What the care home does well Staff keep clear,detailed and accurate records of all aspects of the handling and use of medicines in the home. Further staff training together with improved procedures and systems mean that people can expect to receive their medicines correctly as prescribed with a reduced likelihood of mistakes. What the care home could do better: Although the service has a mission statement that provides basic details about it, the statement of purpose and service users` guide must to be updated to ensure people who are thinking about using the service have sufficient information about it to make an informed decision about it. Medicines requiring cool storage should be kept securely in a fridge intended and used exclusively for this purpose. Whilst we saw evidence the service was being appropriately managed and told the provider intended to make sure there is a registered manager for the service, a recommendation is made that appropriate action is taken about this to ensure the management and administration of the home can be maintained at all times. The home provides the primary stage for detoxification and therapy. The home provides care for men and women. Lynwode Park is within a housing estate on the outskirts of Barnsley. All bedrooms are single and each bedroom has an en-suite. All of the en-suites have washbasin and toilet; one of the en-suites has a shower. Shared bathroom and shower facilities are available close to the bedrooms. There is a Care Homes for Adults (18-65 years)
Page 2 of 8 Brief description of the care home dining and kitchen area where clients can make drinks and snacks. There is a lounge with T.V books and games. There is a designated smoke room. The local amenities include shops, chemist, library, church, and a bus route to town. Lynwode Park does not encourage clients to use the local amenities during their stay. This is because of the nature of the treatment and therapy. The home does make special arrangements for people to attend church if needed. Information provided by the manager on 8th August 2006 indicated the fees and charges were 2000 pounds per week. Additional charges include newspapers. Since the last Key Inspection of the home, we saw evidence that contracts issued to people using the service had been developed to make them easier to understand. As part of the assessment process for the service, people who are thinking about staying at Lynwode Park are referred to the homes counsellor and provided with a variety of information about it. The service has a mission statement which provides basic details about the nature of the service and the acting manager told us its statement of purpose and service users guide were in in the process of being updated. We examined the homes medication procedures and records including the current medication administration record (MAR) charts to check whether all medicines in the home had been recorded correctly and could be accounted for. We also reviewed and the storage & handling arrangements for medicines prescribed by the homes medical officer and for those brought in by people using the service. Medicines administration practices were not observed during this visit. All the medication procedures and recording documents have been revised by the management team and with some input from local NHS health care professionals. A newly introduced code of practice for the prescribing, custody and administration of medicines means that all staff know exactly what is expected of them. The new procedures and medication handling processes are much improved and appear to be followed consistently resulting in a reduction in the risk of mistakes being made. Complete details of all medicines prescribed and used in the home (including homely remedies) are recorded accurately and the administration records indicate that medicines are being given correctly as prescribed. A clear audit trail exists and staff make regular checks to ensure all medicines arriving in and leaving the home are accounted for. All medicines are supplied by a pharmacy and we saw that they were suitably and clearly labelled with instructions for their use and expiry dates. We found no evidence of medicines being given without appropriate medical consultation and authorisation. The homely remedy policy now includes the need to seek further directions from the medical officer and staff told us that this improved practice is much safer than before. Storage conditions for medicines have improved since the last inspection visit. Room temperatures and expiry dates are checked daily to ensure all medicines are safe to use when needed. The home does not yet have medication fridge, and although no medicines in use at the time of this visit needed cool storage, consideration should be given to purchasing a new fridge intended for this purpose. All staff involved in the handling and administration of medicines have received further training in the safe handling of medicines since the last inspection visit. We were shown evidence that staff are observed and checked to make sure their practice is in line with current professional guidance. Having well trained competent staff helps to reduce the risk of medication errors. We inspected the system for recording complaints about the service and saw evidence that a new form had been devised for recording issues raised, together with the Care Homes for Adults (18-65 years) Page 4 of 8 actions taken by the provider to resolve these as previously required.
