Milton Lodge is a long established care home catering for older people including those with dementia. It is a traditionally built detached house, with gardens to front and rear. Car parking is unrestricted on the road outside the home. Resident accommodation is on the ground and first floor; a lounge, sun lounge and dining room are on the ground floor. 22009 A stair lift is installed on the longest flight of steps on the main staircase but it is then necessary to negotiate an additional flight of stairs to gain access to the first floor and to reach some bedrooms it is necessary to descend additional steps; residents whose bedrooms are upstairs must therefore be able to independently manage steps. All bedrooms have a wash hand basin; one has separate en suite hygiene facilities and all are close to a toilet. With the exception of one bedroom for shared use by 2 persons, all bedrooms are for single occupancy. There is one bathroom for use by persons requiring assistance; the bath is fitted with an elevating bath seat but the bathroom is not large so suited only for use by persons who require low levels of assistance. Fees are charged weekly; the fee range quoted by the manager at the time of inspection was (per person) from £500 to £570. Up to date information on fees can be obtained from the service.

What the care home does well The storage of the medicines has improved since the last inspection. Staff have the opportunity to undertake a comprehensive training package. The home has clear records of all interventions by healthcare professionals. What the care home could do better: The storage of controlled drugs must be in line with the current legislation. Regular checks on medicines and reagents must be undertaken in order to be sure that no out of date stock is held or used. Care Homes for Older People Page 3 of 7 What we found: Our pharmacist inspector undertook this inspection to follow up on concerns about the safe handling of medicines which were raised at a previous inspection. The medicines were all stored securely and a new fridge had been purchased for the storage of medication that required cold temperatures. The controlled drug cupboard had been secured to the wall but not in the manner that is required by the current legislation. Since the last inspection efforts had been made to improve the clarity of the medication administration records. The instructions for the use of creams and lotions were now clearly marked and these additions to the charts had been signed and checked by two people. However a person who was new to the home had a written medication administration sheet that had not been checked and countersigned. Records were seen where staff had indicated the exact dose of medication that had been given where a dose of one or two tablets had been prescribed. Staff made comprehensive records of visits from the GP and any changes to medication that were ordered, in peoples records. In one case the instructions for the use of a medication prescribed as required was not available as separate guidelines for staff to follow. During the inspection the information from the GP was put into a protocol by the manager. We found some urine testing strips that were out of date and not suitable for use, these were removed during the inspection. All staff who administer medication have been recently enrolled on a training course for the safe handling of medicines taught by the local Primary Care Trust. We observed staff giving the lunchtime medicines in the dining room and this was done in accordance with the homes procedures and safe practice guidance. This means that the shower hose must be prevented from being placed in the bath, damaged and dirty bed rail covers must not be used and throughout the home adequate standards of hygiene must be maintained and good hygiene practise promoted.
