What the care home does well Staff training has been arranged for the staff. Medicines are given to people throughout the day, and some medication is recorded more accurately than is required. Fire drills and instruction for staff is done regularly. What the care home could do better: Liquid medication could be measured more accurately. Information on medicines carried forward from one month to another could be recorded to ensure medication audits are performed accurately. The Leys is situated on the outskirts of Northampton, close to local amenities and can be reached by both public and private transport. There is parking available at the front of the house. It has been owned by Mrs. Sheila Samuels for many years. There is currently an acting manager in post Mrs. Maureen McDonagh, who has recently applied to be the Registered Manager. There is a large lounge, and a bright dining room and conservatory on the ground floor and there are plans to update the kitchen. The home has bedrooms on the ground and first floors, each of which has en-suite facilities; three of these include a bath. Stairs or a stair-lift access the two bedrooms on the first floor. There is a large, pleasant, well-maintained garden with a small patio area that has tables and chairs. This is accessible for all residents. The home can be contacted by telephone or fax. The weekly fees range from £395 to £495 for a large en-suite room. There are additional costs for hairdressing, dry cleaning, chiropody and toiletries. The registration certificate, the certificate of insurance and the latest inspection report are available in the reception area. Copies of the latest Care Quality Commission reports are available from the manager. Care Homes for Older People Page 3 of 8 What we found: The reason for this inspection was to follow-up issues and comments received from an anonymous complaint, about poor medication administration for the group of residents. We also had concerns passed to us regarding the fire and evacuation system. We visited the home on a weekday morning, and the visit lasted three and a half hours. We had a tour of the home, and spoke to some residents, staff and the manager. We looked at the medication storage and operating system, the fire and evacuation system, policies and procedures. We looked at the training for staff to enable them to care for people safely. The staff are trained to give people medication, from a supply delivered by the chemist on a weekly basis. Medication is given in liquid, tablet and capsule form, and staff have policies and procedures explaining how they should give the medicines. Staff are expected to record accurately what medicines people have been given, and return what is not required to the pharmacy. Some of the liquid medication is signed for by two staff, this is over and above what we expect, however staff are not measuring the liquid medication in a consistent way. People receive medicines regularly throughout the day, some of the doses vary, in line with information passed on by medical staff. These were accurately recorded and administered in line with the medical information provided. However there was no way to check what amount of medicine is carried forward from month to month. Staff do not perform regular checks or audits on the system to ensure all medicine is given appropriately. Staff spoke of the training they had undertaken recently, these included medication, health and safety, moving and handling, and the prevention of abuse (known as poca or pova). These were also confirmed when looking at the staff training records. We also looked at the fire and evacuation system, and the files used to back up the staff information and training. Since the Fire Officer visited, updates have taken place on the Fire Risk Assessment and other associated paperwork. Staff fire and evacuation training and fire drills are carried out periodically, and this information is recorded in the files. Staff could audit the MAR charts and medicines, to ensure they are administered appropriatly. Liquid medication could be measured in an appropriate syringe before being administered.
