The inspector found there to be outstanding requirements from the previous inspection report. These are things the inspector asked to be changed, but found they had not done. The inspector also made 4 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

201a Berrow Road provides a comfortable environment for service users who wish to live in a family style home. There is sufficient communal space and facilities to meet service users needs. Service user rooms have been personalised to reflect individual needs and preferences. Service users are free to choose when they get up, when they go to bed and how they spend their day. Both service users are able to access the local community independently. Service users are supported in maintaining contact with family members. Service users often go out on trips or for meals at restaurants with Mr and Mrs Hawkins. Service users stated that they were happy with the care they receive and meals provided.

What has improved since the last inspection?

Care plans have been developed the provide details of the level and type of assistance required by each service user. Brief risk assessments have been completed to address some activities. A new boiler and central heating system has been installed, and the kitchen has been refitted. What the care home could do better:

The Registered Manager must ensure that the Statement of Purpose contains all required information and that a Service User Guide is developed so that prospective service users may be provided with the appropriate information to make a decision regarding admission into the home. Due to the behaviour of one service user, the home must ensure that a comprehensive risk assessment is completed in conjunction with their Social Worker in relation to the service user going out alone, and the social activities that they participate in. Mrs Hawkins has worked in the care sector for many years and completed NVQ Level 2 in care in 1996. Mrs Hawkins has not attended regular training to maintain her professional development and therefore should attend training in relevant areas such as care planning, managing challenging behaviour, equality and diversity and the Mental Capacity Act to ensure that the care provided continues to follow best practice. The home has not developed a fire risk assessment. This must be completed to ensure that all risks are identified and appropriate control measure implemented. The Inspector will ask the Fire Officer to provide Mr and Mrs Hawkins with further advice regarding this.

