Greenbushes is registered to provide nursing care to older people. The home is managed by BML Health Care on behalf of the provider organisation, Dorchester Care Limited Liability Partnership. The house is surrounded by gardens laid to lawns and flowers beds, with space for car parking. During 2004 an extension providing ten additional bedrooms and a large communal room was completed. The home comprises thirty two single and five double bedrooms. Seventeen bedrooms have ensuite toilet facilities. All registered parts of the home can be accessed without the necessity to negotiate steps or stairs. There is a passenger lift and some corridors are sloped. Laundering of clothing and household linen is carried out within the home at no additional cost to residents. Items requiring dry cleaning are charged additionally because it is necessary to send them out of the home for this service. A hairdresser visits the home each week. There is an additional charge for this service. A public transport bus stop, for buses to the nearby town centre, is opposite the home. Fees are charged weekly. The fee range quoted in the service user guide at the time of inspection was (per person) from #550 for a shared room, from #600 for a single room and from #700 for single room with en suite hygiene facilities. Annual Service Review Page 3 of 6 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last key inspection or annual service review. This included the annual quality assurance assessment (AQAA) that was completed by the service and returned to us on 23rd February 2009. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. Notifications received. These are a legal requirement; the service must tell us about certain things that happen in the service. The previous inspection undertaken on 9th April 2009 What has this told us about the service? The home sent us their annual quality assurance assessment (AQAA) when we asked for it. It was clear and gave us all the information we asked for. In the completed AQAA, Beverly Welch, the registered manager at Greenbushes has identified what they feel they do well and gives no cause for concern about the service, which is consistent with the last key inspection undertaken April 2008. A random inspection took place on 4th April 2009 as concerns had been raised; these concerns are subject to separate investigation and the random inspection did not identify any areas requiring improvement. The key inspection in April 2008 made no requirements although three good practice recommendations were made. The AQAA states that a letter is now sent to residents following assessment to confirm that the home is able to meet their needs and meals are provided to residents based on their preferred diet and staff provide support for residents requiring assistance. The third recommendation regarding residents access to the staff call system has not been discussed in the AQAA although reference is made to good, safe working practices. In the AQAA Beverly Welch has identified that improvements in the next 12 months will include an increase in staff training in care specific skills that are relevant to elderly care and more provide more time for key workers to develop stronger relationships with their residents. In the last 12 months, more staff have been employed, the service user guide has been revised and updated, the menu has been reviewed and there has been redecoration and upgrading of some rooms and a sensory garden has been provided. The data provided with the AQAA tells us about the level of occupancy in the home and an outline of the dependencies of service users, the data also provides information about the staffing levels and competencies. 6 of the 15 permanent care staff employed (36 ) have achieved NVQ level 2 or equivalent and a further four care staff are undertaking this award. Correspondence and communications held on file relating to reported incidents have been well managed by the service (The Care Homes Regulations (CHR 2001) expect homes to inform the Commission about certain incidents or events in the home, these are called notifications under regulations 37 of the CHR) Annual Service Review Page 4 of 6 We have not received any surveys from service users giving their views on the service and care provided although it is evident from the completed AQAA that Greenbushes uses an internal quality assurance audit using surveys and questionnaires to residents and relatives, relatives meetings and with residents and their relatives involved in the process in writing and reviewing their care plans. What are we going to do as a result of this annual service review? Depending on the results of the investigation into concerns raised we are not going to change our inspection plan, and will do a key inspection by April 2010. However we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
