What the care home does well People living in the home have a choice of both surgery and pharmacy who will supply their prescription medicines. What the care home could do better: The policy should be expanded so that staff have guidance on how to administer medication safely. Records of administration after medication has been given, this makes sure it is an accurate record. There is a small garden to the front of the property, and a private paved area to the rear where people can sit outside. It is situated on a busy main road close to Middlesbrough town centre, so is convenient for services required by the user group. Accommodation is provided for up to fifteen people in fifteen single bedrooms (seven with en-suite facilities) Of these rooms, three were under size requirements. However, the flats were spacious, with each person having their own kitchen, bed sitting room and bathroom. These Care Homes for Adults (18-65 years) Page 2 of 8 Brief description of the care home were used by people who had completed the first two stages of their rehabilitation programme, and were progressing towards eventual discharge. There are two lounges and a large dining/activity room as well as a counselling room in the main building. People are supported and encouraged to use both the kitchen and the laundry. The home offers a programme of rehabilitation that may include a period of detoxification for both drug and alcohol dependencies. Fees charged by the service range from £448.51 to £514.66 per week. Care Homes for Adults (18-65 years) Page 3 of 8 What we found: The reason for the visit was to undertake a pharmacist inspection of the service to look at the arrangements within the home that support the safe handling of medicines. The visit lasted two hours and involved looking at medicine records, policy, storage and administration. Staff involved in the administration of medication had not any medicine training. This had been identified and training is planned for July 2010. The medication policy needs to be reviewed. The current policy does not provide enough information for staff on current legislation and guidance to make sure that safe practices are followed. Medication is stored in a locked cupboard in an office area. The office is multipurpose and there is now a process in place to prevent interruptions during medicine administration. A record of room temperature is kept but not all staff were sure how to read the thermometer. No fridge was currently available as no medicines required refrigeration. The home uses an in house medication administration record which has recently been updated on the advice of a visiting health care professional. The medicine administration records (MAR) contained some gaps in record keeping, and an audit of current stock and records showed that some medication had been signed for but not given. One person had a dose change noted on their MAR but when asked staff at the home could find no written confirmation of this change. To demonstrate medication is being given as prescribed the MAR should record each administration. One MAR chart had medication not given because there was no stock available. It is important to make sure that the quantity of medication is regularly checked so that a prescription can be ordered in plenty of time to prevent people being without. Accurate records must be kept for all medicines including controlled drugs. This will make sure that people receive their medication correctly and the treatment of their medication is not affected. 2 9 13 Additional training that 31/08/2010 reflects best practice guidelines must be provided to all staff involved in the administration of medication. Having well trained staff helps to reduce the risk of medication errors.
