What the care home does well The tool being used where the night staff team monitor each others side of the home throughout the night is good. This means that by using peer audits any deficit or concern about someones practise will be identified immediately. Management had acted upon previous concerns, they have carried out random spot checks during the night and are going to continue with these. They also make staff specify in a book the actual time that they will be taking their break during the night. Thismeans if the manager undertakes an unannounced visit, staff if they are not working when she arrives will not be able to say they are on a break. The manager would then be able to follow any appropriate disciplinary action with the staff concerned. What the care home could do better: The practise of waking people up at the early hour of 05.30 if it is not their choice to do so, is unacceptable. This must change, people have a right to wake naturally or be woken at a time of their choosing. Date of last inspection Brief description of the care home Elm Lodge is an extended Victorian House situated in Linslade on the outskirts of Leighton Buzzard. There are good road links to Milton Keynes, Bedford, Aylesbury and Care Homes for Older People Page 2 of 8 2 4 0 2 2 0 0 9 Brief description of the care home Luton and the centre of Linslade is within walking distance. There is parking available at the home and it is set in attractive grounds. The home can provide residential or nursing care depending on the assessed needs of residents. There are 49 single rooms and 1 room that can be used for shared occupancy providing a total of 51 places. There are 12 rooms with en-suites and various communal facilities including 6 separate lounges, dining areas, bathrooms and toilets located throughout the home. As the home has been converted and extended, the accommodation ranges across different floors and there is lift access to all areas. The fees for this home vary from £495.00 per week for residential placements, to £615.00 per week. Care Homes for Older People Page 3 of 8 What we found: This unannounced random inspection was undertaken on the morning of 28th October 2009 by one Regulatory Inspector. The Care Quality Commission had been contacted in October 2009, concerns were raised that people living at the home were being woken from 05.30 in the morning and then being assisted to wash and dress and had to get up. In addition there were concerns that some staff had been sleeping when on night duty. Similar concerns had been raised twice about the home in 2009. On the first occasion we looked at these as part of the key inspection, on the second occasion we asked the management of the home to investigate. The focus of this visit was to look at the current practise surrounding people getting up in the mornings, staff on duty and what management checks were in place to monitor night working at the home, we did not follow up on any previous requirements that were made at the last key inspection. Therefore any requirements showing at the end of this report are not outstanding because they have not been met by the home, but because we have yet to assess their compliance. We entered the building at 06.30, at that the time we observed 6 people up and sitting in the large lounge in the residential side. On walking around to the nursing side 4 people were up and dressed in their rooms. The manager was within one of the bedrooms at that time and was assisting a person to get up. At 06.50 we spoke with two staff on duty who confirmed that there were now a further 3 people up on the residential side. So at 06.50 there were 11 people up, washed and dressed. They explained that there were people living in the home, due to their dementia would repeatedly get up out of bed before 05.30. The staff would assist them, but this was only because the person themselves would get up. The staff however confirmed that they would always start to get other people up at 05.30. Through discussion with the manager and viewing documents it was evidenced that management had and continue to carryout checks regarding night time working. A new system had been introduced on 12/10/09 where staff from both the residential and nursing side undertake checks on each others area throughout the night. Sheets were seen of how staff record that these have been carried out. In addition the manager and owners have met with staff. Also the manager has undertaken spot checks during the night, the most recent one she arrived at the home at 02.30 in the morning. Further checks are planned.
