Tanners is a purpose built bungalow that was constructed to meet the needs of seven service users who have learning disabilities and associated physical disabilities. The home, which is operated by Life Opportunities Trust, has seven single-occupancy bedrooms, a large communal lounge that adjoins the dining room, a kitchen, laundry room, two bathrooms with toilets and a separate toilet. One of the bathrooms has a separate shower. Bath hoists are provided. The bungalow is detached and is surrounded by gardens that are mainly laid to lawn with surrounding shrubs and a patio area. Off road parking for visitors and the home`s mini-bus is provided. Tanner is located on a private housing development and is situated at the end of a cul-de-sac. It blends in well with the neighbouring properties. There is a parade of shops nearby as well as a country park and pub. The village of Abbotts Langley is relatively close and there is a local bus stop from which connections to Watford and the local neighbourhood can be made. What the care home does well This service provides a diverse range of support and care to people with both a learningdisability and physical disability. There are various systems in place, which reflect both professionalism and expertise in working practices. These were observed as appropriate to the needs of the people using this service. The service has produced detailed care plans, which give a detailed insight into to the needs of the individual. The people receiving care services at Tanners appear to have some degree of involvement in their care planning which have helped produce a more person centered approach to care planning. However the manager must ensure that all care plans are signed by either the service user or their representative. Tanners provides detailed information about the operation of the service to prospective and current residents. The staff team are both committed and enthusiastic in their approach to people living within this service. Bedrooms and communal areas have been redecorated and re-carpeted and provide a well maintained and well presented private space for individuals which promotes people`s dignity and provides an acceptable level of comfort and individuality. The staff spoken to during the inspection appeared to have a clear understanding of their individual roles and responsibilities. The service has clearly defined job descriptions. Staff have received a series of mandatory training in order to carry out their roles effectively and professionally. The registered manager is competent and experienced in the field of learning disabilities and has created a welcoming and inclusive environment for people to both service users and staff. The manner throughout this inspection process was very professional and open, which made this inspection experience a very positive one, for everyone involved. What the care home could do better: The manager must ensure that all care plans and individual contracts are signed by either the service user or their representative in order to endorse their involvement in the care planning process. There must be an up to date record of the regulation 26 visits that are carried out by the Proprietor as the most recent report available on the day of this inspection was January 2010. Care Homes for Adults (18-65 years) Page 3 of 9 What we found: This inspection was carried out by one inspector over one day and planned at a time to ensure that there would be an opportunity to speak with some of the people who use this service. This service last had a Key inspection in 2007 when it was given a three star rating. The most recent Annual Service Review was carried out on the 9th April 2009 and a further Key Inspection carried out on the 9th April 2010. Five of the people who live at Tanners were at home at the time of this inspection along with one senior member of staff and three support staff, when we arrived. Their home was warm and welcoming. There was a calm atmosphere and people were relaxed and in good humour. We looked at a variety of pictures displayed throughout the home taken in the summer and at a variety of community events. Everyone looked as though they were having an enjoyable time. The residents we met said they were well. One residents bedroom has been redecorated and new furniture provided since our last visit, creating a fresher, brighter and more comfortable place to be. There have also been improvements made to both the main driveway and the pathways in order to make them more accessible for wheelchair users, there has been new communal furniture purchased, including a new sofa in the lounge, new curtains have been purchased and new lighting has been fitted to the dining room, lounge and kitchen areas. The current staffing arrangements are a minimum of three staff on duty at all times and two waking night care per night. Staff spoken to stated that all staff working at the home are very flexible and cover sickness and annual leave between them, with very little bank staff usage. Details of the recruitment checks were not available as the manager was not on duty and therefore all confidential information relating to the employment of staff was locked away. However the completed AQAA confirmed that all recruitment and selection procedures are adhered to and all staff receive CRB check before commencing employment at the home. Staff training records confirmed that all staff have received mandatory training within the last year as well as three staff having completed the Mental Capacity Act training on the 12/02/2010. Four staff have also completed DOLS training also on the 12/02/2010. Other courses for staff are being arranged during 2010 and we would advise there is a focus on person centered care and service user involvement to make sure staff are up to date with current approaches in these areas as some of the care plans do not follow the person centered approach and have not been signed by the service user or their representative. We were shown a variety of examples of the homes quality assurance systems which ensure that people using the service and their representatives receive feedback on the outcome of the annual development plan. Surveys had been sent out and reviewed. We saw a statement about the quality processes within the service. There are also new infection control standards in place that have been devised with the help of the Health Protection Agency and include both daily and weekly checks carried out by all staff working at the home. We were shown evidence of the fire records that are carried out and these included, the last weekly fire check on the 3/04/2010, the last fire drill was Care Homes for Adults (18-65 years) Page 4 of 9 carried out on the 27/03/2010. The last service was carried out on the 26/02/2010. Service users are encouraged and supported to attend weekly residents meetings and evidence was seen of the last meeting held on the 8/03/10. Staff spoken to confirmed that they receive regular supervision from a senior member of staff and feel that the current management structure is both supportive and approachable. There is currently one senior team leader vacant and a few care support staff vacant hours, which are being filled by the existing staff team. There has been a new manager appointed since the last Key Inspection was carried out and they completed their Registered Managers Award on the 9/01/209.
