St Leonards Court is a care home providing personal care and accommodation for up to 25 older people who have a dementia. The home is situated in the village of Mundford, close to the town of Thetford. St Leonards Court is located on the village green and is in the centre of the village. Accommodation consists of 19 single and 3 shared bedrooms. All bedrooms have ensuite facilities. Bedrooms are located on the ground and first floors. There is level access throughout the home and a shaft lift is installed for access between floors. There is ample communal space, including 2 lounges, a period reminiscence lounge and dining room on the ground floor. There is a small sitting area on the first floor. The home has an enclosed rear sensory garden, which is easily accessed by service users. The garden has a ramped pathway and lawns, together with sitting areas and appropriate garden furniture. Annual Service Review Page 3 of 7 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked all the information we had received, or asked for, since the last Key Inspection carried out in March 2009. This included: An Annual Quality Assurance Assessment (AQAA) form. The AQAA is a self-assessment completed by the Provider, which focuses on how well the Provider considers they are meeting the outcomes of people using the service. It also provides information about the service and how they may be intending to improve over the next 12 months. Information from this document has been used in this report where possible. We also looked at information received by the CQC since the last inspection in the form of Notifications. These forms are required to be submitted to the CQC to advise of any issues/incidents that may have happened within the home that may affect the residents welfare. Questionnaires were sent to the service to distribute between residents, relatives and staff. At the time of writing the report 7 service users, 5 relatives and 4 staff questionnaires had been returned. Where possible information and comments from these questionnaires have been included in this report. What has this told us about the service? This review has not changed out opinion of the service and it is concluded that St Leonard Court is still providing positive, good outcomes for people who use it. There were no events that would trigger an early full inspection of the service and the home continued to provide appropriate care to service users. Changes that affected service users were reported in a timely fashion to us, the regulator. A few accidents had been reported, together with the homes plan how for to prevent similar events happening again and the actions taken following each individual incident. The home accommodated 24 service users, all suffering from dementia, at the time of their AQAA submission, in February 2010. Twenty three carers were looking after the residents. Progress of the home was presented in the returned AQAA, (Annual Quality Assurance Assessment) and included: We provide a service which ensures that our residents live in a safe, homely environment. The communal areas of the home are comfortable and are designed to support people with dementia care needs. We ensure that all our residents are treated as individuals and we respect their need to be as independent as possible. Our staff team treat our residents with dignity and respect and we ensure that as far as possible their privacy is upheld. Our staff are caring, kind and show great empathy towards our elderly residents all of whom have dementia. The home has an excellent relationship with the community nursing team and works with them to ensure that end of life care is provided with sensitivity and care. We strive to provide food which is of a high standard and encourages good nutrition. Individual residents needs are considered when planning menus. We aim to provide activities and outings which provide stimulation and enjoyment for our residents. We provide person centred care for our residents and ensure the daily routine of the Annual Service Review Page 4 of 7 home is flexible to allow residents as much independence and choice as possible. The home carried out their own analysis of the achieved standards through their quality assurance surveys and reported: In our Annual Quality Assurance survey results 88 of residents responses stated that we met their expectations and 86 of relatives responses stated that the service we provide met their expectations with l4 stating we have exceeded their expectations. From our own surveys and questionnaires some comments made by the users of the service include: Everything is to my liking. They do well at looking after us. They make us happy. Everything is very good and if I want something I only have to ask for it. I have no complaints at all. The relatives responded in the same, positive way: The care home has a friendly and professional attitude. The managers are always available to speak to and are often hands-on when necessary. I have always found them to be caring, considerable and thoughtful of the residents and relatives. I have a great faith in the manager and care team to look after my aunt, they do a brilliant job and I have experienced several care homes and found that St Leonard is by far the best. The staff and the management are all angels! Staff also felt well supported and motivated to do their job. Some of them praised the training, some addressed the outcomes for service users, but all comments provided indicated that the atmosphere was very good for residents and the staff too: St Leonards is a good happy environment to work in. Happy staff means happy home and happy residents. Care planning was appropriate and the home made progress in this area and the AQAA explained: We ensure that all residents have an individual plan of care, which identifies their health care needs, personal needs and preferences and social care needs. These care plans have been expanded and include more detailed risk assessments, these include falls, water-flow, mental capacity, moving and handling, self administration of medications, they are much more person centred. Care plans are based initially on the pre admission assessment, input from GP, social workers and community nurses where relevant, information from the resident, where appropriate and information given by families. We use the Liverpool Care Pathway to support end of life care and have an excellent relationship with the community nursing team who enable us to provide all necessary care. The provider reported proudly: We have developed new policies and procedures for dealing with medication to include homely remedies and covert medication administration. We have a Best Practice Medication Guide which staff can access for information. We have good support and advice from the pharmacy with whom the manager has developed a positive relationship. We audit medication on a regular basis. We use a nutritional risk assessment to identify residents who may need monitoring and more assistance to ensure they have a good nutritional intake, this also identifies those residents who need a different approach with their food intake ie finger food. The example illustrating homely approach to care and provisions for service users was evidenced in AQAA: We ensure that as far as possible the lifestyle of the home matches the individual needs and preferences of the residents. We use information from residents, their families and their life histories to generate a wide range of appropriate activities within the home. We have an activities co-coordinator, who along with the manager and staff team plans activities, outings and entertainment for the residents taking into account their likes Annual Service Review Page 5 of 7 and dislikes and varied interests. There is a trip for the residents at least monthly relatives are always invited along and regularly join in. With no significant changes in outcomes for service users, apart from the mentioned improvements, and no changes regarding the management, the home was considered to continue to meet the needs of service users and complies with the standards. What are we going to do as a result of this annual service review? There will be no change to the proposed inspection plan. However, the Commission may choose to carryout an inspection at any time, if information received indicates that the service no longer promotes the safety and welfare of the people who use it.
