Residents befefit from being cared for by a small group of staff that know them well. The arrangements in place to promote fire safety are adhered to consistently. What the care home could do better: Care plans should be expanded to include information about a person`s capabilities and needs and about their previous lifestyle, including any interests. This would provide a holistic picture of the person being cared for. Any medication taken by residents should be recorded on a medication administration record sheet so that there is a clear record of the actual medication (including amounts) taken by each resident. Care Homes for Older People Page 3 of 9 What we found: We examined the records for a resident who had been admitted to the home since the last key inspection. The registered person told us that this resident had visited the home with a relative; they had visited other homes in the area but chose Hazemore because it was small and is operated within the home of the registered person. The registered person had completed a care needs assessment and this included a brief risk assessment. Information is obtained from care management when the local authority Social Services Department are involved in the placement, and this information is used along with the care needs assessment undertaken by the home to develop an individual plan of care. The current care plan is brief and we discussed how this could be further developed. For example, the care plan should include information about a persons capabilities and needs, their family history and their previous lifestyle, including any hobbies or interests. The registered person does have this information, as she is in close contact with the family and friends of each resident. However, if this information was included in the persons care plan, a full picture of the person concerned would be in place and would serve as a reminder of the information that should be written in daily records. We saw evidence that care plans are reviewed formally by Social Services care management staff when they have commissioned the placement. Medication currently prescribed to residents is recorded on their assessment form and any changes to this are recorded and dated. Any assistance with the administration of medication is recorded on daily record sheets. The registered person was advised that this information should be recorded on a medication administration record sheet so that there is a clear record of the medication taken by each resident. When as required medication is prescribed, a medication administration sheet allows for the actual amount of medication administered to be recorded. Medication is stored securely; each resident has a lockable medication cabinet in their bedroom. We saw evidence that mobility equipment had been obtained for residents and one resident had been assessed by an occupational therapist. GPs are contacted as and when needed; this was confirmed by residents on the day of the site visit and a GP visited whilst we were present. These visits are clearly recorded, including the reason for the visit and the outcome. We observed that information about medical appointments is retained and that any advice received following appointments or treatment is held in the persons care plan. Residents told us that they have many visits from family and friends. Each resident has a single bedroom and this enables them to see visitors in private. On the day of this site visit, one person remained in their own room and the other two residents sat in the lounge watching the TV. One person went out for a walk in the garden. Meal provision at the home is good; residents told us that they have good home cooking and baking and that they can choose something other than the meal on offer if they wish to do so, and that they can choose anything they like at teatime. The registered person told us that she often tries a different meal to see if residents like it; if they do, it is added to the menu. Care Homes for Older People Page 4 of 9 There is a complaints procedure in place and this is included in the homes statement of purpose. Each resident has a copy of this in their bedroom. There is a complaints book on display on the landing, close to accommodation used by residents. The Hull and East Riding Adult Protection Committee guidance is in use at the home. The registered person has accessed some training information from a private company and plans to undertake training on safeguarding adults from abuse shortly, along with the two care workers. Accommodation provided by the home is of a very high standard. The shower room has recently been refurbished and this provides good quality facilities for residents. There is a bathroom in addition to the shower room, so residents have a choice of whether to take a bath or a shower. All bedrooms are single and bedrooms and communal areas are well decorated and furnished; all furniture and fittings are domestic in nature. The residents have the use of a dining room/living room that has patio doors out on to a large balcony with views over open countryside. There is also a garden to the front and rear of the property. There is a stair lift to the first floor to assist residents that are not able to manage the stairs with access to and from their accommodation. No new staff have been appointed since the last key inspection. The main carer is the registered person; she is available at most times of the day over a 24 hour period, seven days per week. She is assisted by two part-time care workers, one of whom only works on an occasional basis. One of these care workers has achieved the National Vocational Qualification (NVQ) Level 2 in Care and the other is waiting to be enrolled on the NVQ Level 3 in Care award at a local college. The registered person has accessed some training information from a private company; we discussed the priorities needed for arranging these training sessions and agreed that safeguarding adults from abuse, infection control and nutrition should be the next courses for the three of them to attend. They have already attended food hygiene training via this company. The arrangments in place at the home for fire safety are satisfactory. An annual test of the fire alarm system is undertaken by an approved contractor and a monthly in-house fire drill takes place consistently.
