What the care home does well

All service users spoken with were very happy with the service provided. One said that they felt better for being here, another that they would not wish to live anywhere else. Staff describe the team as happy and supportive of each other, and this is reflected in the extremely low turnover of staff. The manager Mrs O Gorman has been in charge for the last 14 years and her skills are praised by staff and service users. There is a warm and friendly atmosphere and dignity and privacy of service users is respected. Residents are encouraged to maintain their existing skills and interests but staff offer appropriate support when this is needed. Staffing levels are appropriate to the needs of service users. Most staff have obtained a minimum of NVQ level 2 in care. Staff recruitment procedures are thorough. The quality of care provided is effectively monitored.

The home has a garden at the front of the property and a car park to the rear. There is a public park opposite the house and the beach is a short distance away. Angelsea Court is registered to provide care and accommodation for up to twenty older persons. There are five double bedrooms. The other bedrooms are single. Communal facilities include lounge, dining room and sun lounge. This is the second of two unannounced inspections to take place in the year March 2005-April 2006. It was carried out by Kathryn Kirk and lasted for two and a half hours. Only standards that were not inspected at the time of the last inspection in September 2005 will be examined on this occasion. As such to gain a more detailed overview of this service this report should be read in conjunction with the one dated 14/9/05. The registered manager, three staff and five service users all contributed to the inspection process. Some documentation was also examined. There were sixteen service users in residence on the day of inspection. No requirements or recommendation have been made and the quality of care provided remains very good. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 and 10 Medicines are handled safely. Privacy and dignity is respected. EVIDENCE: There are policies and procedures in place for the safe handling and administration of medicines. The manager said staff currently administer all medications, with the exception of two people who carry and use inhalers. She said staff monitor their use and are responsible for renewing prescriptions. Medications were seen to be stored securely and tidily in a locked cupboard. The home uses a monitored dosage system and the local pharmacist visits the home to review medication procedures with staff. The manger confirmed that only staff who have received appropriate training administer medicines. The medication administration sheets were examined and were found to have been completed correctly. A record was also seen of medicines returned to the pharmacist. Anglesey Court DS0000011732.V280609.R01.S.doc Version 5.1 Page 10 The manager said that there are currently three shared rooms in use. She said that privacy is respected and that curtains are in place around the beds and the washbasin areas. The manager said that preferred terms of address are recorded in care notes and staff were seen to call service users by their preferred name. Service users said that staff always knocked before entering their bedrooms and confirmed that their dignity was respected. Staff were observed to treat service users in a friendly and respectful manner on the day of inspection. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 Personal preference is respected. EVIDENCE: Service users said that they are able to do as they like, for example they choose when to be alone, when to go to bed and how to spend their time. Service users said that they are able to bring in their own possessions to personalise their rooms. Through discussion with both staff and service users it was evident that independence is encouraged as much as possible. JUDGEMENT – we looked at outcomes for the following standard(s): 18 Appropriate procedures are in place to ensure the safety and protection of service users. EVIDENCE: There are policies and procedures in place regarding adult protection. Service users asked said that they feel safe at Anglesey Court. Staff asked knew what to do in the event of an allegation of abuse being made to them. The manager has undertaken a course as a trainer in adult protection issues and so is able to cascade this to other staff members. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28 and 29 Staffing levels and staff skills are very effective in meeting the needs of service users. Thorough recruitment procedures ensure as far as possible the safety of service users. EVIDENCE: There is a written staff rota. This identifies which staff are on duty during any shift, who is the senior member of staff and the hours worked by domestic staff. On the day of inspection there were three care staff on duty in addition to the manager and one cleaner and one cook. There are a total of three care staff on duty in the afternoons, two care staff are on duty in the evening and one waking staff and one sleeping in staff are on duty each night. Service users said that they felt that there were always plenty of staff available to respond to their needs and said that if they were in their bedrooms and used the call bell, staff always responded very quickly. There is a longstanding staff team. One staff member said that morale is high amongst the staff team. One staff member said that their colleagues are very supportive. The manager confirmed that no staff member is left in charge who is under the age of 21 and that all care staff are over 18. The manager said that of the seventeen care staff employed, fifteen have completed an NVQ level 2 in care. Five staff have continued with their NVQ studies and are doing their NVQ level 3.One member of staff has completed Anglesey Court DS0000011732.V280609.R01.S.doc Version 5.1 Page 15 their NVQ 4 and another is a registered nurse. The manager is an NVQ assessor. The recruitment records of one member of staff were sampled. Their file included a completed application form, two positive references and notification that a CRB check had been completed. The manager had seen evidence of identification and agreed that copies of this evidence, for example birth certificate would be added to the file. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33 35 and 38 The home is very well managed. The quality of service is effectively monitored and evaluated. The health and safety of service users is appropriately addressed. EVIDENCE: The registered manager is Mrs O Gorman. She has been employed at the home for fourteen years. Mrs O Gorman demonstrated throughout the inspection that she was competent and that she is instrumental in achieving the high level of care given to service users. All service users and staff spoke highly of her professionalism and her caring attitude. Mrs O Gorman has completed her NVQ level 4 in management. The home undertakes a quality audit through a contract with a professional company. This takes place once a year. The manager keeps letters from Anglesey Court DS0000011732.V280609.R01.S.doc Version 5.1 Page 17 relatives and one containing very positive feedback was seen. A monthly visit takes place by a representative of the registered provider to review the quality of the home and this is reported to CSCI as required under regulation 26 of the Care Homes Regulations 2002. Service users said that they were consulted about the running of the home and felt that their views were listened to. Some relatively small amounts of money are looked after on behalf of service users. Money was observed to be individually and securely stored There are appropriate written procedures and receipts are kept. One balance checked at random tallied with records kept. The manager confirmed that a staff member qualified in first aid is on duty at all times. COSHH information is available. Service certificates were seen for fire appliances and call system A record of accidents is maintained and this had been completed appropriately.