Mr and Mrs Hawkins own the home and the Registered Manager is Mrs V Hawkins. Both owners live at the home and provide all care and support. The home is spacious and is set in attractive gardens. It is within easy reach of Burnham town centre and the sea front. All bedrooms are located on the first floor and are of single occupancy. The home shares a family type bathroom with a bath and overhead shower. Service users are able to use a small lounge on the ground floor. There is a large rear garden that has a summerhouse. The kitchen and laundry are domestic in style. There are currently two service users who live permanently at the home. The fees are £500 per week. This sum does not include the purchase of toiletries, clothes or monies to engage in social activities or hobbies. 201a Berrow Road DS0000016279.V335953.R01.S.doc Version 5.2 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. This key inspection was unannounced and was completed by one Inspector over one day. The Inspector was able to speak with each service user in private. During the course of the inspection, records were examined, care practice observed and discussions held with Mr and Mrs Hawkins (Registered Manager and Provider). Prior to the inspection Comment Cards (surveys) were sent to the Social Workers for these service users. The surveys had not been received by CSCI at the time of this report being written. Four requirements and two recommendations have been made as a result of this inspection. What the service does well: What has improved since the last inspection? JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3 & 4 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Prospective service users are not provided with the appropriate information to make an informed choice regarding admission to the home. The Admissions policy states that an assessment of need will be completed prior to a service user moving in, to ensure that only appropriate admissions are made to the home. EVIDENCE: The home has a Statement of Purpose that provides details of the facilities provided. This was examined during the inspection. It was found that the Statement of Purpose does not include information regarding whether nursing care is provided, the arrangements for service users to engage in social activities, the arrangements for consultation with service users about the operation of the home or details of any therapeutic techniques used (if any). The home has does not have a Service User Guide. This states that a detailed assessment will be undertaken prior to a service user moving in. Prospective service users will be encouraged to spend short periods of time at the home such as tea visits before moving in. Service users are admitted to the home on a threemonth trial basis. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. 6,7,8 & 9 Since the last inspection the home has developed plans to meet service users assessed needs. Service users are able to make choices regarding their daily life. The risk assessment for one service user must be reviewed and agreed with this service users’ Social Worker, and any other stakeholders involved in their care. EVIDENCE: Two care plans were examined in detail during this inspection. Brief care plans had been developed for each service user. Both care plans had been updated in January 2007 and were due for review in July 2007. Care plans provided details of service users individual personal and social care needs. Daily records had been maintained. Both care plans had been signed by the service user. Care plans included information on any restrictions in place, and the rationale for these. Risk assessments had been completed for specific activities. Both service users are able to express their wishes and needs, and were clear that are able to discuss these with their Social Worker. Both service users confirmed that they are offered choices regarding their daily life. Service users choose whether they spend time in their room, the small lounge or summerhouse. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 11,12,13,14,15,16 & 17 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users appear to treat the home as their home and are encouraged to participate in the running of the home, but mainly choose not to. Service users can access a variety of local leisure and recreational facilities. Meals are provided in the style of a family type environment based on known likes and dislikes, and appropriate records maintained. EVIDENCE: Both service users confirmed that they participate in cleaning their bedrooms but do not wish to join in any other household tasks. Mr & Mrs Hawkins should continue to encourage the service users to participate in the running of the household to develop and maintain independent living skills. 201a Berrow Road DS0000016279.V335953.R01.S.doc Version 5.2 Page 13 One service user attends a work placement for two days a week. The placement for the second service user ended at Christmas, and they are waiting for a further placement to be provided. Service users are encouraged to access local activities. One service user is able to go for walks within the local area independently. The second service user attends clubs and is accesses the local shops. The home maintains records of all activities and local facilities that service users access on a daily basis. The home is family based, and outings and holidays are arranged in accordance with this. The Registered Manager advised that the service users and themselves enjoyed a holiday in West Bay last year, and may visit that area again this summer. Both service users also have TV’s in their rooms and one service user also has a PS2, video, music centre and DVD. One service user has pet goldfish in their room. Both service users are free to choose how they spend their leisure time. Service users have unrestricted access to communal areas, including the kitchen and garden. The owners have a separate lounge and conservatory that is private and not available to service users. The home assists both service users to maintain regular contact with their relatives. Service users stated that they are happy with the food in the home. In line with the nature of the service provided there are no set menus and food is cooked on the day in accordance with known likes and dislikes. Service users have access to the kitchen and are able to make drinks and snacks at any time. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 18 & 19 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users are able to access health care services. The management of medication follows safe practice. EVIDENCE: Service users confirmed that they are able to choose when they get up or go to bed. Service users choose which clothes to wear. One service user confirmed that they receive the help needed to meet their personal care needs. Both service users confirmed that Mr and Mrs Hawkins knock their bedroom doors before entering. On the day of the inspection both service users appeared to be well attired. Service users are registered with local GP’s and are assisted to attend appointments if they wish to have support. One service user stated that they make their own appointments with the GP. Personal files contain details of appointments with other appropriate health care professionals. 201a Berrow Road DS0000016279.V335953.R01.S.doc Version 5.2 Page 15 One service user takes regular medication, which is administered by Mrs Hawkins. All medication is kept in a locked cupboard. Medication Administration Records were examined during the inspection, and it was found that these had been appropriately maintained. 201a Berrow Road DS0000016279.V335953.R01.S.doc Version 5.2 Page 16 Concerns, Complaints and Protection The intended outcomes for Standards 22 – 23 are: 22. 23. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 22 & 23 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users feel that their views are listened to. There are appropriate procedures in place to protect service users. For the protection of service users, records are maintained of all transactions involving their finances at the home. EVIDENCE: The home has a Complaints policy and there is a copy of the Somerset County Council “Safeguarding Vulnerable Adults” policy. Both service users stated that they would be able to raise any issues of concern with their Social Worker. There have been no complaints received by CSCI or the home since the last inspection. The home assists one service user in managing their finances. The home has adopted the system for recording service users finances that has been formulated by Somerset County Council. The home maintains brief records of financial transactions for the second service user, whose parents are their Appointee. The home has appropriate restraint and whistle blowing polices. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 24,25,26,27,28,29 & 30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users live within a comfortable and homely environment. There are sufficient communal areas and bathing facilities to meet their needs. The home has been maintained to a good standard of cleanliness. EVIDENCE: 201a Berrow Road is a detached house situated within walking distance of the sea front and close to Burnham town centre. There is an ongoing programme of maintenance and re-decoration at the home. Since the last inspection the kitchen has been refurbished, a new heating system and boiler fitted, and the electrical hardwiring system has been tested. 201a Berrow Road DS0000016279.V335953.R01.S.doc Version 5.2 Page 18 Mr and Mrs Hawkins live on the premises and have some a private lounge and conservatory that is not accessible to service users. On the ground floor there is a lounge for service users. There is a kitchen with dining area, and domestic style laundry facilities located in a utility area. There is a well-maintained garden at the rear of the property, which includes a summerhouse. Bedrooms are all single occupancy and are located on the first floor. There is no lift so all service users need to be physically able. Both of the service users showed the Inspector their rooms. The bedrooms seen were comfortable and reflected their individual needs and preferences. There is a bathroom and two toilets on the first floor and another toilet on the ground floor. Appropriate locks are fitted. Presently, no aids or adaptations are needed to meet specific needs of the service users. JUDGEMENT – we looked at outcomes for the following standard(s): All care is provided by Mr and Mrs Hawkins, there are no other staff employed, therefore this outcome group has not been assessed at this inspection. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 37,38,39,40,41 & 42 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. The home appears to be well run. The Registered Manager must attend further training to continue their professional development and to ensure that the care provided continues to follow best practice. The home has taken appropriate measures to promote the health and safety of service users. EVIDENCE: The Registered Manager is Veronica Hawkins who runs the home in partnership with her husband. Mr and Mrs Hawkins live on the premises and both provide care and support to the service users. Mrs Hawkins has attended training on health and safety and food hygiene. Updated training is required in First Aid. At the previous two inspections it was recommended that Mrs Hawkins should undertake further NVQ training. This has not been pursued and remains as a recommendation. Mr Hawkins has not attended training in First Aid or Food Hygiene. It is also recommended that the home obtain a copy of the Care Home Regulations 2001 and National Minimum Standards for Younger Adults, so that they can ensure that the service provided complies with these. The home appears well run. Due to the size of the home, daily activities and meals can be flexible to meet individual service users needs and preferences. There are appropriate polices and procedures in place to safeguard service users. Records relating to service users are appropriately maintained and up to date. The home has not developed a fire risk assessment. This must be completed to ensure that all risks are identified and appropriate control measure implemented. The Inspector will ask the Fire Officer to provide Mr and Mrs Hawkins with further advice regarding this. The home is domestic in style and has smoke detectors that are tested on a monthly basis. Since the last inspection a new heating system and boiler have been installed. The electrical hardwiring has been tested and a satisfactory certificate obtained. The hot water outlets are fitted with thermostatic valves. Fridge and freezer temperatures had been checked and appropriate records maintained. In line with providing a family type environment the service user’s bedroom windows are not restricted and wardrobes have not been secured. The home displays appropriate Employers Liability insurance.
