Conditions of registration: 1. All re-development work detailed in the refurbishment plan submitted to the Commission on 25.04.04 must be completed by March 2009. 2. The home may continue to accommodate 1 named service user who falls within the registration category A(E) 3. No one falling within the category of PD(E) may be admitted into the home where there are 5 service users who fall within the category of PD(E) already accommodated within the home Date of last inspection 14th January 2005 Brief Description of the Service: Abbott House is owned by Shaw Healthcare(de Montfort)Ltd, and is registered to provide twenty four hour care for 32 Older people, of whom up to 7 may have a diagnosis of a dementia related condition, and up to 5 may have a physical disability.The home is situated on the outskirts of Oundle in East Northamptonshire, and is within walking distance of the town, which has a variety of small shops. The home is divided in to small units and is surrounded by pleasant gardens. This is an overview of what the inspector found during the inspection. The focus of inspections undertaken by the Commission of Social Care Inspection is upon the outcomes for Residents, and upon their views of the service provided. The primary method of inspection used was ‘case tracking’ which involved selecting 2 residents and tracking the care they receive through review of their records, discussions with them, and with the care staff, and observations of care practices. The inspection took place during a weekday over a period of 6 hours and was carried out on an unannounced basis. Communal areas, and some bedrooms were visited. A selection of care records, and essential records of the home were reviewed. A number of the residents and staff were spoken to as part of the inspection process. The Registered Manager Mrs Linda Taylor was present during the inspection. The Commission had received a number of comment cards from residents and relatives prior to the inspection. In the main the comments recorded were positive and praised the home and the staff for the way in which the care is provided. What the service does well:
The home offers a friendly warm environment in which the residents and the staff are very friendly and welcoming. Residents are encouraged to be as independent as possible and to maintain links with relatives, friends and the community. Residents and a visitor highly commended the staff, and feel that they are able to talk with the Registered Manager and staff about anything. One visitor, who visits her friend on a regular basis, said that she couldn’t wish for better care for her friend, and a resident described the home as ‘ the best home in Northamptonshire’.

All residents spoken with were pleased with the variety and choice available, and had particularly enjoyed a recent boat trip. The Registered Manager, Mrs Linda Taylor is keen to raise standards further in the home. Her management style is approachable and open, and one that provides leadership and guidance for the staff. Essential training is provided for all staff. What has improved since the last inspection? What they could do better: This home continues to provide a service where the priority is to ensure that residents lead an independent life as possible, and reach their full potential with support from a committed staff team. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3,5 The pre-admission process of all potential residents to the home is well managed. EVIDENCE: Potential residents are issued with information, such as the Statement of Purpose, outlining the services provided at the home, so that older people and their families can make a fully informed choice about whether the home is suitable and will be able to meet their needs. No admission is made to the home until full assessments from Care Managers and health care professionals have been provided, and a member of the home’s staff has met with the potential resident and carried out a homes assessment. Residents move in on a trial basis. during which time a comprehensive assessment takes place, including the completion of a ‘getting to know you document describing the person and their history’. Following the trial period a review, meeting is held with the resident and relatives to decide if a permanent place can be offered.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,10. The arrangements for planning care in this home are good ensuring that health, personal and social care needs of people living in the home are fully met. EVIDENCE: Since the last inspection the staff have received training in the completion of the care plans. Two files were chosen for reviewing and case tracking during the inspection and both were found to have been completed to a high standard, indicating identified needs, short term and long term goals, and the clearly defined tasks required by care staff for residents to achieve those goals. Frequent reviews of the care plans are conducted, and necessary changes made to the care plan. Concise reports of individual night time support, had been recorded. Comprehensive risk assessments had been developed. Residents said that the Doctor was called if they were unwell, and there was evidence of Doctors, Chiropodists and Opticians visits within the files. The

