What the care home does well People are assured their needs will be met at the home through a clear admissionsprocess. Social and health care is well planned and reviewed regularly. All risks are assessed, with appropriate action taken and medication is well managed. There is evidence of close liaison with other agencies and a range of professionals. An activity coordinator plans activities and all staff are involved in providing relevant activities and supporting people to take part. There are facilities on site for art, pottery, gardening and a range of fitness machines are provided in a gymnasium. People told us of regular day trips and they were taking part in a meeting to plan a holiday during our visit. Comments from people living there included: "I like to use the gym in the evenings", "We go out in the minibus every Thursday and Saturday", "We had a good time in London" and "I like all the activities and I want more". Meals are well liked with plenty of choice of food to suit individual needs and preferences. People said: "lovely meals", "Good food and menu options" and "meals are good, I can have what I want". Clear records show that any concerns are managed well, with all appropriate action taken to keep people safe. The premises are generally kept clean and are well furnished. The pantry floor was recently replaced and various areas have been redecorated with people being involved in choosing colour schemes. We observed staff treating people with respect and talking to them sensitively. Some people spoke with us and said the staff were very helpful and gave them the support they needed. People told us in our survey: "Staff are polite and pleasant", "Staff are understanding", "Good Staff" and "the staff are very friendly". There were staffing records to show that staff are competent and given appropriate ongoing training in order to meet people`s needs. Staff told us that they get pleasure and job satisfaction in their work at Thistle Hill Hall. One of the staff said, "If I want any additional information to gain more knowledge then I know I will get the help and support I need from the senior staff and management at all times." Another of the staff told us, "Training and other relevant support are always given." The new manager is supported by the general manager, who is also based on the premises, and an administrator. The management team ensures the home is run in the best interests of people living there. What the care home could do better: The general manager is continually improving systems for managing the service and supervising staff. We did not need to make any requirements, but we saw an area ofmildew on the ceiling in the gymnasium due to damp penetrating the roof and recommended that this receive attention as soon as possible to ensure the health and comfort of people using the room. The general manager has informed us that the recommended work was completed immediately after our visit. Date of last inspection Brief description of the care home Thistle Hill Hall is a Grade II listed building, which provides residential care for adults who have enduring mental health problems. The accommodation is situated on two floors with the upper floor being accessed via stairs or a passenger lift. The accommodation is spacious with high quality fixtures and fittings, and is tastefully decorated throughout, while still incorporating many of the original features. Outside Care Homes for Adults (18-65 years) Page 2 of 9 Brief description of the care home there are ample grounds much laid to lawn, although an abundance of shrubs and trees add to the privacy of the house. To the front of the building is a small car park for staff and visitors. All bedrooms have en-suite facilities, and there are two training kitchenettes as well as a central kitchen and dining room. On site, there is a gymnasium, as well as a hobbies/games room, with table tennis, pool and a variety of tabletop games and activities. Thistle Hill Hall has two lounges, a visitors room, and ample bathrooms and toilets in addition to the en-suite facilities. These bathrooms are spacious, and a person with mobility difficulties could be accommodated. The fees at the home start at £1,400 per week with additional payments for specialist care needs. Care Homes for Adults (18-65 years) Page 3 of 9 What we found: The inspection process focuses on outcomes for people and their views on the service that is provided. We consider the providers capacity to meet regulatory requirements and minimum standards of practice and focus on aspects of service provision that need further development. We carried out this unannounced random inspection under the Care Standards Act 2000 and Fees and Frequency Regulations 2007, which state that services shall receive an inspection visit within a three year period. The quality rating was determined at the last key inspection, which took place on 24 May 2007. We reviewed all of the information we have received about the home since the last key inspection to consider if the quality of the service had changed. We gathered information from people through our survey forms and through observations and discussions during the inspection visit. We also considered information contained in a recent annual quality assurance assessment (AQAA) that was sent to us by the manager. The general manager had informed the Commission of various incidents and these were well managed, with appropriate procedures being followed and immediate action to safeguard people. Overall, we found that the quality of the service has remained at the same excellent level. Since the last review of this service, a new manager has been assessed as fit and registered with the Commission. She is supported by the general manager who had comprehensively completed the AQAA to give all the information we needed. He told us: Thistle Hill Hall has adopted a number of ways to ensure we are listening and responding to our clients. A bi-weekly community meeting is held which clients and staff are encouraged to attend to discuss issues, concerns and plan future activities. We also involve clients in the writing of care plans, as well as maintaining open dialogues with clients through informal communications and designated 1-1 sessions. Through listening to people they had already made the following changes: We have revamped and improved our Sports & Recreational programme in response to the views of clients obtained through an annual Activity Survey conducted in November/December 2009. Through requests made in community meetings, and Meal Comments Book, menus have been changed in the kitchen to accommodate the changing tastes of our client group. Our staff are now provided a meal to eat at the same time as the clients to encourage greater unity and integration. This was as a result of client comments about how they wanted meal times to be conducted. We have employed a Clinical Psychologist to join the MDT and offer greater support to our clients, either through 1-1 sessions or through advice and support offered to staff with regards to client care/support plans.
