Conditions of registration: 1. 2. The maximum number of service users to be accommodated will be 3 (three). The service users accommodated will be aged over eighteen (18) and under sixty-five (65) years on admission. 30th November 2005 Date of last inspection Brief Description of the Service: 6 High Beech Close is a modern executive style house with four bedrooms; the smallest of the bedrooms is now the office and staff sleep-in room. One bedroom has and en-suite bathroom and the two other bedrooms share a family size bathroom on the same floor. The home has a kitchen, utility room off the kitchen, dining and lounge areas and downstairs cloakroom. The home also has a reasonable sized back garden, a driveway and garage to the front of the house. The home is registered for three younger adults with learning disabilities, and is part of East View Housing Management Limited (EVH). This is an overview of what the inspector found during the inspection. The reader should be aware that the Care Standards Act 2000 and Care Homes Regulations 2001, uses the term ‘service user’ to describe those living in care home settings. For the purpose of this report, those living at 6 High Beech Close are referred to as ‘residents’. This report reflects a key inspection based on the collation of information received since the last inspection and an unannounced site visit conducted by an Inspector on the 19th September 2006 starting at 09.00am and leaving at 5.00pm, and returning the following day in the afternoon at 2.00pm and finally leaving at 5.00pm. This also included the inspection visit of 5 High Beech Close as the registered manager of 5 High Beech Close has management responsibilities for both homes. The site visit included a tour of the premises and an examination of various records including medication, care and staffing records. The Inspector met with the Registered manager of 5 High Beech Close, the staff member on duty during this inspection visit. During this site inspection visit, the inspector met two of the residents and spoke with them individually and together, and they were both very positive with the care they received at 6 High Beech Close. Unfortunately the third resident was not at home, and was going out for the evening directly from the day centre, therefore her views are not expressed in this report. However, the CSCI did receive ‘Have your say about 6 High Beech Close’ surveys from the residents and two comment card from relatives, and all were positive about the home. What the service does well:
The home was found to do most things well. The management of the home was found to be especially good in supporting residents and staff. The staff member interviewed expressed how much she enjoyed her work and was very complimentary about the manager and the EVH organisation. The home has leadership and direction from the registered manager of 5 High Beech Close, which is next door, and she has management responsibility for this home. All prospective residents are only admitted following a full assessment undertaken by the manager and other stakeholders to ensure as far as possible the home can meet the needs of the individual. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3,4 & 5 Quality in this outcome area is Good. This judgement has been made using available evidence including a visit to this service. Prospective service users can be confident their needs will be assessed, and when they take up residence and their aspirations will be met. EVIDENCE: Since the last inspection a resident from the home next door (5 High Beech Close) has moved in to 6 High Beech Close. The decision to move was made jointly by the organisation and the resident. The organisations procedures were followed as far as necessary, as the resident was well acquainted with the home. There was documentation to confirm the transfer and the resident confirmed to the inspector the she visited the home, had meals with the other residents and made her decision to move because the residents are more her age, and share similar interests. She was seen to be very relaxed and a good rapport was observed between her and the other resident who was home during this inspection. 6 Highbeech Close DS0000061409.V307696.R01.S.doc Version 5.2 Page 9 Individual Needs and Choices

The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6,7,8,9 & 10 Quality in this outcome area is Good. This judgement has been made using available evidence including a visit to this service. The manager demonstrated her knowledge of the individual residents, and are aware of the complex needs of the residents and encourage them to have an independent lifestyle as far as possible. EVIDENCE: The inspector viewed care plans, and inspected in depth the care plan of the resident that has recently moved into the home. The new resident spoke to the inspector and she confirmed how she was happy with the move and felt this home was better for her she stated “I like the girls living here, we get on well together.” The resident was very relaxed with the staff member on duty and the other resident at home. When the inspector arrived to undertake the inspection they were all were sitting in the garden enjoying a hot drink. All residents have a contract and service user guide with are signed by the residents and dated.

This document was seen by inspector and was found to be very comprehensive. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,15,16 & 17 Quality in this outcome area is Good. This judgement has been made using available evidence including a visit to this service. The manager and staff team are enthusiastic and are always looking for new ideas to enrich the residents’ lives and variety of activities are thoughtfully sought. EVIDENCE: At the time of the inspection visit, two of the residents were at home, and had returned from their different day activities. One had been to college and another had been working at a stables. The third resident had gone out for dinner, and therefore the inspector did the meet with her on this inspection visit. However, the ‘Have your say about 6 High Beech close ‘ stated she was very happy living at 6 High Beech and she had no worries or concerns. 6 Highbeech Close DS0000061409.V307696.R01.S.doc Version 5.2 Page 12 The programme of day services is very tailored to the individual and is a combination of traditional day services, college courses, voluntary work and 1:1 activities at home and in the community. The venues and activities are all documented in the individual resident file. The weekends tend to include food shopping and general housework and leisure activities. The staff member interviewed during this visit was very enthusiastic about her job and enjoyed supporting the residents. 6 Highbeech Close DS0000061409.V307696.R01.S.doc Version 5.2 Page 13 Personal and Healthcare Support
The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19 & 20 Quality in this outcome area is Good. This judgement has been made using available evidence including a visit to this service. Residents receive personal support appropriate to their physical, emotional and healthcare needs. All personal and healthcare needs are well documented, including a full medical review for the new resident. EVIDENCE: The newest resident has moved from next door (5 High Beech Close) and all documentation held on her has been transferred to home. All three residents are registered with local GPs and when required have the services of other health professionals, including the support from the local Community Learning Disability Team (CLDT) which over the last year has provided psychological support and advice. The medication files and storage were inspected and found to be in order. All staff have had the relevant training and are all deemed to be competent in the administrating of medication to the residents.

