If yes, what have they been: Date of last key inspection: Date of last annual service review (if applicable): Brief description of the service The Old Village School Nursing Home is a 60 bedded home which comprises of three units, two of these are located in a building attached to the original home by a partially covered corridor. There are plans for alterations to join the two buildings, and in the process completely cover the walk way, and reduce the number of shared rooms in the older building. The two units in the newer building provide care for younger adults with physical disabilities and complex healthcare needs, and the unit in the original converted schoolhouse accommodates older adults with nursing care needs. The newer building is purpose built and the accommodation is over two floors in separate units with passenger lift access. The rooms are all single occupancy and they all have en suites. The accommodation in the schoolhouse is all on the ground floor. There is one shared occupancy rooms with screening, and three rooms with en suite facilities. The home is in Marston Mortaine, which is situated between Bedford and Milton Keynes with good road links to both towns and easy access to the M1 motorway. There are

What did we do for this annual service review? We carried out this review of the service on 8th of January 2010, approximately one year after the last key inspection. At that time the home was rated as a 3 star/excellent service. We looked at all the information that we have received, or asked for, since the last key inspection. This included: 1) The annual quality assurance assessment (AQAA) that was sent to us by the service. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. 2) Surveys returned to us by people using the service and from other people with an interest in the service. 3) Information we have about how the service has managed any complaints. 4) What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement. 5) The previous key inspection and the results of any other visits that we have made to the service in the last 12 months. 6) Relevant information from other organisations. 7) What other people have told us about the service.. What has this told us about the service? The home sent us their annual quality assurance assessment (AQAA) on the 30/10/09 when we asked for it. It was clear and gave us all the information we asked for. We looked at the information in the AQAA and our judgment is that the home is still providing an excellent service and that they know what further improvements they need to make. It advised us that all policies and procedures had been reviewed within the last 12 months, and audit processes within the home are working efficiently and effectively. We received 25 completed questionnaires, 14 from residents and 11 from staff. Comments recieved were generally very positive, such as: It supports you and helps you with your everyday life and the food is very good. The carers and nursing staff are very efficient in their work. Annual Service Review Page 4 of 8 The Home looks after us well. Im very happy, home from home. Support for staff, training opportunities NVQ 2, 3 AND 4, and flexible approach to staffing issues There was however one resident that felt they were not involved sufficiently in decision making, relating to their daily routine, and four residents stated that they would like a covered area for people to go to if they wanted to smoke, as this is not permissable in the building and there is no external designated area at present. The manager has advised us. we have applied for planning permission to erect a structure for this purpose and are currently awaiting approval from the planning office before we can proceed. We are confident these issues are being addressed by the manager. At the time the AQAA was completed there were 56 people living and receiving care at The Old Village School Nursing Home and they were supported by approximately 55 regular qualified nurses and care staff and a team of 23 non care staff. The staff are from a variety of ethnic backgrounds thus enabling a greater understanding of the different cultural needs of the residents in this home. In the AQAA the manager told us in the summary: 1. What we do well: We work upon a person centred approach where the Service User is encouraged to participate in choosing their pathways of care. We have an honest and open approach to all aspects relating to the Home where any concerns can be openly discussed by both staff and Service Users and the appropriate action taken. Relatives are also encouraged to participate in all aspects of their relatives care and progress. A programme of maintenance is in place to ensure that the Homes interior and exterior are effectively maintained ensuring pleasant and comfortable surroundings for Service Users to reside and Service Users are actively encouraged to become involved in refurbishment projects. Regular Residents forums are arranged and overseen by our Activities Officers to ascertain any matters that they would like to be brought to the attention of the management. Staff recruitment procedures are consistent and all new employees receive in depth induction training. Regular supervision and staff appraisals are carried out for all staff. Our staffing levels are high due to the high dependency of our patients but contain a skill mix of employees appropriate for our client group throughout the 24 hours, 7 day

We have a large number of overseas staff who we endeavour to encourage to integrate into the team and do this by arranging entertainment evenings and outings for meals out, etc. Care is provided by well trained staff who promote Residents safety, dignity and independence at all times. Policies and procedures are in place to manage complaints and incidents effectively. We arrange regular outside entertainers for both our young adult and elderly residents which encourages them to integrate and participate. We provide a good intermediate care service for rehabilitation as we have a wellequipped physiotherapy department and competent physiotherapy assistants. We work well in a multi-disciplinary team where other professionals from the community are welcomed into the Home so we can provide holistic care for our Residents. 2. What we have improved in the last 12 months: We have appointed new senior staff. We have appointed an assistant Training Officer. We have appointed a Senior Nurse for one of our Units. We have promoted our Training Officer to Deputy Manager/Training Manager. By doing this we hope to improve training, communication and leadership and, therefore, the care of our Residents. We have started the building works to join the buildings, build new offices and a conference/training room. No statutory requirements were made in our last Inspection Report but we feel that we have continued to improve our training practices to ensure that the level of care and expertise offered to our client group is of the highest standard. 3. What we could do better, and how we are going to do this: We are continuing to work with all members of staff to increase their knowledge of documentation practices. We feel that this is a core requirement in our efforts to achieve high standards of documentation practices. We will continue to train employees in all relevant government legislation relating to adult protection and the Mental Capacity Act and Deprivation of Liberties to ensure that our staff are sufficiently qualified and knowledgeable to deal with our client group. We will continue in our refurbishment and redecoration programme to meet our
Annual Service Review Page 6 of 8 maintenance targets. We will continue to improve communication with the help of our new management structure. The home continues to let us know about things that have happened since our last key inspection, by submitting notifications as required, and they have shown that they have managed issues well. They work well with us and have shown us that their service continues to provide excellent outcomes for the people who use it. What are we going to do as a result of this annual service review? We are not going to change our inspection plan, as this is an excellent service will do another ASR by 8th of January 2011. However we can do an inspection at any time.
