What the care home does well People living in this home receive sensitive, person centred, individual and detailed assessments. People are encouraged and supported to make choices about what they wish to do and contribute to the running of the home through the weekly resident`s meetings. They live in comfortable accommodation safe accommodation. The staff help and encourage the people to gain skills whilst, at the same time, making sure that they are safe. The people are encouraged to try a range of courses, which help them gain independenceand confidence. There are plenty of leisure activities and outings for them to enjoy. They are also offered choices about what they wish to do and to make decisions about how they spend their lives. They are encouraged and supported to have control over their lives. The people like the way the home is run, and enjoy staying at the home. People living in the home are cared for and supported by a caring, educated and skilled team of staff. They are lead by an experienced manager who has extensive knowledge about the needs of people with a learning disability. There is a comprehensive programme of education and training provided for staff, which ensures that staff know how to care and support the people who live at the home. The staff feel valued by the management and feel part of a team to improve, to help and support the people living in the home. What the care home could do better: Linkage have very well established systems in place to monitor quality and seeking the views of the people. Regular detailed annual questionnaires are sent out to obtain their views of the service. Where improvements are required they are either planned or being addressed by Linkage or the manager and staff within the home. Date of last inspection Brief description of the care home Scremby Grange is a large period property registered to provide personal care for 16 people with learning disabilities. It is situated just outside the small village of Scremby on the A158 towards Skegness. It is approximately 3 miles from the market town of Care Homes for Adults (18-65 years) Page 2 of 9 Brief description of the care home Spilsby and directly on a bus route. The home is surrounded by extensive grounds and within these are a day service and the offices of Linkage Community Trust who provide the service. This home is a long stay home as long as assessed needs can be met and there are opportunities to move to more independent living if this is appropriate. All accommodation is provided in single rooms. During the week the people living in the home attend a variety of off-site activities ranging from vocational interests to work placements. The weekly fees at the time of our inspection visit ranged from £509 to £1,000,54p. Information about the day-to-day operation of the home is explained and given to each person and is available in the home for others to refer to. The statement of purpose, service users guide and a copy of the last inspection report can be obtained from the manager of the home. The last key inspection took place on the 14/6/2007. Care Homes for Adults (18-65 years) Page 3 of 9 What we found: This was an unannounced visit which formed a random inspection visit over 3 hours and took into account previous information held by us including the previous inspection report, their service history and records of any incidents that we had been notified of since the last inspection. During this visit we talked to the people to obtain their views about whether they felt their needs were being met and about their quality of life. We also spoke to staff to learn their views about how they felt their training and development needs were being met. We also spoke with the manager to learn of any changes which have taken place and if there were any risks to the people living in the home. An (AQAA) Annual quality assurance assessment was completed by the manager and sent to us prior to this report being completed. This is a self assessment document completed by the manager of the care home. It sets out evidence from the provider to demonstrate that they are meeting the Care Home Regulations 2001. The information was very clear, up to date and detailed. We were told that over the last year they have involved the people more in recruitment of staff and have a representative on the self advocacy group. They have offered person centred plans to those who had previously declined and ensure that those with plans are active and updated. They have continued to make care plans more person centred. They have enabled and supported one person to travel independently to his family on public transport and continue to encourage the people to take risks to enable them to experience learning opportunities in and outside the home. We saw there were 4 individual bungalows in the grounds housing 4 people. All of these people had some one to one support at various times. The manager told us that they have made the job coach a permanent position and this person is employed to support people when they go to work and keep track of their progress. This has enabled 3 people to have work experience. We spoke to 5 people who use the service, three members of staff and the manager. We also spoke with the care services manager and deputy care services manager for Linkage who were present for part of our visit. Feedback was given about the continued positive outcomes at the end of the visit to the manager. On the day of our visit the people were having a taster week and were attending activities including ceramics, woodworking, painting, art, gardening and sports health and fitness. Some were at Scremby Grange others were off site. This allowed them an opportunity of choosing what they wished to do. Other people were going to supported work or out for the day. Everyone we spoke with was positive about the home and the staff. Comments included, I like it here, we have a good time, I can have a laugh with the staff and its great here. We spoke with 3 staff who told us, when I first came here I received a thorough induction which prepared and gave me the skills and knowledge to care and support these people, since coming here my outlook on life has changed, I feel that I have achieved something when I go home at the end of the day, the training and support is Care Homes for Adults (18-65 years) Page 4 of 9 very thorough, we get a lot of training and support and we work well as a team and the manager is very supportive, helpful and approachable. We saw during our visit all staff working with one another and the relaxing and friendly atmosphere throughout the home. We saw the home was well maintained, relaxing, clean, odour free and an attractive place to work and live in. There were comfortable seating areas throughout the home and the dining room was a very attractive place to dine. The surrounding garden areas were attractive with flower borders and hanging baskets and were well maintained. We saw throughout our visit that the people were being treated in a dignified manner, staff spoke to them in a kind and caring way that was appropriate to the individual. We saw that over the past year they had received 6 complaints all of which had been investigated by Linkage using their complaints procedure. In addition they had made 3 safeguarding adults referrals to Lincolnshire County Council all of which had been thoroughly investigated. We had received no complaints about the home. Staff we spoke with told that us they were correctly recruited with a check by the Criminal Records Bureau and a supported comprehensive induction. We saw that staff had continued to receive a lot of training over the last year. This had included, moving and handling, fire safety, safeguarding adults, diabetes, epilepsy, medication administration, non violent crisis intervention, understanding challenging behaviour, equality and diversity, total communication and principles of care. In addition, staff had obtained or were studying for a qualification in care to national vocational qualifications standards. All staff we spoke with told us they received regular one to one supervision. We saw that monthly monitoring visits took place and reports of these visits were available with action plans where there were any concerns. Linkage have very well established systems in place to monitor quality and seeking the views of the people. Regular detailed annual questionnaires are sent out to obtain their views of the service. We looked at the results of the last survey in 2010. These were analysed and showed that the majority of comments were very positive about their experience in the home. The views of the people are also obtained through the monthly meetings. There is also a formal quality auditing system European Foundation for Quality Management (EQFM) which uses surveys from people receiving care and support and staff to obtain their views about the service and there is a Pointers committee. It is a committee, which is run by and for the people who use the organisations wider services. We were told that one of the people living in the home was on the committee. This group enable the people receiving services to influence the development of the service.