The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22 & 23 Quality in this outcome area is Good. This judgement has been made using available evidence including a visit to this service. Residents and their relatives’ complaints would be taken seriously and investigated, and the staff have the knowledge and understanding to take the correct action to safeguard residents from abuse. EVIDENCE: It is now evident the manager is managing both homes well, 5 & 6 High Beech Close. The staff member on duty during this inspection visit was interviewed and had a good understanding of the organisations policy and procedure on adult protection. Since the last inspection there haw been an Adult Protection alert for one of the residents following an allegation of incidents outside the home. The manager and staff are supporting the individual through this experience and she is having 1:1 support during the day, when not attending organised day activities. It was evident from documentation held within the home that when this allegation was revealed the home took appropriate action and informed all the relevant people. During the interviews with the manager and residents the inspector asked about the home’s complaints and concerns procedures. The residents said they understood this concept all said they would speak to the staff and felt confident they would ‘sort things out’. The manager and the staff member have a good understanding about the protection of vulnerable adults and what action they would take if they saw or were told of any form of abuse within the home.

Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24 & 30 Overall the quality in this outcome area is Adequate. This judgement has been made using available evidence including a visit to this service. The premises are suitable for the needs of the residents and are kept clean and homely. However, there are signs of wear and tear and there are areas in need of decorating. Also better arrangements need to be made for the housing of the pet rabbits. EVIDENCE: The inspector accompanied by the manager and for some of the time by a resident undertook an full tour of the home including all communal areas, including the kitchen, bathrooms the garden and individual bedrooms. The home was found to be very clean and homely, but there was a strong odour from the pet rabbits in the garden. The lounge patio window was open and the odour was coming into the house, also the resident had brought the rabbit food into the home and this had a strong smell. During the tour areas of the home were showing signs of wear and tear, especially in the front bedroom, which is in need of re-decorating, 6 Highbeech Close DS0000061409.V307696.R01.S.doc Version 5.2 Page 16 Better arrangements need to be made for the pet rabbits, as there was a strong odour in the dining room during this inspection visit. The management has identified this and the resident has been informed of the seriousness and she has been reminded of her responsibilities for the care of the rabbits and the effect of her pets have on the other residents. There needs to be a review of the rabbits habitat and the storage of their food, at the present time they are by the patio windows and the resident will insist on bringing the rabbit food into the lounge, as she thinks the food will get wet. 6 Highbeech Close DS0000061409.V307696.R01.S.doc Version 5.2 Page 17 Staffing

Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 34,35 & 36 Overall the quality in this outcome area is Good. This judgement has been made using available evidence including a visit to this service. The staff employed have the skills and experience to meet residents needs and aspirations. EVIDENCE: The registered manager at the present time has management responsibility for 5 and 6 High Beech Close and this includes the recruitment and supervision of both staff teams. The staff team at 6 High Beech Close are very familiar with residents in both homes and staff work in both homes at times of staff shortages. This arrangement is satisfactory to the residents, and the inspector saw the good rapport between the staff and residents of both homes. All staff have completed a full induction and many were progressing to various levels of NVQ’s. 6 Highbeech Close DS0000061409.V307696.R01.S.doc Version 5.2 Page 18 The newest member of staff came on duty in the afternoon on the second day of the inspection visit, and she confirmed the recruitment procedures were followed and she has undertaken an intensive induction and other relevant training. The manager supplied a copy of the training matrix for the home as part of the pre-inspection questionnaire information and the training within the last year has been very comprehensive and will ensure the staff have the training and knowledge to meet the changing needs of residents. All staff have regular supervision, which occurs at eight weekly and is recorded and signed by the supervisee and supervisor. The manager also receives regular supervision and support from the Senior Management Team of the EVH organisation. 6 Highbeech Close DS0000061409.V307696.R01.S.doc Version 5.2 Page 19 Conduct and Management of the Home

The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 37,39 & 42 Overall the quality in this outcome area is Adequate. This judgement has been made using available evidence including a visit to this service. The manager has a good understanding on the area in which the home needs to improve, she has a clear development plan and vision for the home, but she is not the registered manager for this home. Also health and safety records relating to the home must be available for inspection. EVIDENCE: The registered manager is very experienced and holds the Registered managers Award, and she is also in the process of completing a Psychology Diploma. She has successfully undertaken the management of both 5 and 6 High Beech Close and continues to improve the service offered to residents through good management of the health, welfare and safety of the residents and staffing responsibilities. However, this home does need to have a named registered manager to met with the regulation.
6 Highbeech Close DS0000061409.V307696.R01.S.doc Version 5.2 Page 20 The EVH organisation has a robust Health and Safety policy and procedures and the organisation’s health and safety officer makes regular audits. However, the manager believes all safety checks have been carried out and especially the portable electrical appliances, but the records were not at the home.
