0Sycamore Lodge is situated in the Ashby area of Scunthorpe, it is close to the local amenities and on a number of bus routes. The home provides care including nursing for up to 40 service users over the age of 65. Accommodation is provided on two floors; there is stair and passenger lift access to the first floor. There are thirty two single rooms and four shared rooms; en- suite facilities are provided in nine of the single rooms. The home has a good variety of day space; there are two lounges, a dining room and a conservatory. All rooms are decorated and furnished to a good standard. The home has a pleasant, homely inclusive atmosphere. The home is owned by Mr S Marjara. The registered manager is Mrs M Williams. What the care home does well The home is bright , clean and welcoming. It has homely touches in the shared communal rooms and people have personalised their bedrooms. People said that there are no restrictions put upon them in respect of how they live their lives. One gentleman said he chooses when to go to bed and when to get up and chooses from a range of choices at mealtimes. People are very positive about the level of care and support they receive at Sycamore Lodge and made comments such as; `The staff are marvellous` and `The care here is very good, I can`t fault it`. The atmosphere at the home is warm and friendly and visitors are made to feel welcome.The staff show respect to people living at the home and were observed being kind and courteous throughout the visit. The management consult regularly with the people who live in the home so they can have a say in how the home is run.Staff and people who use the service said that they found the manager very supportive, approachable and that she took action if issues are raised. What the care home could do better: Although there is good evidence that the staff provide individualised care, the care plans should be developed to include more information about an individual`s choices and goals which will support a more person centred approach. Staff should ensure that they have a consistent approach to people`s moving and handling needs to ensure people`s safety and comfort. Care Homes for Older People Page 2 of 9 Care Homes for Older People Page 3 of 9 What we found: There was evidence in peoples care records to confirm that their needs were properly and thoroughly assessed prior to being offered a place at the home, the home obtains care plans and assessments from placing authorities to support this process. Peoples care plans are detailed and outline the level of support and care each person requires in ensuring their health and welfare needs are being met and reviewed. Discussions were held with the manager about how they should add more detail about peoples preferences within their lifestyles to make the care plans more person centred, although staff are currently working with people and their relatives to produce life history records and this information is beneficial in terms of getting an insight into the kind of hobbies, leisure interests and significant events that make up a persons life. The manager told us that in recent weeks she had identified some gaps in the completion of daily records, there was evidence that action had been taken and improvements made to the frequency of recording. Peoples health and personal care support is generally well documented however staff should try to record more detail about how people have spent their day and what they have enjoyed doing. During the visit some inconsistencies with moving and handling practices for one individual were observed, this matter was discussed with the manager who confirmed that she would follow the issue up to ensure that the persons care plan and risk assessments were up to date and clearly identify the level of support needed and that staff practices would be monitored, which will better protect peoples safety. All people spoken to confirmed their satisfaction with the care and services provided by the home. Comments from individuals and their relatives included The staff are excellent and I and very pleased with the care and support my wife receives, she is settling into the home really well. During the visit, systems for the storage and administration of medications were checked, this showed that safe systems are in place. The home has recently changed the pharmacy provider and the manager said that the new systems were much improved. Improvements were seen in the variety and amount of social support people receive. People spoken to said how much they enjoy the activities, entertainment and trips out arranged by the home and there was lots of evidence to support the positive rapport the activity co-ordinator has developed with the people who use the service and their relatives. Observation during lunch showed that people enjoy their meal in pleasant surroundings and at times convenient to them; the meal was unhurried and staff had time to show people what was available and to provide support when needed. The menus have been reviewed more regularly, people said that they have opportunities to discuss menu changes and make personal requests. If people have concerns with their care, they or people close to them know how to complain. People told us that the staff and management are very approachable. The home has received one complaint since the previous inspection, records were in place to support the investigation findings, outcome and follow up action taken by the management. Care Homes for Older People Page 4 of 9 The home continues to provide people with a good standard of facilities, all areas seen were very clean and well maintained.Building work has commenced on a new extension which will provide more single accommodation and reduce the number of double rooms available. The atmosphere on the day of the visit was warm and friendly and people looked comfortable whilst sitting in various parts of the home. There were sufficient numbers of staff on duty at the time of this inspection to meet the needs of the people living in the home. Discussions with people, relatives and staff confirmed that they considered the staffing levels were satisfactory.One person said Staff are always available when I need them. Records in care files show that dependency levels are reviewed each month. There was evidence that the manager was currently carrying out the staff appraisals and that the training needs identified were being included in the new training programme. The manager said that some staff were still reluctant to access courses other than the mandatory ones and this is being followed up through the appraisal and supervision programmes. Information provided by the home prior to the visit showed that the home has now achieved the target for having 50 of the staff qualified at NVQ level 2 which means care staff will have more knowledge and skills in providing care for people. Checks on staff files show that the recruitment procedures in place promote the protection of people. People, their representatives and staff spoken to during the visit consider that the home is well managed. The management have further developed and embedded the quality assurance programme which better demonstrates how the home provides a quality service and is listening to peoples views and opinions. The homes policies and procedures have been reviewed and updated which promotes the health, safety and welfare of the people who use the service and the staff. Checks of risk assessment and safety check records show that improvements have been made to the management of bed rail provision in the home which will better promote peoples safety in the home.
