Conditions of registration: Date of last inspection 23rd June 2005 Brief Description of the Service: Acorn Lodge provids residential care for ten service users. Four are registered, Mentally Disordered and six Dementia. The care home is situated in a residential area close to all community amenities, including shops and the public transport system. Access to the home requires service users to be mobile without the need for mobility aids. The care home has two floors accommodating service users. They include five ground floor single rooms one with en- suite facilities and five first floor rooms, one double and four single. There is a ground floor shower and toilet room , close to the entrance of the home. There is no lift access to the first floor therefore service users occupying first floor rooms must have good mobility. This is an overview of what the inspector found during the inspection. The inspection took place during a daytime period. It was the second inspection and was unannounced. During the inspection the registered manager was available and assisted the inspector with the inspection process. The inspector spoke to staff on duty. Six residents were spoken to as part of the inspection process, all said positive comments about the way they live in the home. All resident’s personal allowance records must be kept up to date, and confirm each resident has received their individual personal allowance. There were some areas of concern in respect of the homes environment. These require attention to ensure the comfort and safety of the residents.

Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 10 Privacy is upheld for residents by staff who carry out personal tasks. This makes the residents feel safe and secure. EVIDENCE: There was evidence resident’s rights to privacy and respect are met by the staff of the care home. One residents spoken to said “They help me if I need help when I’m having a bath, but they leave me alone when I ask them.” There are locking mechanisms on bedroom, bathroom and toilet doors to make sure residents using these facilities have their rights to Privacy met. Staff spoken to say “We always respect residents wishes when they are attending to personal needs, although this is based upon their individual risk assessments”. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 21, 24, 26 Some work is required to the environment to ensure residents have the necessary facilities, to ensure their comfort and safety. EVIDENCE: There is evidence of general on going maintenance of the homes environment. Two residents were in their rooms during the inspection and said they felt comfortable in them. There was evidence of personal effects in all rooms, which helped people feel they belonged there. One resident said, “I like having my things around me”. Two rooms seen require replacement carpets and replacement sinks, in order to make sure they are a comfortable environment for the people who live in them. The ground floor toilet and shower now provides residents with better access, due to its repositioning. However there is a requirement for the work including the surrounding step into the shower to be completed, for the appearance of the room.

This is reflected in the homes written literature and addressed with residents or their representatives during the assessment process. One resident spoken to said, “So long as I can go out and about when I want it doesn’t matter”. There was evidence of a cleaning programme in place, to make sure the home is clean and hygienic at all times for the comfort of people who live and work in the home. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 35, 38 Residents living at the care home are protected by systems in place for their personal health and safety. All financial records must be kept up to date for the protection of residents. EVIDENCE: The systems in place for the protection of residents and staff were examined and it was confirmed they are in general up to date. A recent fire inspection recommended a number of areas to be addressed. It was noted work is to be carried out to comply with these recommendations. The manager must ensure good records are kept for the recording of personal allowances. This should be maintained weekly with evidence of the persons signature who is receiving the personal allowance. This will ensure residents’ finances are adequately protected.

Standard 19 Regulation 23(2)(b) Requirement There is a requirement for the ongoing maintenance work to continue including replacement of the carpet in room 6, replacement sink and surround in room 7. Ground floor shower room to be completed. All personal allowance records must be complete and up to date at all times, with evidence of signatures for those providing and receiving the allowance.
