Conditions of registration: 1. Care of one service user who is under 65 is restricted to one person whose date of birth is 21/11/40 Date of last inspection 13 December 2004 Brief Description of the Service: Rogers House is a modern purpose built home for the care of the elderly located in a residential area within easy walking distance of local shops. The home is currently registered for the care of 41 older people accommodated in 37 single and two shared bedrooms all of which have en-suite facilities or a dedicated toilet close to the room. All bedrooms are fitted with a call bell system and a telephone and TV point. There are 5 day rooms and also a large conservatory opposite the main entrance. The home has 5 assisted bathrooms, assisted toilets and has an 8 person lift providing access to all 3 floors. This is an overview of what the inspector found during the inspection. This unannounced Inspection was carried out by Regulation Inspectors Fiona Holdaway and Joanne Griffiths. A tour of the home took place and the Inspectors met with service users and staff, the Manager was present in the home throughout the Inspection, care records, policy documents and staff files were inspected. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3, 4, 5 Service users needs are fully assessed to ensure that the home can meet their needs. Prospective service users have the information they need to make a choice about living in the home and an opportunity to visit beforehand. EVIDENCE: Rogers House is a modern purpose built home for the care of the elderly and is equipped with mobile hoists and specialist equipment where a need has been identified. Service users needs are assessed prior to being offered a place at the home to ensure that the home is able to meet the needs of the individual. Assessments and care plans sampled were in good detail and provide staff with clear guidance to support service users with personal and health care needs. Service users and their relatives are encouraged to visit the home before the person moves in and the first month of stay is a trial period for both parties. Service users said that they and their relatives had visited the home and been provided with sufficient information to make the decision to move in. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9 and 10 Service users personal, social and health care is provided according to service users needs and wishes. Service users receive a high standard of personal care and are treated with respect and their privacy is maintained at all times. The systems for medication administration are good with clear arrangements in place to ensure service users medication needs are met. EVIDENCE: Service users users confirmed that staff always knock on doors and support them with bathing and personal care in a manner that maintains their privacy and dignity. Service users have access to healthcare services such as the G.P., Dietician, District Nurse, Dentist and Optician, Chiropodist etc as required. . Care records included healthcare appointments and the outcomes and assessments of moving and handling, nutrition and risk of pressure sores. Medication was seen kept securely in a dedicated room. A monitored dosage system is in place and the homes MAR (Medication Administration Record) sampled were properly recorded. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15 Service users have access to a range of activities in the home. They are encouraged to make choices about all aspects of their daily routine. Service users are supported to maintain contact with family, friends and the wider community. Service users benefit from an enthusiastic cook keen to promote good nutrition. The meals in the home are good offering both choice and variety and cater for special dietary requirements. Many of the activities are run by volunteers and include games, bingo, crafts, music and movement, sing-a-longs and listening to a piano player. There are large print books, puzzles and videos available in all of the lounge areas. Minutes of a residents meeting were displayed on the notice board and evidenced that service users are consulted about all aspects of the home and their comments and suggestions are acted on. From the comments made by service users it was evident that they make decisions based on information and choice. The home operates an open visiting policy and service users stated that friends and family can visit at any time. The volunteers that support the home also run a trolley shop 2 days a week. The home has links with the local church and a service is held in the home weekly. Menu records were seen and these showed that a balanced and varied diet is offered. Service users are asked to make a choice from 2 main meals on offer. It was evident from discussion with the cook that service users are consulted about meals and he promotes cooking methods that enhance nutritional values of food. JUDGEMENT – we looked at outcomes for standard(s) 16 The home has a clear complaints procedure and service users and relatives are aware of how to complain. EVIDENCE: The homes complaints record was viewed and evidenced that service users benefit from having any concerns or complaints they have listened to and responded to promptly. Service users spoken with were well aware of their right to complain and generally presented as very satisfied and happy with the service provided. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,22,23,24 Service users live in a safe, well-maintained and clean environment. The standard of décor, furnishing and fittings are high providing a homely and pleasant living environment. EVIDENCE: The home is a modern purpose built home that is decorated and maintained to a high standard and is comfortably and attractively furnished. The overall appearance was very welcoming and pleasant.. The home is accessible throughout. And has an 8 person lift to all 3 floors. There are 5 living rooms and also a large conservatory opposite the main entrance. Some lounges were without televisions for service users who preferred a quiet area. The gardens were attractive and well maintained there is lawn and a large paved area with raised flowerbeds. There was easy access to the gardens from the home. The home had CCTV at the entrance of the home and also a security controlled entrance. The bathrooms were of a high standard with modern assisted baths in place in spacious, clean and attractively decorated bathrooms. Bedrooms viewed were attractively decorated and comfortably furnished and were personalised with pictures and ornaments etc. Service users confirmed that they had been able to bring some items of their own furniture and were very satisfied with their rooms. All bedrooms are lockable and all residents have a lockable facility within their rooms. All bedrooms have a nurse call system. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 29 The numbers of staff employed are sufficient to provide the level of care required. The home has a good recruitment procedure which supports and protects service users. EVIDENCE: Four staff files were seen including a new member of staffs’. Staff files included application forms, written references, proof of identity, and CRB (Criminal Record Bureau) checks as required to ensure sound recruitment practices that safeguard service users. Staff rotas evidenced that there are sufficient numbers of staff to meet the needs of service users and consistent planning to cover any absences. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33, The atmosphere of the home is friendly and welcoming. The manager is open and approachable to service users and staff, she offers clear guidance and support to staff and runs the home in the best interests of service users. There is a high level of consultation with service users and their representatives. Service users meetings are every 3 months the minutes of the most recent meeting were on display on the notice board and evidenced that service users views are sought and acted upon. Service users commented that they welcomed these opportunities to have their say. Rogers House has a Volunteers Forum and a representative of the group attends the meetings. A suggestions box is operated in the home for any comments or suggestions on the service provided. Abbeyfield operates an annual satisfaction survey for service users and their representatives.
