with the home and the providers since the last inspection. Questionnaires were sent to service users, their relatives and staff. At the time of writing the report 5 service users, 6 relatives and 4 staff questionnaires had been returned. Where possible information and comments from these questionnaires has been included in this report. What has this told us about the service? The home continued to provide good care to its service users. The home housed 16 people, 14 female and 2 male. Twenty permanent staff managed to cover the needs in the home without engaging agency staff to cover for absences. The home explained their admission procedure: Any enquiries are encouraged to come and visit the home at any time to see if it meets their needs. I also encourage them to visit many other homes in the area to ensure they have a balanced picture of what is available. They are offered to come and spend a day with us (free of charge) to get a better feel of the home. All prospective residents are assessed by up to two members of staff experienced in this role. On arrival further assessments are made in relation to MUST, falls assessment, skin / pressure risk assessment and Manual Handling. All residents come on a four week trial to ensure that their needs are how they have been conveyed to us and that we can satisfactorily manage these. This trial period can be extended for those that are finding it difficult to make that final decision. The home also reported: There is an exceptionally high standard of personal care at Ealing House. We have a shop to allow residents to choose their toiletries and to ensure they never run out. These are all individually labelled. Pressure area risk is assessed and recorded and acted on accordingly, pressure relieving aids are used. Ealing House owns 3 pressure mattresses and numerous topper mattresses. All residents are assessed via the Continence team and appropriate toileting regimes are put in place with use of allocated individual named pads. We are constantly looking at the individuals needs both psychologically as well as physical. Residents are actively encouraged to keep mobile and move about the home. They walk through for meals and coffee, and this is actively encouraged. All residents are screened using MUST, this is regularly reviewed (monthly) and appropriate action is taken. New sitting digital scales have been purchased. Generally all residents are transferred to the local GP surgery as this is policy from the PCT. Ealing House has the input of CPNs, district nurses, dentists, chiropodist x 1, Speech Therapy, GPs, Continence nurses. Annual Service Review Page 4 of 7 Medication is handled safely by trained staff and according to regulations. Staff attends external approved medication training. Local PCT (Primary Care Trust) pharmacist audits medication and offers advice when necessary in addition to homes own audits done on a weekly basis. New, lockable fridge was purchased to ensure that medication is stored at the appropriate temperature. Ealing House has a phone system so residents can have phones in their bedroom with personalised bills. As the home is upgraded the doors are being fitted with locks so that residents can hold their own key. Privacy and dignity are well respected within the home. The home arranged activities according to the residents wishes and preferences and coordinated and conducted activities through care staff that know residents well are offering different tings at different times, thus creating flexibility. The manager reported in their AQAA: Ealing House is participating in an Assistive Technology Project where we are looking at the benefits of using Assistive Technology within the care home setting. This has highlighted looking at peoples abilities and trying to give them more independence. The home promotes individuality, but also cares about peoples safety and protection: Residents are encouraged to manage their finances if able. For those that cannot, money in a cash tin is kept and administered to by a few senior staff. Personal possessions are encouraged to be brought. This includes furniture to ornaments, pictures etc. In January 2009 the home was awarded 5 stars on inspection by Food and hygiene inspectorate. The home also reported in their AQAA and the returned questionnaires confirmed excellent standards of food: We are also using Safer Food, Better Business, so we have reviewed our practice throughout. Meat and meat products are delivered in from the local butcher several times per week. Fresh vegetables are bought locally where possible or local cash and carry. The proprietor has an allotment so when there is an abundance used in the home. We have two cooks that work opposite each other and they produce the lunch and tea. Both Cooks undertook NVQ Level 2 in Professional Cookery. There were no complaints, concerns or allegations made from the home. All staff are trained in Awareness of Adult Protection. The manager and the owner reported on the environmental aspects: Ealing House is an exceptionally well maintained home with many areas of the home already refurbished to a high standard. Facilities are being improved in all communal areas. Our new building and upgrading of existing areas exceeds what the Fire Service requires. All areas are decorated to be homely and domestic in appearance. Improvements in the environment included a new laundry cupboard upstairs, new medical room, a small hairdressing salon, a new sliding door on downstairs bathroom to allow easy access for hoist and equipment as well as residents. The home has a very dedicated and caring staff team, with excellent carers who are committed to providing high standards of care. There was a further improvement in staff qualifications, with 58 holding NVQ now. The AQAA also reported: All new staff are inducted into the work place and have supernumery status for up to three weeks. Induction day programme in place coving the statutory requirements. Training plan is in place for next 12 months. The manager, who is also the owner, is a senior nurse and a NVQ assessor and a qualified Manual Handling trainer. The owner reported in their AQAA showing her commitment to achieving high standards: We are a family run home. We have invested financially as well as emotionally into this home and maintaining and improving our high standards. By the Annual Service Review Page 5 of 7 time the improvements are complete we will have spent in the region of £400,000. We carried out a survey and returned questionnaires confirmed the report presented in AQAA. Residents commented: The home keeps everything nice and clean. Staff are very cheerful and supportive. Relatives responded in a good number and stated in their questionnaires: The home is very well run. All the residents are very well cared for. The staff are always pleased to see you when you visit. They are all very efficient. I have no complaints about the home, only praise. The services at Ealing House are excellent right from the manager to the cleaning staff. I am extremely happy with all services at Ealing House. Staff responded in the same, positive manner and maybe the best response came from a staff member who stated: The home treats people as individuals and encourages their independence. Staff work well together and we receive good support from senior staff and the manager. What we could do better? We need more time to spend with residents. Listening, talking, doing their nails, but can there ever be enough time, especially for people with dementia? The manager described the home and care: We treat everyone as individuals, find out about their life history and incorporate into their care plans. We are fully aware of the individuals needs and assist them accordingly. This is down to being a small personal home as opposed to being a large institution. What are we going to do as a result of this annual service review? We will continue to monitor service and the wellbeing of service users. We plan to inspect the service at the anniversary of this report if anything relating to service users or the process of operating does not indicate the need for an earlier inspection.
