Worcester Intermediate Care Unit is a nurse led 20 bedded specialist facility providing nursing care, physiotherapy and occupational therapy and is covered by Worcester City General Practitioners. Annual Service Review Page 3 of 7 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last key inspection or annual service review. This included: The annual quality assurance assessment (AQAA) that was sent to us by the service. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. Information we have about how the service has managed any complaints. What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement. The previous key inspection. Relevant information from other organisations. What other people have told us about the service. What has this told us about the service? The service was last inspected on 18th November 2008. Worcester Intermediate Care Unit (WICU) returned the annual quality assurance assessment (AQAA) when we asked for it. This was well completed, clear and comprehensive. The AQAA and the last key inspection report showed that the service obtain comprehensive information about peoples needs and expectations before they agree to admit. This helps the service decide if the potential admission is likely to be a success for everyone. The AQAA tells us the information gained from the referral and from our own assessment of needs is used to formulate individual care plans. The last inspection report told us the Care Manager, or her deputy, conduct the admission assessments. The AQAA told us how the service is operating and the improvements they have made and wish to make. We read the AQAA and our judgement is that the service is still providing a two star good service. The key unannounced inspection, which was carried out on the 18/11/08, formed a judgement that the service was providing a two star good service. To ensure equality and diversity is addressed, we saw at the last key unannounced inspection that the care records reflected personal needs and preferences, with care planning organised for people using the service, involving them and their families at all stages. This confirms a person centred approach to care. The AQAA told us, we have improved the paperwork we use in the care planning process. Care is more patient centered and individualised. Staff are focusing more on individual goals and involving service users in planning short and long term goals. The AQAA contained numerical data which told us they provide a service for people of both sexes, who have a range of Annual Service Review Page 4 of 7 needs such as mental health needs and dementia, physical and sensory disabilities. The service currently employs a multi-cultural team of male and female care staff. The last inspection report mentioned an ongoing problem with lack of storage facilities on the unit as one of the two bathrooms available were being used to store equipment. The AQAA states we have had plans drawn up for making one of our bathrooms in to two rooms, one for storage and one as a bathroom. We are waiting to see if the PCT renews our contract this year prior to implementing this change. We will also then purchase a height adjustable bath and bath hoist. We received nine surveys from people who use the service, and five staff members. This is what the people who use the service said about the service and its staff when we asked them what the service does well: Staff are friendly and very helpful/caring. They do their best to ensure that the patients are comfortable and well looked after. Cheerful happy atmosphere. The staff are caring and courteous. The younger carers are very kind and try to make one happy. It is very welcoming to both patients and visitors. They have helped me learn about my mothers future needs and also supported me in getting my mother out and about. This is a facility which we have very useful as a halfway house between hospital and home. It has given us the opportunity to plan for the future. This is what the people who use the service said about the service and its staff when we asked them what the service could do better: I think that more staff are needed at bedtime as I find I go to bed too early before staff change over. On occasions the home appears to need more staff. Speedier response to call bell in daytime. Provide higher chairs, low chairs are hard to get out of. There is a choice of food and can usually find a menu to sort out ones preference. My main problem the food is not hot enough and some vegetables are overcooked, although it is eatable. We received five surveys from staff. This is what the staff said about the service when we asked them what the service does well: Provides good rehabilitation. Cares for its clients extremely well considering the limitations of staffing numbers and the lack of adequate facilities. Support for staff is excellent - both by management and by the team members themselves for each other. Good communication, very informative handovers, training adequately provided most times. All the staff at WICU work hard to ensure the best care and rehabilitation for each resident. Care plans are individualised and monitored frequently to ensure the needs of the residents are being met. This is what the staff said about the service when we asked them what the service could do better: Have more staff. More time should be given to each client for optimum care to be achieved - impossible with the staffing numbers and the increasing dependency of clients. Incorporate daily activities (group or individual) to keep the residents from getting bored. Increase the number of staff on all shifts to improve the quality of care. Change some of the old furniture e.g chairs in the residents rooms that are no longer suitable for the residents needs. The unit is working with the same ratio of staff to clients as 6/7 years ago despite the considerable increase in dependability of the clients. Staff are often exhausted and frustrated when they are unable to give the quality of care they would like because of time constraints and poor staffing levels. Annual Service Review Page 5 of 7 Under the section barriers to improvement the manager has stated, we have attempted to negotiate with our landlords to allow us more space for storage and a meetings room but this has not been forthcoming. We are also hoping to secure increased funding from the PCT to enable us to improve the ratio of staff to residents as their dependency levels have increased. Information in the last key inspection report dated 18/11/08 also tells us that the people using the service were pleased with the standard of care they receive. The AQAA told us the service has received no complaints in the last 12 months. We have not received any complaints about the service since the last inspection. The service has sent in notifications telling us about any significant events or incidents in the home and how they have managed these situations. There have been no changes to the management of the home since the last inspection. The AQAA tells us about the policies and procedures the service has in place and when they were last reviewed. Some policies have not been reviewed for over two years. These policies need to be reviewed to ensure they reflect current legislation and practice. What are we going to do as a result of this annual service review? There will be no change to the inspection plan and will do a key inspection by 18/11/2011. However, we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
