This inspection was carried out on 27th October 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The proprietor is in daily contact with the home and has good communication with the registered manager. The home is now fully staffed except for a weekend domestic and the work force has remained stable since the summer when there was a change with the directors of the company and the administrator left. The home has a good range of care plans and risk assessments in place and these are regularly reviewed.

What has improved since the last inspection?

All requirements from the previous inspection have now been met. The administrator left in the summer leaving a lot of work to be done in the office. This has now been achieved with the new employee and is running satisfactorily. The home has now recruited an activities co-ordinator.

What the care home could do better:

Five requirements have been made at this inspection and they include accurate records to be kept concerning nutrition. The management to continue to update the beds at Abbey Chase to ensure height adjustable beds are available to the residents that require them. All call bells must be within reach of the resident and carers to make sure they are left with this means of communication before leaving the room.

Up to five named people accomodated may be in the catergory DE(E), older people with Dementia 19th October 2004 Date of last inspection Brief Description of the Service: Abbey Chase Care Home is privately owned and operated by Abbey Chase residential and Nursing Homes Limited. Nursing care for up to sixty older people, personal care for two older persons with physical disability and care for four named persons with dementia is offered in premises situated next to the Thames in a rural part of Chertsey. Accommodation is provided in fifty-three single rooms and five shared rooms. The gardens are well maintained and are accessible to the residents if they choose to sit and enjoy the scenery and life on the river. There are arrangements for disability access, which includes ramps and pathways. There is a pleasant shaded and safe central courtyard which `can be enjoyed by residents in the ample seating areas and also viewed by residents from bedrooms. The home also provides plenty of car parking facilities. This announced inspection took place over eight hours and was the third inspection to be undertaken in the Commission for Social Care inspection year April 2005 to March 2006. Lesley Garrett, Lead Inspector for the Service, carried out the inspection. Jean Short, Registered Manager, Mr Kennedy, Proprietor and Hilary Manders, unit manager represented the establishment. A full tour of the premises took place. The inspector saw most of the residents and spoke with a few in more detail. A pre-inspection questionnaire and a number of returned ‘comment cards’ were also used to write this report This was a positive inspection. The inspector would like to thank the residents, staff and manager at Abbey Chase for their time, assistance and hospitality during this inspection. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1 The homes Statement of Purpose and Service User Guide provide residents and prospective residents with details of the services the home provides enabling an informed decision about admission to the home. EVIDENCE: Since the last inspection on the 19th April 2005 the Registered Provider has updated and reviewed the current Statement of Purpose. It is now a detailed residents handbook in larger print containing the mission statement, statement of purpose and service user guide. Each resident is given a copy on admission and the master copy is kept in the office and is updated to include the most recent inspection report. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 There is a clear care planning system in place to adequately provide staff with the information they need to satisfactorily meet resident’s needs. EVIDENCE: The inspector sampled a number of the care plans and spoke with the manager about the systems in place for care plans. Each resident has their own individual file containing all the necessary documentation. Most files contained photographs of the resident’s only new admissions are remaining, assessment sheet, care plans and risk assessments. These are reviewed and updated regularly. There is a daily record which cares write and registered nurses countersign. The inspector noticed that although there are sample signatures some entries are signed with the first name only and should contain both names. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12 The home has activities but no formal structure with a co-ordinator to oversee and organise a programme for the residents. EVIDENCE: Following the last inspection in April 2005 it was a requirement that the home look at the provision of activities in the home to meet the needs of all resident’s at Abbey Chase and this was outstanding from the inspection in October 2004. A number of the returned service user ‘comment cards’ stated that they did not feel the home provided suitable activities. The home has now recruited a suitably qualified activities co-ordinator who will be starting in November. At the moment the home has keep fit classes held weekly, a weekly reminiscence session, bingo, monthly church services and organised concerts. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18 The staff has knowledge of Adult Protection issues, which protects residents from abuse. EVIDENCE: The home has the February 2005 copy of the Surrey Multi Agency Procedures for Adult Protection. The manager stated that these were the procedures the home followed and she showed knowledge of them. Ten staff has received training in July of this year and the manager was about to apply for the next course details of which she had just received. Training certificates are kept in the staff files and the inspector saw these at inspection. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24 The home has two wings and the west wing is much lighter and brighter than the East wing. EVIDENCE: The west wing is the new extension, which was added approximately four years ago, and the décor is light which makes this a very bright environment and the east wing, which is older, needs to be bought up to the same standard. Gradually the paintwork in the east wing is being changed to white and the rooms decorated when they have been vacated. This re-decoration programme needs to continue to improve the overall appearance. The home has four double rooms and they have adequate screening to provide privacy for each resident. Each bedroom door has a lock but the manager stated that not many of the residents lock their door and they are often left open The home does not have many height adjustable beds and they are being purchased gradually. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 & 29 Resident’s needs are met by the number and skill mix of staff. There were some gaps in the recruitment process. EVIDENCE: The manager told the inspector that she does monthly dependency levels for the proprietor so that the numbers of staff needed can be determined for the rota. Some of the returned ‘comment cards’ stated that the residents felt there was a shortage of staff and therefore the manager must continually assess the changing needs of the residents to ensure adequate staff numbers. The east wing has appointed a new unit manager and she is also the deputy. The manager stated that there are no nursing or carer vacancies now. The home has five domestic staff working Monday to Friday and the home has one vacancy to cover the home at weekends. The home on the day of inspection was clean and tidy and free from offensive odours. The inspector looked at a sample of staff files. All had application forms, two written references, contracts and CRB checks had been done. The files did not contain recent photographs of the employees and this will be a requirement at the end of the report. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 35 The residents are protected by the homes policy on finance. EVIDENCE: The manager stated that there is no reason for the residents to have large amounts of money in the home. The home provides all toiletries so they would only have to buy something if they wanted something special. Extras like chiropody, hairdresser or newspapers are itemised on their monthly bill. This is in their contract and families are told this on admission. In an emergency valuables can be kept in the safe until the family or representative can collect it. A book is kept and the items are signed for when they are collected.
