What the care home does well

As well as the `Hotel` style aspects of the Home being of exceptionally high quality, the overall impression is one of Residents being `put first`. Activities are frequent, well-planned and well organised, with clear evidence of being strongly influenced by input from Residents. The recent building and refurbishment programme has been quite extensive and it is clear thoughtful and careful management of the process has avoided the potential disruption to Residents.

What has improved since the last inspection?

The Manager and Staff have worked very well to meet all `Requirements` cited at the previous Inspection. These included `Requirements` relating to the frequency of care planning review, the accurate maintenance of medicine administration records, the introduction of a clearly stated `Statement of Conditions` and the sharing of questionnaire findings with Residents, Relatives and other interested parties.

What the care home could do better:

Judging by the verbal made by Residents, together with observations during the Inspection, there appears nothing of significance to which the Home could turn their attentions, over and above that which they are currently doing, as all aspects of care provision are of excellent quality.

Service users in the category OP may be 50 years and over. One service user (male) identified in the variation report dated 26 July 2005 may be accommodated at the home in the category DE(E). This category will be for the lifetime of the service user only and whilst the home is able to meet the service users needs without adversely affecting other residents who are accommodated at the home. At such time the registration category will revert back to frail elderly. 26th July 2005 Date of last inspection Brief Description of the Service: Aldridge Court is a privately owned two-storey Victorian house situated on the outskirts of Aldridge. Set in large grounds, among mature gardens and trees, the Home enjoys extensive views over open countryside with ample car parking. The Home has recently been extended and now provides a Home for up-to 57 older people requiring nursing care. Refurbished to a very high standard the accommodation comprises mainly single rooms, with a small number of shared double rooms located in the original building, and a several thoughtfully located lounge/dining areas. Rooms on the first floor are accessed via a shaft lift and services including laundry, catering, housekeeping and maintenance are all provided in-house. This Unannounced Inspection commenced at 10.00 a.m., lasted 3.0 hours, and was undertaken by one Inspector. This Report is a product of observations made during a tour of the Home, a review of care related documentation, including staff recruitment/ deployment records and a range of documents/ records reflecting the general operation of the Home. In addition, discussions were held with the Proprietor, Manager, members of Staff and 5 Residents, one of whom was receiving ‘respite care’. Previous high standards of direct care provision and overall management have been maintained, and are implemented in a friendly and open atmosphere, which is now further enhanced by the recent completion of work to a ‘newbuild’ extension, together with major internal re-building and refurbishment of the original building. The ‘new build’ provides for an increase in Residents from 36 to a maximum of 57. At the time of the Inspection the Home had 30 Residents but was expecting to work up to full occupancy over the coming months. Residents spoken with were all in agreement the quality of care they receive was of very high quality. Comments supporting this included: - “Exceptionally committed and caring Staff, well led and motivated…”, “The Staff are always available and make great efforts to look after me…”, ”I enjoy my food. I have been a Resident for many years – it’s like a posh hotel…” “I have a chronic medical condition, which I was not coping with very well at all. I now have a very good understanding about how to manage it myself, all thanks to the staff. I will be going home soon...” What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2. A ‘Requirement’ cited at the previous Inspection relating to ‘Choice of Home’ has been met. EVIDENCE: Examination of a randomly selected sample of Residents’ personal files, demonstrated the Home now issues Residents with an appropriate ‘Statement of Terms and Conditions’. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,9. All Residents benefit from a Care Plan, which identifies their individual care needs and sets out a clear and appropriate programme of care. The storage, administration, and disposal of medicines, are fully in accordance with accepted good practice. EVIDENCE: Examination of a randomly selected sample of Care Plans showed the model of Care Plan, utilised by the Home, to be of an excellent design, diligently and effectively applied in aiding the provision of care pertinent to Residents’ individually assessed care needs, and is subject to monthly review. Inspection of medicine storage provision and administration records demonstrated the Home’s practices meet current related legislation, and the guidelines of the Royal Pharmaceutical Society. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20,26. Residents benefit from accommodation, décor, and furnishings, of the very highest standard. The standard of cleanliness in the Home is excellent. EVIDENCE: Virtually all of the building works relating to the new extension, partial internal rebuilding/refurbishment of the original accommodation, have been successfully completed to a very high standard. However, due to the Resident population being at virtually half capacity it will be a more meaningful exercise for a full ‘Environment’ assessment at the next Inspection when occupancy will, no doubt, be much higher. A tour of the premises showed levels of cleanliness in all areas of the Home are a credit to the Housekeeping Staff. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,29,30. Staff numbers and skill-mix on duty were sufficient to meet the assessed care needs of current Residents. Recruitment and employment practices are consistent with the safeguarding of Residents. The Home has a high commitment to providing training for Care Staff. EVIDENCE: Staff numbers and skill-mix on duty were consistent with that shown on the rota, indeed providing a very generous high Staff : Resident ratio – particularly in respect of trained nurses due to the Home having a full complement in post pending full operation of the Home. Staff Personal Files demonstrated evidence of full compliance with the Standard and Schedule 2 of the Regulations. Staff are subject to a thorough, and relevant, orientation/induction programme, which is followed by comprehensive ‘foundation’ training, e.g. ‘manual handling and lifting’, ‘fire safety’, ‘simple infection control’. Staff were observed to be carrying out care provision to a high standard, sympathetic to identified care needs. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,33,35,38. The Home has excellent leadership from the Manager, who, in turn, is very well supported by the Owner. Operationally the Home is very well organised, with the central purpose being ‘the best interests of Residents’. Residents financial interests are safeguarded. Health and Safety Policies/Procedures/Practices were satisfactory. EVIDENCE: Residents and Staff were seen to approach the Manager with a range of issues, which were received in an interested and involved manner. In turn the Registered Manager is well supported by the Owner who visits the Home on a regular basis. Evidence was seen which demonstrated the management of Resident’s personal monies, and related documentation, are in accordance with the requirements of the Standard. COSHH requirements were satisfactory, with maintenance and servicing of equipment regularly undertaken, and appropriately documented. Records are maintained for hot water supply to baths, and water tested during the Inspection was found to be satisfactory. COSHH data sheets were up-to-date.
