What the care home does well Comments from surveys indicates that: `They treat each person as individuals.` `Whitworth Lodge has shown us all new ways of enjoying [the residents] independence, [their] personality has been shown in so many ways, we didn`t think possible. A joy to see.` Whitworth Lodge organises care around the needs of the residents. Surveys received from people who live in the home state that they are happy with the care provided and their needs are met. What has improved since the last inspection? The recruitment process for staff now meets the requirements of the Regulations and Schedules. People living in the home can be confident that appropriate checks are carried out prior to staff commencing employment. Recommendations from previous inspections have also been acted on. Care documentation, staff files and a tour of the premises were examined on the site visit. Surveys were received form three residents and one relative. The manager and one member of staff were spoken with at the time of the site visit. The site visit lasted a total of four hours. What the service does well: What has improved since the last inspection? What they could do better: A comment from a survey suggested that: ‘At this moment in time I cannot find any reason to think that they need to change things to improve.’ Minor adjustments are needed to make sure that the Statement of Purpose reflects the service provided by the home. JUDGEMENT – we looked at outcomes for the following standard(s): 2 Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. Significant time and effort is spent making admission to the home personal and well managed. Prospective residents and their families are treated as individuals and with dignity and respect. There is a high value on responding to individual needs for information, reassurance and support. EVIDENCE: All three residents who responded to CSCI’s survey indicated that they had made a positive choice to move into Whitworth Lodge and received information on the service. The Statement of Purpose needs reviewing to make sure that it reflects the Registration certificate of the home. Assessments of prospective residents are undertaken prior to admission. These included details from other health professionals. Assessments undertaken on admission are person centred and evidence involvement of the resident and their representative. Good detail on specific communication and behavioural needs was present. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 7, and 9 Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. The care plan is person centred and focuses on the individual’s strengths and personal preferences. It sets out in detail how their current requirements and aspirations are to be met through positive individualised support. The plan is an up to date working tool used by the individual and all staff involved. The plan can be easily used by people who are not familiar with individual’s needs, to deliver care. EVIDENCE: Two residents files were examined. These were noted to lead from assessments and all identified needs were addressed. Residents are involved in the structure of the care plans and regular reviews are done. Daily records indicated that staff have clearly developed knowledge of behaviours, moods, signals and temperaments of residents. Individual risk assessments are in place, which give clear guidance on risk reduction, without impinging on individual rights. This was demonstrated when a resident went to the shop during the site visit. A time was agreed when they would return and the manager made sure that the resident was aware of the dangers of crossing the road. All resident surveys received indicated that they were able to choose how to spend their time. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 15, 16 and 17 Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. The home understands the importance of enabling individuals to achieve their goals, follow their interests and be integrated into community life. EVIDENCE: Residents are supported to attend day centre and local colleges. Residents are able to access the local community to go shopping or meet with friends. Residents have an individualised activities plan, which reflects their personal interests and hobbies. Holidays are planned around residents’ preferences. One resident was visiting relatives in Scotland at the time of the site visit. Another resident likes to have a daily newspaper and is support to go to the shop and purchase this, on days when they do not attend a day centre. It was noted that when residents are involved in community activities reports are obtained from the other services on progress. Residents are able to maintain contact with relatives and significant others. Personal, emotional, cultural and sexual needs are recognised in care planning. The manager reported that a weekly menu is drawn up at residents meeting, but this can be subject to change, depending on resident choice. One resident enjoys Nigerian foods and dress. Staff support this person to make sure this aspect of need is met. Support is given to maintain and improve independent living skills, such as cooking, making hot drinks and laundry. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 18, 19, 20 and 21 Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. Staff make sure that care is person led, personal support is flexible, consistent and is able to meet the changing needs of the residents. Staff respond appropriately and sensitively in all situations involving personal care, ensuring that it is conducted in privacy. EVIDENCE: Emphasis is placed on meeting needs in a way, which respects the privacy and dignity of residents. Same gender care preferences are documented and guidance is available for staff to meet personal care needs. Daily routines and house rules promote independence and individual choice for residents. Routines are not rigid and residents are able to choose when they have meals and preferred sleeping and waking times. Residents are able to have a key to lock their room when they are out. No gaps in administration were seen. Medications were noted to be stored correctly. Residents’ wishes on end of life care and death were noted within care plans, when the residents had agreed to discuss these issues. Care plans clearly demonstrate how residents’ physical and emotional needs are met. The home has an open culture that allows residents to express their views and concerns in a safe and understanding environment. Residents and others involved with the service say that they are happy with the service provision. EVIDENCE: All three surveys received from residents stated that they knew how to make a complaint if needed. Care documentation evidenced that residents are informed of how to make a complaint on admission. Aids such as large print and picture boards are available to help with this process. There have been no protection of vulnerable adult investigations since the previous inspection. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. This judgement has been made using available evidence including a visit to this service. The home is pleasant, safe place to live; the bedrooms and communal rooms meet the needs of the residents. People who live in the home are encouraged to personalise their bedrooms. EVIDENCE: Whitworth Lodge is a domestic property. Residents are able to personalise their rooms and have a choice of furniture. The home was clean and tidy on the day of the site visit. Communal areas are homely and provide sufficient space for rest and relaxation. The dining room is able to seat all residents if they chose to eat together. Repairs are carried out as required and the manager stated that redecoration is undertaken as needed. This judgement has been made using available evidence including a visit to this service. People who use services have confidence in the staff that care for them. There are consistently enough staff available to meet the needs of the residents. The home has a good recruitment procedure, which clearly defines the process to be followed. Staff receive relevant training which is focused on delivering improved outcomes for residents, EVIDENCE: Two files on staff recruited since the previous inspection were examined. There was a clear recruitment procedure in place and necessary checks had been carried out. Each file had a job description and person specification form. There was evidence of interview notes. Two references had been obtained and there was a record of employment history. Proof of identity was present on files. Appropriate induction and NVQ training were evidenced. Records also showed that staff had undergone training in First Aid, food hygiene and English as a second language when needed. Residents indicated in their surveys that they are confident in the skills of the staff team to meet their needs.
