St Vincents is a registered care home for 18 older residents who are ex-service men and women. The registered provider is SSAFA Forces Help. St Vincents is a fine period building set in its own grounds on the main Binstead to Ryde road. It is close to all amenities. The home is spacious, well maintained and has attractive gardens and a roof terrace. All 18 rooms are en-suite and decorated to a high standard. There is a large passenger lift providing access to all floors. The home has a mini-bus and a car; these are used to take residents out for activities and appointments. The home is fully accessible. The current scale of charges is from £483 to £574 per week with additional and varying charges for hairdressing and chiropody. What the care home does well The home consistently provides a high level of care to the residents and this is due to the home being very well managed with a good staff team who are committed to continual improvement in the best interests of the residents. The building is very well maintained, spacious, comfortable and well-furnished and set in grounds that are accessible and attractive. Comments received indicate that the home is very good at promoting resident well-being and respecting individuality and choice. This report presents a summary of information about St Vincents since the last inspection of 15 February 2006. Information was received in advance of an inspection visit in the form of comment cards from 5 residents and 8 health and social care professionals who visit the home. This report also uses information supplied by the registered manager in the Annual Quality Assurance Assessment. An inspection visit took place on 20 June 2007 between 11am and 6 pm and the inspector spoke to some of the residents, two visitors, the manager and some of the staff. The inspection visit also included a tour of the building and inspection of some of the home’s records. All of the comments received from residents, visitors and health and social care professionals were very positive about all aspects of the care provided by the home and some of the comments are included in ‘What the service does well’. Residents only move into the home if the manager is confident that the home can meet the assessed needs of the prospective resident. Standard 6 is not applicable, as the home does not provide intermediate care. EVIDENCE: All new residents receive a full comprehensive needs assessment carried out by the manager or senior staff with sensitivity and respect for the important decisions prospective residents need to make when moving into a care home. Residents are encouraged to visit as many times as they would like and to stay for a trial period, visit for meals, and to meet the other residents. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. Residents in the home receive effective personal and healthcare support and staff respect residents individual personal needs and preferences when providing care. The systems for the administration of residents’ medication are well managed and safe. EVIDENCE: Individual care plans are well documented and reviewed in consultation with each resident and their key worker. Evidence that the home is good at meeting health care needs is provided in comments from some of the health and social care professionals who visit the home: “They manage complex medical needs well and are excellent at supporting the clients in managing their own conditions as appropriate” “We enjoy an excellent working relationship with the home” Comments from residents in the comment cards and during the inspection also confirm that they are very satisfied with the way that personal care and support is provided. Residents’ comments, and observation of practice in the home demonstrate that staff respect privacy and dignity at all times. The storage of medication is well organised and safe and there are systems in place to ensure that all staff follow the home’s policies and procedures for the safe administration of medicines. Residents are supported to manage their own medication where this has been assessed and agreed with individual residents. The home arranges training for staff on health care topics that relate to the health care needs of the residents. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15 Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. The home has sought the views of the residents and considered their varied interests when planning and arranging activities both in the home and in the community. Residents are also consulted on menu choices and emphasis is placed on providing choice and quality of meals, served in attractive and homely surroundings. EVIDENCE: Residents meetings are held on a regular basis that gives residents the opportunity to make suggestions and comments about the kind of activities they would like in the home. In addition, the manager and staff have a creative and innovative approach to ensuring that the lifestyle in the home meets residents’ expectations and interests; an example of this is the gallery on the first floor that is dedicated to memorabilia from all of the armed services, particularly relevant as the residents are all ex-service men and women, and the residents have all contributed to the very well presented display. Residents also told the inspector that the home plans to purchase a special back chair and adjustable washbasin suitable for hairdressing – in response to requests from the residents. This facility will be installed in one of the new bathrooms in the extension currently under construction. The home St Vincents DS0000012538.V338736.R01.S.doc Version 5.2 Page 13 has two vehicles for use by residents, and one is wheelchair accessible and there are dedicated drivers who are able to take residents out shopping, on outings or to appointments, with a member of staff if required. There are regular consultations with residents about meals and the manager was in the process of reviewing the menus to include particular requests and choices following a residents’ meeting. Comments and observation confirm that meals are well planned and presented and residents have lots of choice, and the quality of the food is good. There is a bar in the sitting room with a lunchtime ‘Happy Hour’ and residents can also choose to have a drink with their meal. During the inspection visit, discussion with two visitors demonstrated that the home provides a friendly welcome for visitors of residents. The gardens are accessible and there is a well set out and tended memorial garden and also a roof garden terrace on the first floor. There are plans to landscape the rest of the garden when the current building work is completed. Residents in the home are able to express their views and have access to a complaints procedure and are protected from abuse and have their rights protected. EVIDENCE: There have been no complaints about the home but there is a written complaints procedure that it available to residents and their visitors and residents who completed comment cards confirmed that they would know what to do in the event of any concerns or complaints. The manager is committed to ensuring that residents’ legal rights are protected and that residents are able to