The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The home is currently being extensively refurbished. Phase one of the refurbishment has been the completion of the new extension which has been completed and furnished to a very high standard. All areas of the home seen were found to be clean, tidy and free from any offensive smell. Residents and relatives all state that the home provides excellent care for residents under the very experienced and capable manager and her staff. Care plans and care risk assessments are available and have been updated for all residents. Residents spoken to said that the home has a range of activities available for residents to choose whether they wish to take part, which meet the needs, interests and capabilities of its residents. Food at the home is very good, is excellently presented with a choice of meal always available. Special and individual touches such as individual sauce boats for residents to put their own sauces or gravy on their dinner as they wish are available. Meals served at the home would not be out of place in any restaurant.There is sufficient and appropriately skilled staff to meet the needs of residents. Residents say that staff treat them with respect and ensure that their right of privacy and choice is upheld. All residents and relatives said that the Manager is very approachable and they would have no hesitation in raising any concerns with her. The home has a comprehensive complaint policy which support residents and staff to highlight any concerns.

What has improved since the last inspection?

The new extension is now in use and meets all requirements of the National Minimum Standards. The number of double bedrooms has been reduced and there is now a variety of assisted baths to meet the differing needs of residents. The new extension is furnished to a high standard, with the original building also being refurbished to the same high standard. All prospective residents have a comprehensive assessment of their needs before coming into the home. The Manager also now confirms in writing that following an assessment of the prospective residents needs, the home is able to meet their needs. Care plans are now available for those residents who are bed or room bound to address the risk of isolation.

What the care home could do better:

The majority area of development for the home is a need for robust recruitment and selection procedures that safeguard vulnerable people from people who are not suitable to work with vulnerable people. The home has also developed new terms and conditions that need slight development to meet the requirements of the regulations. Terms and conditions of residency must also be given to all residents. Care records need to be reviewed at least monthly with a detailed record of the review and when appropriate changes that have been made to their care plan. Staff do not always record whether medicines have been administered, or if medicine has not been administered the reason why it has not been given.

