We give a quality rating following a full review of the service. We call this review a ‘key’ inspection We do an annual service review when there has been no key inspection of the service in the last 12 months. If yes, what have they been: Date of last key inspection: Date of last annual service review (if applicable): Brief description of the service Earlham House is a privately owned care home for adults who have continuing mental health problems. The home is registered to provide care for seven adults between 18 and 65 years. The care providers are Dr and Mrs Clayton. The home is a large semi-detached property with accommodation spread over three floors. The home has recently been extended and in addition to the seven bedrooms provides communal space that consists of a comfortable lounge, which offers access to a raised patio and garden. There are two bedrooms on the ground floor both with Annual Service Review Page 2 of 6 No 0 7 1 0 2 0 0 8 en-suite bathroom facilities. The other bedrooms are upstairs. The office is located at the top of the house and this is also where staff sleep-in at night. The home is situated near to Wood Green, North London and is near to shops and other amenities. There are good public transport links to the home. The stated commitment of the service is to treat residents as individuals with individual needs and needing individual care. We strive to promote independence and ensure privacy and dignity is maintained. Annual Service Review Page 3 of 6 Service update since the last key inspection or annual service review: What did we do for this annual service review? This Annual Service Review uses information gathered from a variety of sources to assess the standard of performance of the agency since the last key inspection and over the course of this year. The information taken into account includes the agencys self assessment document called the Annual Quality Assurance Assessment (AQAA); notifications; the previous key inspection and any other information received by the Commission related to the agency What has this told us about the service? The last key inspection completed on 7th October 2008 gave the home a Three Star Excellent rating. It reported that the people living in the home benefited from a service that had experience in supporting people who have enduring mental health issues and offered a professional and supportive environment for them to live. The report acknowledged the stable management team who helped to ensure consistently high standards of care. The residents spoke positively about the staff working in the home and the support they received from them. The AQAA received in August 2009 is comprehensively well written. Meeting the needs of the residents and ensuring their independence and wellbeing is central to the report. The AQAA states its improvements in the last 12 months as follows: Reviewed the referral form to include the same data in a more user-friendly and specific format and made it possible to complete electronically. Formalised the keyworker system Reviewed the format and content of the Care Plan document to include more specific information and to enable it to be filled in electronically, making it more easy to read and understand for service users and staff. Increased service user consultation Included service users in employment selection We have seen a greater interest in unaccompanied outings since introducing more daytrips, the increase in days out seems to have inspired the service users to go out more. New volunteer assistance means that service users spend more time with people who dont have the same mental health issues as themselves. It also means that they have more choice available in activities inside and outside the home. An ex-resident regularly re-visits as company in the home and goes on outings with service users. Connections with Reed in Partnership have increased our access to pathways to work for those service users willing to engage. Annual Service Review Page 4 of 6 Improvements in the level of practical domestic tasks undertaken by some service users. We have created a dedicated self-administration of medication risk assessment and process to identify readiness to self-medicate. Service users are encouraged to attend their own appointments wherever possible, are accompanied by keyworkers if required. We have developed an in-house Adult Safeguarding training programme delivered with use of appropriate examples relevant to the home Redecorated exterior of house Redecorated sitting room Gardens maintained and improved to allow more light and accessibility. The service considers that it continues to provide good value for money. The Commission has not received any complaints or information that indicates any change in the the quality of service. What are we going to do as a result of this annual service review? The Commission will continue to monitor activities linked to the Agency through intelligence gathered by any notifications, complaints, concerns from staff, people using the service, their representatives or any other third party. The agency will receive another Annual Service Review next year and will examine all key standards at the next key inspection. This will be due in three years from the last key inspection.