make informed choices wherever possible. The manager would arrange access to advocacy for any resident who did not have the capacity to make informed decisions. The home has a wide range of information and self-help leaflets and has made links with the National Charity ‘Help the Aged’ who provide a comprehensive range of information and advice to older people. It is planned to develop a dedicated ‘Residents Information Area’ when the building renovations are complete. Residents who have a sight impairment are supported by staff to discuss relevant information or have information read to them. In the Annual Quality Assurance Assessment the manager stated, “We take the protection of our service users very seriously and ensure that service users are protected from abuse”. There are procedures in place to respond to any St Vincents DS0000012538.V338736.R01.S.doc Version 5.2 Page 15 suspicion or evidence of abuse or neglect and staff receive training in this topic “so that they have an understanding of this area of their responsibility”. Records show that the home has a thorough and robust staff recruitment procedure that is designed to protect residents. Practice in the home is regularly monitored and staff are well supported and supervised to ensure safe working practice at all times for both residents and staff. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 23, 25 and 26 Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. The building is a fine period detached house and the home has managed to preserve the unique period features but also ensure that the home is fully accessible for the particular lifestyle and needs of the residents. The living environment for the residents is homely, clean, safe and comfortable and well maintained. EVIDENCE: This judgement was confirmed from observation and from comments from residents and visitors. Particular attention is given to ensuring that the home environment is safe, secure and comfortable. The home is very well decorated and furnished and well maintained both internally and externally. There are aids and equipment in place necessary to support any residents who have limited mobility. The home has a very large passenger lift that comfortably accommodates several wheelchairs and gives access to all floors. St Vincents DS0000012538.V338736.R01.S.doc Version 5.2 Page 17 The inspection visit was unannounced and a tour of the building found every area to be clean, tidy and pleasant. Residents and visitors confirmed that the home is “always very clean and tidy”. Residents enjoy a choice of communal space both inside and outside with a large and attractive sitting room, dining room, entrance and hallway, well maintained gardens with seating and a roof terrace garden with seating and sun shades. All bedrooms are single with en-suite facilities and in addition there are two bathrooms and a shower room with assisted facilities. Residents have keys to their bedrooms. The inspector spoke to one resident in their own room and observed that the room had good natural light, was clean and comfortable and well furnished with sufficient space for a table and chair and an armchair as well as bed, bedside table and light, and storage space. The en-suite bathroom was spacious and clean. Decoration, fittings and furnishings were of good quality - matching and colour co-ordinated. The resident had an alarm call with a cord within easy reach of the bed and confirmed that it worked. Residents are encouraged to bring as many of their own possessions as possible when they move into the home. The resident enjoyed sitting in the armchair with a good view of the garden from the large windows. Every toilet, bathroom and en-suite bathroom is fitted with liquid soap and paper towels to promote good hygiene and it was evident that there are policies, procedures and equipment provided for staff to ensure good hygiene and the control of infection. Residents were fully consulted before the current building work began and the manager explained that she has taken all necessary steps to ensure the safety of the residents and staff at all times whilst the work is in progress. The builders have separate access and all areas are clearly marked with signs and hazard warnings both inside and outside. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 and 30 Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. There is consistently enough staff available to meet the needs of the residents in the home. The home puts a high level of importance on training and development for all staff to ensure they are supported through training to meet the needs of the residents. EVIDENCE: The manager ensures that all staff have regular supervision and an individual training programme based on their area of work, this includes care staff, catering staff, cleaning staff, administrative staff, and maintenance staff. The inspector had a group discussion with some of the staff, in the training room. It was evident that the manager and staff recognise the benefits of a skilled and trained workforce and encouragement and resources are available for staff to undertake both ‘in-house’ training and external qualifications such as the National Vocational Qualification (NVQ) in care. When staff have completed a course of training, there is often an ‘award ceremony’ when certificates are received, involving the residents as well who are interested in the training that staff undertake. The manager was clear that all training had to be of a high standard and relevant to the home and the manager for suitability and value initially assesses any training. There are good and thorough and well documented systems for recruiting new staff with a commitment to equal opportunity in practice and the protection of St Vincents DS0000012538.V338736.R01.S.doc Version 5.2 Page 19 the residents. All new staff follow a thorough and comprehensive induction programme whilst shadowing more experienced staff. Staff have clear job descriptions, work contracts and supervision contracts. All of the home’s recruitment, training and supervision records are in good order and securely stored. The home regularly reviews aspects of its performance through a good programme of self-review and consultations, which include seeking the views of residents, staff, relatives and other visitors. EVIDENCE: There is strong evidence that the organisation of the home is open and transparent. The views of both the residents and the staff are listened to, and valued. Comments from residents, staff, visitors and health and social care professionals confirm that the home provides a consistently high quality service. All working practices in the home are safe and there is good monitoring and spot checks in place to ensure that policies and procedures to promote and protect residents and staff are followed. The home monitors its health and safety performance, consults other experts and specialist agencies about health and safety issues as required. Evidence of this is provided in the five star quality rating from the Environmental Health Food Safety inspection, and an excellent Fire Safety inspection report. There is also evidence of organisational monitoring by the management group of SSAFA.