A visitor confirmed that she was informed if her relative was unwell. A district nurse, visiting the home said that both herself and her colleagues were satisfied that the health care needs in the home were met by the staff, and said that appropriate referrals were made and health issues were well monitored by care staff. The residents whose care was case tracked through this inspection were not able to self- medicate. The medication records identified a robust system that ensures safe practices, and an audit of one of the resident’s medication was satisfactory. Three residents confirmed that staff respect their privacy, and assisted them to maintain their dignity at all times. From a discussion with a member of staff, about how the residents needs are met, it was evident that the staff have a sensitive approach to the provision of personal care, and are aware of privacy, dignity and independence issues. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,15. Social activities are well managed and creative, providing daily variation and interest for people living in the home. EVIDENCE: An activity co-ordinator is employed for 18 hours per week, and a discussion was held with her, whilst some of the residents were engaged in completing cross-stitch items. The activities organised consist of a weekly music/ movement session called ’ Full of Beans’, Tai Chi, a weekly bowls session, and other group activities. Activities are also created specifically for the individual resident, based on information obtained during the assessment period. Outings are arranged and residents were particurely enthusiastic about a recent boat trip, which had included a picnic. Residents are informed about the forthcoming activities through a news sheet and through items posted on the notice board. The local community are involved in the home through church services, mobile library visits, bowls/bingo afternoons, and support from the Mothers Union and Rotary club. During the inspection a volunteer from the community arrived to go to the local shop and purchase items for the residents.
Abbott House Care Home D C51 C08 S60179 Abbott House V234115 110805 Stage 4.doc Version 1.40 Page 13 Visitors can visit at any reasonable time, and residents can receive visitors in the privacy of their own room, or there are areas around the home where they can comfortably meet A visitor said that she was made welcome by the staff at any time, and a resident said’ my daughter visits every day and is made to feel very welcome’. Most of the residents commented favourably about the food, and the menu, There is a 5-week menu and the food on the day of the inspection looked wholesome and nutritious. One resident said the choice of food was excellent. The Cook attends the residents meetings where the menu is discussed. and changes made, according to the wishes of the residents. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,18. The complaint and protection process within this home is adequate and sufficient to protect residents. EVIDENCE: A resident confirmed that she was aware of the complaints procedure, although had not had a reason to use it, but would complain to one of the senior staff if necessary. 16 relatives had responded through the comment cards to the Commission for Social Care Inspection that they were aware of the complaints procedure although had not used it. One complaint investigated since the last inspection has been dealt with and found to have been unsubstantiated. The home has a Protection of Vulnerable Adults policy. Protection of Vulnerable adults training has taken place within the last 12 months, and further training has been arranged. Staff spoken to were aware of the types of possible abuse and the reporting procedures required. All residents who had completed the comment cards had responded that they felt safe in the home. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,26. The environment provides safe comfortable surroundings and is maintained to an acceptable standard. EVIDENCE: The owners and the Commission are aware that the building does not meet the current standards for care homes and is in need of substantial investment to bring the home up to standard. The company have submitted plans to the Commission for a new building on the site. The observations made during this inspection were concerned with the safety of the building rather than the décor. A tour of the building was undertaken during the inspection and the home was found to be accessible, safe and fit for its stated purpose. New dining room furniture had recently been delivered, and two new carpets had been fitted. Residents spoken to commented that they find their rooms to be comfortable and well maintained. Rooms are comfortably furnished and personalised with small furniture and belongings. One lady said that she did not like the

The home has a laundry area, with the walls and floor easily cleanable, and with hand-washing facilities. Washing machines, with a sluicing facility, and dryers were available. Policies exist to deal with the control of Infection. Radiator covers have been fitted to prevent the danger of a resident burning themselves in the event of a fall. There was a good standard of cleanliness within the home, and a member of the cleaning staff said that she was provided with sufficient cleaning materials to be able to do her job. The kitchen was clean, tidy and well organised and in place were robust monitoring procedures for hygiene and food preparation standards. Cleaning schedules, temperature checks and storage were noted to be carried out in the correct manner. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27.29.30 The staff team are experienced, knowledgeable about the resident group, and committed to improving the quality of life of the people they care for. EVIDENCE: There was evidence within the staff files that the homes recruitment processes are robust and ensures that the residents are supported and protected. The training programme demonstrated that the necessary training was provided, to ensure that staff have a sound knowledge base to provide care for the residents. The training plan identifies when staff are due for training so ensuring that their knowledge is in line with current good practice. Approximately 75 of the existing staff team have obtained National Vocational training level 2 or above. Residents and a visitor commented, about how good the staff were and one resident said’ The staff are lovely and always willing to help’ whilst a relative described the staff as ‘quite remarkable’ Recruiting to staff vacancies is an issue in Oundle, and the home relies on the help from agency staff, however tries to minimise the effect on the residents by always using one agency and using staff who are known to the residents. Abbott House Care Home D C51 C08 S60179 Abbott House V234115 110805 Stage 4.doc Version 1.40 Page 18 3 relatives commented through the comment cards that there was not enough staff on duty, although staff and residents on the day of the inspection indicated that the staffing level was satisfactory. The number of staff hours in respect of residents needs, based on guidance recommended by Department of Health Residential Forum indicated that the number of hours used in the home is above the recommended minimum. The calculation was based on the dependency levels at the time of the inspection, and therefore may not reflect future need, if the level of dependency increases. The Registered Manager is aware of the need to regularly review the hours and where necessary increase the staffing levels, in light of the residents needs. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,35,38 There is an effective and proactive management structure in place that is accessible and responsive to the needs of both the residents and staff. EVIDENCE: Mrs Taylor meets the standards for Registered Managers as set out, and has completed the National Vocational Qualification level 4 in management, and the Registered Managers Award. In addition the Manager attends periodic training to update her skills. Mrs Taylor is continually assessing the provision of care in the home, and ensures that staff receive appropriate training, and have the knowledge and skills to meet the needs of the residents living in the home. Residents and staff confirmed that they were able to talk to the Registered Manager about anything and were confident that she would listen to them and sort out problems in a professional way.
Abbott House Care Home D C51 C08 S60179 Abbott House V234115 110805 Stage 4.doc Version 1.40 Page 20 There are clear lines of accountability in the home, and staff spoken to by the Inspector were sure of the management structure and responsibilities in the home. Most of the residents manage their own finances and are provided with a lockable facility. The home takes control of four residents finances, and the records for these were found to be accurate with three signatures obtained for withdrawals. Appropriate insurance cover was in place. The home is proactive with regard to health and safety and has good systems in place to ensure that risks to residents are minimised and as far as possible eliminated. The Owner is knowledgeable about her responsibilities under health and safety legislation The fire logbook was examined during the inspection, and was found to be satisfactory. A member of staff was able to explain the fire procedure. There was evidence within the training plan that staff had received training in Control of substances hazardous to health, Infection control and First aid training.
