What the care home does well

The home provides a safe, comfortable and supportive environment. Residents are provided with a wide range of activities, with good support for social, leisure and development opportunities. Staffing levels are good, and staff have been provided with a good standard of training appropriate to the needs of the client group. Records are maintained to a high standard and clear and easily accessible.

What has improved since the last inspection?

Although there were satisfactory risk assessments in place, the format has been reviewed in order to make them neater and easier to read. Medication procedures have been also been reviewed.

What the care home could do better:

The acting manager agreed that the lack of a long term manager may have had an impact on the general running of the home and perhaps communication with families, and that although this situation has improved, more work is need to ensure the stability is maintained.

The home is in a residential area of Ashford, within 15 minutes walking distance of the town centre, with easy access to public transport, health and adult education centres, shops, churches, a swimming pool and other amenities. The house provides accommodation on 2 floors. There are 6 single bedrooms for residents, lounge, dining room, kitchen and bathrooms. In addition there is a staff sleeping in room, office/sleep in room, laundry, storage areas and a large enclosed garden. This announced inspection was carried out over approximately 5 hours. Prior to this inspection the home had submitted a pre-inspection questionnaire completed by the registered manager, together with questionnaires completed by the residents or their families. The Inspector toured the building and inspected a number of records, policies and procedures. The Inspector spoke with 1 of the current 5 residents and met another in passing. 2 of the residents were visiting their families. The Acting Manager, who is in the process of applying for registration, 2 team leaders and one other member of staff assisted with the inspection. The manager commented on how well the staff had coped during the period when there was no permanent manager. What the service does well: What has improved since the last inspection? Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2 There are sound systems for assessing prospective residents’ individual needs and aspirations. EVIDENCE: The acting manager and team leaders explained that comprehensive preadmission assessments are carried out prior to residents gradually being introduced to the staff and other residents. These include input from the existing placement and/or from healthcare professionals. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6,7,9 There is a clear and consistent care planning system in place and this includes all the information necessary to ensure that staff can meet residents’ needs. Residents participate in their assessments and reviews wherever possible, and are consulted on all aspects of the running of the home. Residents are enabled and supported in taking risks as part of an independent lifestyle. EVIDENCE: 1 resident said that he is always consulted over issues relating to his care needs and that he is able to make decisions on how he chooses to live and on the day to day running of the home. Residents’ care plans included health care and social details, aims and objectives, risk assessments and clear guidelines on how best to assist them. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,15,16,17 Residents have good access to a wide range of leisure activities that are appropriate to their behavioural needs, age, peer group and culture. The home’s relationship with the community is a positive one and enhances their social, recreational and educational opportunities. Residents’ nutritional needs are well catered for, with a varied and balanced selection of foods available to meet their choice and cultural needs. EVIDENCE: The information in the residents’ daily records confirmed that they enjoy a wide range of leisure activities and that there are sufficient resources, in terms of funding and staffing, to support these activities. Staff and 1 resident referred to having attended various events within the community. The acting manager said that advice is given as and when necessary regarding personal relationships and the records confirmed this. There was a good selection of foodstuffs, fresh fruit and vegetables etc in the store cupboard. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18,19,20 Staff have a good understanding of residents’ support needs, and personal support is offered in such a way as to promote and protect residents’ privacy, dignity and independence. The health care needs of residents are well met with evidence of referrals to a range of healthcare professionals. There are sound systems for the receipt, storage, administering and disposal of medication. EVIDENCE: The residents’ care plans included clear guidelines on providing support and the daily diaries showed that there is regular monitoring of health care and social care needs. 1 resident said that he is very well supported and always feel able to talk things through with staff. Another resident with communication needs was seen to be relaxed and comfortable interacting with staff. Staff confirmed that only trained staff would administer medication and that the procedures are in the medication file. The medication administration record charts were clear and current, with appropriate details for “prn” medication or anything out of the ordinary. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. JUDGEMENT – we looked at outcomes for standard(s) 22,23 There is a clear and easily accessible complaints process and residents’ views and listened to and acted upon. The systems in the home are designed to protect residents from abuse, neglect and self harm. EVIDENCE: Staff were aware of the home’s complaint procedure which is issues to all residents and staff. 1 resident said that he would feel comfortable telling staff about anything he was unhappy with in the home. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. JUDGEMENT – we looked at outcomes for standard(s) 24,30 The standard of the environment within the home is good, providing residents with an homely, comfortable and safe place to live. The home is clean and hygienic. JUDGEMENT – we looked at outcomes for standard(s) 39,42 The home regularly reviews aspects of its performance through self review, and consultation with residents, their relatives or representatives and staff. There are clear and comprehensive systems in place to ensure that the health, safety and welfare of residents is protected and promoted. EVIDENCE: Although the lack of a long term manager may have had an impact on the general running of the home, this situation has improved since the appointment of the acting manager who is seeking registration. Staff and 1 resident said that they are regularly consulted on the general management of the home. All records, policies and procedures relating to health and safety, such as risk assessments and fire alarm/lighting testing, were clear and current.
