This inspection was carried out on 11th October 2007.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The service is a home in the real sense of the word. The service user says they are happy and content. They join in with Mrs Upstone`s family both in Spain and England. Mrs Upstone believes that the service user should be well presented and receives the services they are entitled to such as health care. Generally the service user spends extended holidays at Mrs Upstone`s apartment in Spain, which they enjoy, preferring life there to life in England. They like the better weather and more relaxed attitude to life. They enjoy swimming in the apartments pool. However due to health problems two holidays were enjoyed this year in England. The home is clean, well maintained and well decorated.

What has improved since the last inspection?

The service user has had two operations and now enjoys both better health and mobility. Several rooms in the house have been redecorated.

What the care home could do better:

The standard of care and support is adequate and no specific improvements are required.

This key inspection was carried out over a period of time and concluded with an announced visit to the home between 1.45pm and 3.15pm. Where the regulatory inspector spoke at length to the service user and Mrs Upstone. The inspection process consisted of information collected before and during the visit. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 3 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. The service users needs remain well met. EVIDENCE: The service user has lived here for 21 years. Their care was arranged by a social worker. An assessment would have been undertaken at that time. The service user is content to remain at 82 Beagleswood Road where they are well looked after and feel at home. Mrs Upstone had previously worked in a hospital and care and therefore has care experience. 82 Beagleswood Road DS0000023811.V352921.R01.S.doc Version 5.2 Page 8 Health and Personal Care The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 8, 9 & 10 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. The service users health, care and support are provided in a manner, which suits them and promotes their independence. EVIDENCE: The service user has been in hospital twice this year to have two operations. Both have been successful and their health and mobility is now much better. The service user did not enjoy their stays in hospital although Mrs Upstone visited every day. Mrs Upstone obviously motivates and encourages the service user to promote good health. The service user said they were having their flu jab shortly. The service user is independent in all their personal care needs. During the periods after the operations they were more fragile and Mrs Upstone did assist with some aspects. However they are now able to do all this themselves again. All aspects of the service users life are discussed between them and Mrs Upstone on a daily basis. There are no records in place. 82 Beagleswood Road DS0000023811.V352921.R01.S.doc Version 5.2 Page 9 The service user is clearly able to make decisions about their day-to-day life and future plans both they and Mrs Upstone have. Where it is needed Mrs Upstone gives the service users guidance. Mrs Upstone is clearly aware and assesses any possible risks. She gave examples of two situations where there were possible risks. These were discussed with the service user and action was taken to reduce these risks. Mrs Upstone manages the medication and ensures the service user takes it at the right times. Mrs Upstone always makes sure the service user is very presentable and buys all their clothes of which they are obviously very proud and took time to show me one of their jumpers and new coat. 82 Beagleswood Road DS0000023811.V352921.R01.S.doc Version 5.2 Page 10 Daily Life and Social Activities The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 & 15 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. The service user enjoys a range of educational, social and recreational activities. EVIDENCE: The service user is able to travel independently on public transport and has a free bus pass. They used to carry a piece of paper with their details on but they now know this off by heart. The service user is currently attending college for reading and writing. Although the times have been changed which do not coincide with the buses. Mrs Upstone is going to discuss this with the college. The service user continues to meet their friend in Tunbridge Wells each Saturday. Daily activities include walks, games, going to the family or family visiting, some gardening tasks such as weeding and sweeping, television as they like all the soaps, a place in the sun, nature and other films. The service user walks to get their daily newspaper each day. The family has a large dog. The service user says they attend the local church each Sunday. The service user is able to speak some Spanish. They have not been to Spain this year due to the health problems and hospital stays. There were problems with swimming in the sea, this was discussed and agreed that they will now only swim in the swimming pool. It is planned they will go to Spain again next year. Mrs Upstone has a two-bedroom apartment there. At Christmas they went Centre Parcs, Longleat with the extended family and will be going again this year but to a different location. They also enjoyed a holiday in Winchelsea this year with the family. The service user has very limited contact with their family. The occasional telephone call such as at Christmas and when they were in hospital after Mrs Upstone advised them of the operations. Mrs Upstone plans, shops and cooks all the meals and is well aware of the service users likes and dislikes. The service user says they does not want to be involved in the cooking and likes Spanish food best paella, which is Mrs Upstone’s speciality. The main meal is taken in the evening and the service user has a very good appetite although didn’t think much of hospital food so Mrs Upstone took in daily snacks. 82 Beagleswood Road DS0000023811.V352921.R01.S.doc Version 5.2 Page 12 Complaints and Protection The intended outcomes for Standards 16 - 18 are: 16. 17. 18. This judgement has been made using available evidence including a visit to this service. The service user says they have no concerns and are happy with their quality of life. EVIDENCE: The service user said they have no concerns or complaints and are quite happy. This judgement has been made using available evidence including a visit to this service. The service user lives in a homely, comfortable and clean environment which meet their needs. EVIDENCE: The house was clearly a family home. It was clean and decorated to a good standard. Since the last inspection several parts of the home have been redecorated. Mrs Upstone’s does the cleaning and the service users laundry with help from a member of her family. The service user has their own bedroom, which contained personal photographs of their family and also Mrs Upstone’s family. They are obviously proud of their clothes and possessions. 82 Beagleswood Road DS0000023811.V352921.R01.S.doc Version 5.2 Page 14 The service user has the sole use of a toilet and shares a bathroom with Mrs Upstone, which has shower over the bath. There is a well-maintained garden (which a member of the family helps maintain) with pleasant views and a patio area with tubs and seating, lawn and flower borders. Mrs Upstone ensures that the service is run to include the service users best interests. Mrs Upstone ensures that the service user has sufficient money for their day-to-day needs.
