The Cottage Wing was registered as a new service in October 2007. The home is within Barton Health and Community Centre, which provides facilities for General Practitioners and Health and Care Workers. There are also private apartments, where people are supported by Shaw Care Domiciliary Services. The Service was developed to replace the former Cottage Hospital in Barton under Needwood. The Cottage hospital has now been closed. The home is built on the ground floor with two distinct units linked by a through corridor. The unit is self-contained with access restricted to other parts of the building. There are nine large single bedrooms off the main corridor with full en-suite facilities. The bedrooms have been furnished to a high standard and include a personal television and lockable facilities. These rooms are to provide intermediate care and palliative care to people from the community. Along the bottom corridor there are three well-appointed en-suite rooms that have been designed to meet the needs of people with a physical disability, including a fixed track hoist in the bedroom and bathroom. These rooms are to provide respite care for people with a diagnosed neurophysiological condition. There are two assisted bathrooms and separate toilets available for visitors. People receiving a service move into the home from the local hospitals, or from the community. The service liaises with health care professionals to ensure suitable packages of care are available before leaving the home. The Service User Guide records that South Staffordshire Primary Care Trust fully funds all of the 12 registered beds; the home does not enter into private funding arrangments.

Annual Service Review Page 3 of 7 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last key inspection. This included: The annual quality assurance assessment (AQAA) that was sent to us by the registered person on 12/05/09. The AQAA is a self-assessment tool, which focuses on how well outcomes are being met for people using the service. It also gave us some statistical information about the service. Surveys were sent to the home to distribute to people who use the service, staff and professionals. We received five surveys from people who use the service, three social and health care professionals and five staff. This gave us information about how the service is managed and whether people using the service are satisfied. What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement. The previous key inspection report. Information we have about how the service has managed any complaints. The quality rating for this service was made during the last key inspection on 21 April 2008. What has this told us about the service? The registered person completed and returned the AQAA within the required timescales and it provided some good information about the outcomes for the people using the service. They told us that they foster an enabling culture to ensure that all people can become as independent as they can prior to discharge from the surface. Most people can return home within six weeks, and individuals are able to continue to receive support from the Enablement team for six weeks after discharge. People who use the service for palliative care may on occasions stay longer. People who require respite care within the service can book two weeks of care. They told us there is a very high demand for this service and they are currently reviewing how this is provided to ensure all people have an equal opportunity to access the respite facility. The AQAA recorded that each person has an individual plan of care to meet their own specific needs. The plans are developed from information gained on assessment, and people who use the service and their families and friends are able to contribute. Information is recorded about personal preferences, care and support, and health care needs. The plans are continually evaluated on a daily basis to ensure they remain appropriate and any issues are dealt with quickly and effectively. The service encourages people to be as independent as possible and each person is Annual Service Review Page 4 of 7 treated with courtesy and in a sensitive manner. We received five completed surveys from people who use the service who responded positively and reported, The service gives good personal care, and the staff have patience and understanding for different kinds of needs. The food is excellent, hot and well served. I have never been looked after so well before. My various disabilities are understood and I am treated which such care by every single person who works there. Nursing care at all times is first class. It is apparent that follow-on shifts are kept well informed, as all care is ongoing and continuous. During my stay, I could not fault the care I received. The service has restored my faith in nursing care. The service has excellent food and is always clean and tidy. Staff are always kind and friendly. The AQAA recorded that as a result of listening to people who use the service, the questionnaires given to people have been reviewed so people can make comments about the service provided. The range of social activities has been increased to include board games and jigsaws, and people are encouraged to mix with each other and socialise with their families. The occupational therapist and physiotherapist have group activity sessions, which includes music orientated activities. The service encourages people to be involved in community activities during their stay if possible. Since our last visit, they told us a new unit has been developed to support people with their rehabilitation. The AQAA recorded that the standards of care have improved as a result of this, and length of stay have become shorter. The service has use of an occupational and physiotherapist for the units. Therapists are part of the Community Intervention Team based within the service. The AQAA recorded that as part of their own quality assurance process they receive consistently positive feedback from general practitioners and other health care professionals, and occupancy levels are always maintained and high standards of care are provided on all occasions. We received three completed surveys from health and social care professionals who commented that, The service has excellent communication and cross disciplinary working. The level of care and effort to meet patients needs and provide holistic treatment is excellent. Excellent knowledge base of all staff the service is a welcoming warmth and very friendly unit, hygiene levels are outstanding. The service provides personalised individual care, with good medical and social care interaction. The staff respect the patients as an individuals. Annual Service Review Page 5 of 7 We looked at the information in the AQAA and in the surveys, and our judgment is that the home is still providing a good service and they know what further improvements they have to make. What are we going to do as a result of this annual service review? We are not going to change our inspection plan, and will do a key inspection by 19 April 2011. However we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
