What did we do for this annual service review? We looked at all the information that we have received or asked for since the last key inspection. This included The annual quality assurance assessment AQAA that was sent to us by the service. The AQAA is a self assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. Information we have about how the service has managed any complaints. What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement. The previous key inspection and the results of any other visits that we have made to the service in the last 12 months and relevant information from other organisations. What has this told us about the service? The home sent us their annual quality assurance assessment AQAA when we asked for it. It was very clear and gave us all the information we asked for. We looked at the information in the AQAA and our judgement is that the home is still providing an excellent service. The AQAA told us that the person using the service has lived in the family home for fifteen years. The homes environment is domestic and offers a comfortable atmosphere where the individual has their own bedroom, shower room and a sensory relaxation room with free access to all areas of the house and garden. The home has no additional staffing and the registered persons have supported the individual over the fifteen years and through this time have continued to provide an environment which promotes the individual to lead a full life as far as possible with routines which are structured yet flexible. A comprehensive person centred care plan remains in place which identifies the individuals unique method of communication, their care and support needs, daily living and sleep patterns, dietary and eating and drinking care plans, the individuals behaviour, social interaction and emotional needs and documented risk assessments to support the individual to make appropriate decisions and take reasonable risks. The person centred plans and risk assessments are updated, changes made where necessary and annual reviews held. A range of domiciliary services including dentist, optician and speech and language therapist have visited the home since the last inspection and other specialist health care professionals visit regularly to remain involved in the care and support of the individual. Specialist advice from the CTPLD Team in relation to managing autistic Annual Service Review Page 3 of 6 spectrum behaviours has been sought with the registered providers receiving reassurance that they are proactive in seeking to provide the best possible environment and management of the individuals needs. The individuals equality and diversity is promoted and also includes their spiritual needs and being part of making decisions and having choices about their life on a daily basis. Support is available through a twenty four hour period and facilities remain available within the home for the individual to spend time taking part in activities that are relaxing and meaningful to them. The individual continues to be supported to be part of the family and maintain contact with other members of the family, keep contact with a range of health and other professionals and take part in being a member of the local community visiting neighbours, going out and having holidays. The AQAA advises that the registered providers have been told by professionals that to provide a family living setting supporting an individual with such profound difficulties is quite unique and that the individuals personal development and quality of life has far exceeded expectations than would have been possible in a standard residential care or hospital setting. Meals are prepared in a domestic manner, and the family have meals together. The AQAA advises that the individual is supported to make choices about what they like to eat and when they wish to have their meals. Snacks and drinks are available throughout the day. The AQAA advises that the individual receives a small amount of prescribed medication and medication administration records are completed clearly and accurately. Letters from the individuals consultant would be available to evidence any changes made to the individuals medication. The registered providers remain very aware of Safeguarding Adults Procedures and have a copy of the local authority safeguarding protocols. They have continued to be actively involved in raising the profile of safeguarding vulnerable adults including the Deprivation of Liberty Safeguards legislation in order to promote peoples rights to protection and well being. The AQAA advises that no complaints or safeguarding referrals have been made or received since the last inspection. The registered providers are the main carers for the individual and have extensive experience of working in social care. No staff have been or will be recruited to the home. The registered providers have achieved their NVQ Level 2 and 3 in care and have arranged a refresher training day to update themselves on core practices. CRB disclosures have been achieved and there have not been any concerns about the way in which the home is run. Feedback about the home is obtained through the range of health professionals who regularly visit the home and by the individual in their daily life. Surveys were sent to the home for distribution by the registred persons yet none were returned to the commision by the date set. The comments received following this annual service review may be kept to inform part of the next key inspection. The AQAA
Annual Service Review Page 4 of 6 advises that gas and fire safety checks have been completed as required and the fire officer has assessed the house in respect of the Nov 2006 regulations and has concluded that as a family home it falls outside of the regulatory framework. The day to day management of the home has been robustly maintained and the home continues to let the commission know about things that have happened since the last key inspection and they have shown that they have managed issues well. What are we going to do as a result of this annual service review? We are not going to change our inspection plan, and will do a key inspection by the 30th June 2010. However we can inspect the service at any time if we have concerns about the quality of the service or the safety of the individual using the service.