Date of last inspection 8/11/04 Brief Description of the Service: Aldridge Court is a privately owned two-storey Victorian house which has been extended and refurbished to provide a home for people requiring nursing care for up to 34 older people. The home is set in extensive grounds among mature gardens and trees with ample parking separated from the service users garden and large patio. The home is on the outskirts of Aldridge town and has extensive views over open countryside. The home has a number of lounges and a lounge/ dining room with an extensive refurbishment of the original building currently being undertaken. An increase in the number of residents that the home can be accommodate will be accessed when the refurbishment of the original home has been undertaken. Residents bedrooms are on two floors and can be accessed by a shaft lift. Services including laundry, catering, housekeeping and maintenance are provided in-house. This was an announced inspection undertaken by one Inspector. Total time spent at the home was 7 hours and included a tour of the new extension ( the original house is currently not occupied as it is being extensively refurbished), a review of records, talking to visitors, service users and staff. Care records were reviewed as part of the “case tracking” of four residents. Aldridge Nursing Home is privately owned. Mrs Judy Neadle is the Registered Manager. Three of the previous four requirements were found to have been fully addressed, three new requirements were made as a result of this inspection. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2,3, Prospective residents needs are assessed prior to their admission to the home, this gives assurance that the home is able to meet their needs. All residents do not have comprehensive terms and conditions of residency and therefore are not aware of important information relating to their residency at the home. EVIDENCE: All prospective residents have a comprehensive assessment of their needs before they come to stay at the home. All assessments of care needs are undertaken by a senior nurse, these assessments enable the home to decide whether the home is suitable to meet their needs. Whenever possible the prospective resident or their representative are involved in the assessment of needs but this is not always recorded. The Home Manager writes to the prospective residents confirming that the home is able to meet their needs should they wish to come and live at Aldridge Nursing Home. There is a need to extend the trial period identified within the terms and conditions of residency from four to twelve weeks as identified within the legislation for property disregard. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,10 The healthcare needs of resident’s are identified and met but are not always reviewed as frequently as required this may mean that the resident does not receive the required care. Residents are treated with respect and their right of privacy is upheld EVIDENCE: All resident’s care records seen contained care plans and care risk assessments. Risk assessments available include the risk of pressure sores, moving and lifting of residents, continence, nutrition, falls and for the use of bedrails. One resident has been admitted on a temporary basis before a final decision can be made that their complex care needs can be met at the home. This resident’s family were very pleased in all aspects of care that their relative had received. Care plans and risk assessments are reviewed but not at the required minimum of monthly, some care plans identified a need for three monthly review which is not appropriate. It was also evident that the evaluation recorded of some care plans was insufficient and that care plans did not always reflect the care that was being given. Relatives and residents state that they are informed of any changes to their care needs although this is not always recorded. A selection of medication administration charts were seen and which had gaps for the administration of some medicines. Staff do not record the reason for not administering medication as required. All residents comment cards returned and observation made during the visit identify that staff treat residents with respect and ensure that their right of privacy is upheld. Staff knock on toilet and bedroom doors before they go in. Residents may either receive their visitors in their own room or in one of the lounges if they wish. Staff also call residents by their preferred name which was confirmed by residents spoken to and is also identified within care records. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,15 Service users find that the lifestyle experienced in the home matches their expectations and preferences and satisfies their social, cultural, religious and recreational interests and needs. Contact with family, friends and the community is maintained. Diet is wholesome and choice and presentation of the food served is excellent. EVIDENCE: Residents spoken to during the inspection and comment cards forwarded before the inspection, said that routine within the home is flexible. Residents chose when they go to bed, get up and where they have their meals, greater choices will be available when the refurbishment of the original building has been completed. The home has an Activities Organiser who is employed for twenty-eight hours a week. An activity programme is available with residents having a choice of activities such as bingo and board games, creative activities, exercises, film club, reminiscence. Entertainers regularly provide sing-along sessions with karaoke also popular with residents. Trips out are arranged and include regular attendance at a tea dance and individual shopping trips. The home have commenced care plans that address the social isolation of those residents who, due to their high level of dependency or their choice, spend Aldridge Court Nursing Home E55 S20786 Aldridge Court NH V234869 260705 Stage 4.doc Version 1.40 Page 14 long periods of time in their room, this is good practice. The residents’ survey identified that the home has suitable activities available to meet the needs and capabilities of residents. The home has “open visiting”. On the day of the inspection there were a number of visitors coming and going throughout the day. Visitors spoken to and visitors comment cards forwarded to the Commission for Social Care and Inspection before the inspection said that they are always made welcome when they visit the home. Visitors also said that they are able to visit their relative in either their own room or in one of the lounges. The home has a four weekly menu which offers extensive selection and choice at each mealtime of both normal and soft/pureed diet. Residents all spoken to and comment cards received identified that the food is tasty and nutritious. Breakfast is served from 8.30am with staff ensuring that those residents who prefer an earlier breakfast do have one. Lunch is served at 12.45. Tea is served at 17.00. Supper is available at 20.00 although hot and cold drinks and snacks are available throughout the day. Residents choose their meal from the days menu selection for the following day. On the day of the inspection there was: Chicken wrapped in bacon with tarragon sauce with a choice of carrots, cabbage with duchesse potatoes. Jacket potato with cheese and baked beans. Corned beef pie with creamed potatoes and swede and carrots and cabbage. There was cherry and apple strudel or apple crumble for pudding- with ice cream and yoghurt was also available. Special attention is made to the presentation of the food. All food served was excellently presented with finishing touches such as garnishes and would not look out of place at any top restaurant. The cooks are also commended for the way that the “soft” diet is also very attractively presented for dependent residents. Dining tables were attractively laid with tablecloths, appropriate cutlery, flowers and matching cruets and gravy boats to the pretty dinner service in use. Staff were seen to offer discreet assistance to residents cutting up their food and feeding those residents who are totally dependent. