What the care home does well It was not possible to establish whether the home was complying with The Care Homes Regulations and National Minimum Standards, as the service was not operational at the time of this inspection. Age range 18 years to 65 years Date of last inspection 27th July 2005 Brief Description of the Service: Kashgar Care Home was registered in March 2002 to provide personal care and accommodation for three adults with a learning disability. This inspection took place on 03.08.06 between 09:30am and 11:05am. Prior to the inspection the service file was examined to assess information that had been obtained and received about the home since the last inspection. The inspector spent time talking with the Registered Providers about progress with the previous requirement and recommendations and undertook a tour of the home. This home is currently unoccupied. A number of standards could not be assessed for this reason. The findings from this inspection are based solely on the information that was viewed during the inspection. Prospective service users did not have access to adequate written information about some aspects of the service. EVIDENCE: The inspector was told that no changes had been made to the homes Statement of Purpose or Service Users Guide. The previous requirement to include additional information in both documents was outstanding. The Registered Provider told the inspector that this work was in progress. See requirement 1. No service users had been admitted to the home since it opened in 2002. It was not possible to assess the key standards. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22 and 23. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home had procedures in place to respond to concerns and complaints. EVIDENCE: The homes complaints procedure was assessed in 2004 and was found to comply with The Care Homes Regulations. The inspector was advised that the procedure had not changed. The home had not received any complaints in the period since the last inspection. A copy of the local authority Adult Protection Procedure was kept in the home. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24 and 30. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. This home provides a clean and comfortable environment for service users and their visitors. EVIDENCE: The home was clean, tidy and comfortable. No outstanding maintenance issues were noted. The standard of décor was satisfactory and most of the furniture and fittings were new. All of the bedrooms were unoccupied, so it was not possible to assess whether there was adequate furniture and fittings to meet individual service users needs. The Registered Provider should assess whether service users would like additional furniture such as another comfortable chair prior to admission. It was not possible to assess the key standards. Service users are confident their views underpin all self-monitoring, review and development by the home. Service users’ rights and best interests are safeguarded by the home’s policies and procedures. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 37, 40, 42 and 43. (It was not possible to assess standard 39). Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The management team have the skills and experience required to manage a small care home for people with learning disabilities. The home had a policy and procedure file but some of the documents were not suitable for use in a small home. EVIDENCE: The Registered Providers were jointly responsible for the day-to-day management of the home. Both managers had experience of supporting people with a learning disability in a community setting and had a care qualification. The Registered Providers said that they were not able to commence the training recommended for managers whilst the home was empty, as the course requires them to reflect upon and provide evidence of work they have undertaken with service users in the home. Some of the information listed in the procedures manual was assessed during the previous inspection and did not appear to be relevant to a small home for younger adults. See recommendation 1. Some routine health and safety checks had not been undertaken because the home was not occupied. The Registered Providers said that all of the necessary checks would be undertaken prior to admitting service users. The public liability certificate was not available for inspection during this visit. The Registered Provider agreed to forward a copy of the current certificate to the inspector. See requirement 2. A local environmental health officer inspected the home in July 2005. Some verbal advice was provided regarding a window that did not have a restricted opening. The window was locked as advised. The Registered Persons should consider restricting the opening of the window in the first floor bathroom to 100mm. Although this home has been vacant for the past two years there was no evidence to suggest that the home was not financially viable. The Registered Persons are not reliant upon the home providing them with an income.
