Hill House is a detached property sited approximately 2 miles from the town centre of Colchester in Essex. Hill House is currently registered to provide 20 residential care places for those who are 65 years and over. There are no further conditions of registration. Hill House does not offer nursing or dementia care. All rooms are single with en-suite, containing toilet and hand basin. There is a passenger lift to the second floor. At the rear of the property there is ample parking and a pleasant garden. All prospective residents are provided with a Statement of Purpose that incorporates the Service User Guide and supplies them with up to date information on the home. What the care home does well The service offers a tailored provision of care meeting individuals needs and choices at all times. People using the service reside in a very well maintained, homely environment, which feels like home. The service goes the extra mile to ensure that people feel safe, protected and happy. One person using the service stated, "I really like it here". The staff were observed administering medication in a person centred approach, ensuring that people received all their required medication as prescribed and in a way that they proffered to take their medication. Staff were directly observed supporting people and respecting their personal choices and preferences. People using the service can be assured that the staff are recruited safely ensuring all pre employment checks are made. Following this staff receive a sound, well structured induction programme and all mandatory training as required. What the care home could do better: Staff should access the Mental Capacity Act Training and Deprivation of Liberty`s and safeguards training. Care Homes for Older People Page 3 of 8 What we found: This was a random inspection completed by one person. The inspection took place over a period of three hours and reviewed a number of documents regarding people using the service, care plans, risk assessments, environmental issues, training, observation of staffing practices, talking to people using the service and their relatives as well as staff. In addition to this aspects of the services recruitment processes were checked. The service is registered to occupy twenty people. At the time of the inspection fifteen people were residing at Hill House. On arrival a short period of time was spent with the newly appointed deputy manager to explain the process of the inspection and discuss some aspects of the training. Training for staff working at Hill House is provided through an external sources and is in the form of DVD, lecture, handouts and an exam. Formal certificates are then provided for all staff who successful complete the course. Records showed that all mandatory training is up to date. Records showed that staff have recently received training in Safeguarding of adults, Dementia awareness, Infection control and Manual Handling. Staff have not received training in the Mental Capacity Act or Deprivation of Liberty and Safeguards. Staff have, however received information booklets regarding the act. Certificates of employer liability insurance were on display and up to date. The building is Grade II listed externally and presents extremely well, including a large, well maintained garden area with suitable cover for people to sit in the shade as they wish. One person using the service stated its really lovely here, we can come and go as we please, use the wonderful garden of sit in the conservatory. I think its very well maintained and always clean. During the inspection it was noted that the service has dotted around the building boxs of tissues, flowers and at lunch time the dinning room was laid with condiments and napkins, adding to the homely feel and a more personal service being offered. All bedrooms are personalised and people are supported and encouraged to bring in their own furniture and belongs to ensure that their personal space is reflective of their personalities and home. One person using the service stated I really like my room, especially with all my photos of my family. The service offers a library system, encouraging people to exchange books on a monthly basis. On the day of the inspection a small group of approximately six people were playing a name game in the dinning room area, with other people chatting and reading in the conservatory or sitting in the variety of seating areas available in the building. The service provides ten hours of direct activity work per week. The service is a member of the National Association for Providers of Activities for Older People (NAPA). Time was spent talking to the activities organiser, people using the service and a relative about the provision of activities. It was established that a recent trip to Frinton On Sea had occurred. One person suing the service stated it was a great day out, we visited this big house and had tea and cake in the garden, although I did not get to see the sea. The activities organiser commented that it was hoped to organise another trip soon to the sea side. Staff were directly observed to support people with total dignity and respect for their privacy and Independence. One person using the service stated its lovely here you know, the staff are so kind and friendly. The food is always lovely too. On the day of the inspection the food served looked and smelt appetising. Two choices were made Care Homes for Older People Page 4 of 8 available. Feedback from the dinging room was positive form all people. One relative commented, the staff are always so attentive, kind and caring. My X is fully supported here and I feel X is safe. Its a nice atmosphere too Two care plans were reviewed. Care plans determined the specific needs of an individual and stated what care and support a person may require in order to meet their needs and preferences. It was noted that the service strives to ensure that peoples difference and diversity is recorded and acted upon. Examples of this were for retiring and rising times, breakfast preference, name preferences and likes and dislikes. The care plan are reviewed monthly and detail any change in a persons needs. In addition to this the service also conducts nutritional assessments, monthly weight records, pressure sore risk analysis, and a basic risk assessment covering issues specific to the individual including wondering, falls, self harm, mobility, sensory, medical and night time risks. , Staff files reviewed evidenced that people are recruited and that satisfactory checks are made, including a criminal records bureau check prior to commencement of employment. The staff files also evidenced the services records for supervision and appraisal of staff. It was noted that the service provides supervision to individuals, however the frequency of these was sporadic. This was brought to the attention of the manager during feedback. The manager added that the service is reviewing the monitoring and completion of supervisions now and improvements will be made. It was noted that the service provides a sound induction process and records are maintained.
