The previous key inspection was on 23rd January 2008. The Nightingales provides residential care for up to 23 people who have dementia. Most are older people, but whether they are under or over the age of 65 does not affect the suitability of this service to them. The house is a substantial nineteenth century building, close to Torquay town centre. There is a large communal lounge that looks out over the garden, a small lounge leading on to an enclosed courtyard, and a separate dining room. There are 21 bedrooms on two floors, some of which have en-suite facilities. Fees are currently 300 - 500 pounds, depending on care needs. What the care home does well The admission process was working well, with visits to the home arranged for prospective residents, and on-going consultation with professional health and social care workers. A keyworker system had been introduced, in order to promote individual care and attention. The care plans were seen to have been updated in accordance with residents` changing needs, and daily care was properly recorded in the case of a resident who was unwell. Social activities were provided both morning and afternoon, and individual interests were catered for. There was plenty of choice with meals. Fruit and biscuits were offered mid-morning, two main choices for lunch, andother options available daily. Potatoes were prepared in four different ways every day, as this was popular with the residents. Individual records of food eaten had been recorded, so that staff could monitor residents` food intake where necessary. These showed the variety of meals that had been served. A new kitchen had been built, which was spacious and had a new doorway on to the front garden. This was a great improvement, with space, light, hygienic conditions and modern facilities. The office had been moved to the site of the former kitchen, which is closer to the residents` communal areas than the former office, giving easier contact between the two. Residents were happy with attention they receive from the staff. `I don`t have to call for help`, said one, `they`re already around`. Another said, `the staff are super`. Visitors to the home told us that they work as `a good team`, they were `very happy with it all`, and they were pleased with the new Manager, who kept them up to date with developments. They said they would be confident in taking any problem to the Manager. Management support and monitoring were provided from elsewhere in the organisation. What the care home could do better: Residents had lockable drawers in their rooms, but the keys were not available. These should be available if residents are assessed as being able to use them. It would be good practice to record any minor concern that is brought to the Manager`s attention, so that he may show what has been done, if necessary to put it right. This would also make it obvious if mistakes were repeated. We saw that two fire doors needed attention, to ensure that they would close effectively when the emergency call bell sounds, in order to protect residents. Care Homes for Older People Page 3 of 8 What we found: This Random Inspection followed our Annual Service Report, which found that many changes had taken place in the home since our Key Inspection of 23rd January 2008. A new Manager had been appointed, who was in the process of preparing his application to register with the Commission for Social Care Inspection. We met with the new Manager, the home owner, eight residents, two regular visitors to the home, and four staff who were on duty. We found that the home was well run, with good levels of staffing, accurate recording, and plenty of choices being made available to residents in a relaxed atmosphere.