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,18 The home has appropriate policies and procedures to highlight concerns and complaints to safeguard residents and protect them from abuse. EVIDENCE: The home has a detailed complaints procedure which is displayed in the reception area of the home and is also in the service user guide. No complaints about the home have been received by the Commission for Social Care and Inspection in the previous twelve months. One complaint was made directly to the home which was found to be partially substantiated, this complaint related to residents moving out of the original building into the new extension. Residents and their families spoken to said if they had any concerns they would not hesitate to discuss them with the Home Manager. The home also has appropriate policies for staff to highlight concerns whilst feeling safe to do so. Comprehensive adult protection procedures are in place and identify appropriate contact with the Police, Social Services and the Commission for Social Care Inspection. It is recommended that the home have a policy available that identifies the process of referring people onto the Protection of Vulnerable Adults list who are not suitable to work with vulnerable adults when appropriate. A large proportion of staff have received training in the awareness of abuse and actions to be take in relation to alleged abuse. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,22, 26 The home is pleasant, homely, well maintained and is furnished to a high standard with no offensive odours. A range of appropriate aids and adaptations are available for dependent people. The home adheres to appropriate infection control procedures to safeguard its residents and minimise the risk of cross infection. EVIDENCE: A new extension to the home was opened at the beginning of April 2005 and is newly decorated with new furniture and furnishings to a very high standard. The original building was not occupied at the time of the inspection as it is being completely renovated and refurbished. The refurbishment is due for completion in September 2005. All areas of the home seen were clean, homely and welcoming. The home has received specialist advice on required aids and adaptations with appropriate equipment available to meet the needs of residents who are accommodated. There is a full staff call system grab rails, moving hoists, lifting belts and a range of pressure relieving equipment, and assisted baths. The new extension has one lounge and one lounge/ dining room. All bedrooms are single, bright and cheerful and meet the size requirements of the regulations. Each bedroom has ensuite facilities of a toilet, hand wash and a level access shower that enables dependent people to be showered without moving out of a wheeled shower chair. Bedrooms seen were adorned with residents’ treasured possessions. The main garden is situated at the back of the home and has been landscaped to ensure that the garden is level and is accessible to the residents. The garden, has a large pleasant patio, mature shrubs, trees and flowers with garden furniture. There are a number of bedrooms available that have french doors onto individual patios and are proving to be very popular. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,29 The home has sufficient and appropriately skilled staff to meet resident’s needs. Recruitment and selection procedures are not robust and do not safeguard residents. EVIDENCE: Staffing levels meet the needs of residents, with additional staff available at busy times. The home is staffed with the following: 07.30-14.00 1 to 2 trained nurses and 4/5 care staff (total of 7 staff) 14.00-21.00 1 to 2 trained nurse and 4/5 care staff (total of 6 care staff) 21.00-07.30 1 trained nurse and 3 care staff Additional domestic, catering and laundry staff are also available. The home has ten of the twenty care staff with National Vocational Qualification (NVQ) level two or above, giving the required 50 of care staff with a care qualification as required by the regulations. In addition there are three care staff with NVQ level three. The staff records of the four most recently recruited staff and were looked at. Recruitment procedures were found to require development to meet the requirements of the regulations and safeguard residents from people who may be unsuitable to work with vulnerable adults. All four new staff were employed prior to the return of either a criminal records check or protection of vulnerable persons check (none were available at the time of the inspection) this could Aldridge Court Nursing Home E55 S20786 Aldridge Court NH V234869 260705 Stage 4.doc Version 1.40 Page 20 allow people who are unsuitable to work with vulnerable people access to the homes residents. Staff files generally did contain two written references although one member of staff only had one employment reference on file. All new staff had completed an application form that was available on their file. There is a need to further explore gaps in employment history which is not currently undertaken. Staff files seen did not include any details of their medical history. A photograph of the staff member was not available in the staff files seen and there was not always any proof of their identity as required. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31 The home is run and managed by an experienced manager, service users benefit from effective leadership, an ethos of choice, respect and high care standards. EVIDENCE: Mrs Neadle is a highly experienced registered nurse who attends ongoing training to update her knowledge and is currently undertaking her National Vocational level 4 qualification in management. Mrs Neadle has been the Home Manager for several years. Residents, relatives and staff all speak highly of Mrs Neadle confirming that that she is very approachable and demands high standards of care for residents. The homes maintenance contracts were all reviewed in April following completion of the new building. Mandatory training is ongoing for all staff with training dates currently identified for further training. The Manager was advised to set up a training matrix to more easily identify those staff that require training updates. STATUTORY REQUIREMENTS This section sets out the actions which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. Standard 33 Regulation 24(2) Requirement The registered person must provide the Commission with a report of the quality survey recently undertaken and make a copy of the report available to service users. Partially met- survey undertaken but a report of the findings has not been shared with the services users until the refurbishment has been completed.- THis requirement should have been addressed by the 30/11/04 Terms and conditions of residency must be made available for all residents and include all elements required by the regulations. The service users plan of care must be comprehensive and identifies all required care, is updated at least monthly or more frequently if clinically indicated. Whenever possible the service user or their representative is included in the review if their plan of care.
