What the care home does well The standard of care provided to people living in the home is good and demonstrates appropriate concern to maintain the dignity and respect of the individual receiving care and support. The manager and staff are provided with basic dementia care training and additional more detailed training as appropriate to enable them to provide care to people who live in the home with dementia in an appropriate way that reflects current good practice. The home is well- managed and the manager and deputy manager are well-supported by the responsible individual so as to ensure the quality of care is robustly monitored and action taken to improve care practice where that is shown as possible. This in turn benefits the people who live in the home and provides confidence for those responsible for them that they can raise any concerns they may have easily and openly. What the care home could do better: When we spoke to the responsible individual, the registered manager and the deputy manager they indicated that they were committed to continue to improve and develop the care service at Lime Tree Manor, with especial reference to the provision of the highest quality of care to people in the home who live with dementia, in line with current good practice. There are no requirements or recommendations made following this random inspection and as stated above the assessment is that Lime Tree Manor continues to achieve good care outcomes for the people who live there. Detailed information about the services offered, including the latest report by the Care Quality Commission (CQC) is displayed prominently in the reception area of the home. Fees for services range from £590 to £621 per week. Care Homes for Older People Page 2 of 8 Brief description of the care home Additional charges are made for newspapers, toiletries, chiropody and hairdressing services. This information was correct at 25.05.10 The home is owned by Wilton House Limited and is situated on the Adeyfield Road, a residential area of Hemel Hempstead, within easy reach of shops and facilities with good public transport links. The home is arranged over three floors in five units. Each unit has dining and lounge facilities, kitchenettes and assisted bathrooms and toilets. All the homes bedrooms are single accommodation with en-suite facilities. There is a passenger lift and the home has a reception area, benches to sit on at the front of the home and two enclosed garden areas to the rear accessible to residents. Parking is available to the front of the building. Care Homes for Older People Page 3 of 8 What we found: The last key inspection of Lime Tree Manor was carried out in June 2007, when it was assessed as achieving good care outcomes for the people who live there. This judgement was confirmed following the last annual service review of the service carried out by the Care Quality Commission (CQC) in October 2009. Under the revised methodology of the CQC and following a risk assessment process, this random inspection took place to confirm that there are currently no issues or concerns arising from any information received by the CQC to indicate that a full key inspection is required ahead of the registration application to be made by the service by the end of October 2010 under the provisions of the Health and Social care Act 2008. This random inspection took place over the course of one morning and was carried out by one compliance inspector. Where this report refers to we it recognises that it is written on behalf of the CQC. In carrying out this random inspection we have drawn on the information provided to us by Lime Tree Manor in their latest annual quality assurance assessment (AQAA). The AQAA is a self assessment document that sets out how well care outcomes are being achieved for people living in the home and also includes some useful statistical information. We have also taken account of any information received by the CQC about Lime Tree Manor since the last annual service review in October 2009. This might be from health or social care professionals associated with the home, people who live or work there or relatives and other people associated with the home in any way. This random inspection started early in the morning which provided an opportunity for us to observe the routine of the home as people were being helped to get up and ready for the day ahead. We found that there was a calm and unhurried feel to the home, with people being assisted to get up and ready for breakfast. We were able to observe choice being offered to people, for example as to where they ate their breakfast. Some people preferred to have their breakfast in their own room and others were being helped to dining rooms. It was observed that some people were still able to remain in bed and we were told that they can choose when they decide to get up and that breakfast would be provided for them when they chose to. The interaction we saw between people living in the home and care staff was good, with appropriate attention being given to the dignity of people living in the home by care staff. We saw that the homes manager and deputy manager were already going around the home, talking to people who live there and to staff. This informal supervision and monitoring of care is an important element of the quality assurance process for the home and enables easy and stress free regular access to the management by people living in the home. We found that the domestic staff were already in action throughout the home and at the time of our visit the home was clean and tidy and provided a pleasant environment for Care Homes for Older People Page 4 of 8 the people who live or work there. We were able to have an extended discussion with the responsible individual, registered manager and deputy manager about their current assessment of the home, the implications of the new registration process under the Health and Social Care Act 2008 and we also spent time discussing quality assurance, dementia care training and involvement of relatives and people living in the home in the decision making process. The assessment following this random inspection is that Lime Tree manor continues to achieve good care outcomes for the people who live there.
