Date of last inspection Brief description of the care home Appleby is a 55 bed care home with nursing. It provides care for older people with enduring mental health problems. The home is owned and managed by Southern Cross Healthcare Limited a large national provider of services for vulnerable client groups. Care in the home is provided by Registered Nurses supported by care staff. The home is situated in North Shields in North Tyneside close to local shops and good public Care Homes for Older People Page 2 of 8 transport links. The building is comprised of two floors with 55 single bedrooms all with en-suite facilities. Each floor has separate lounge, dining areas, bathrooms and additional toilet facilities. The home also has a kitchen and laundry room. There is a garden and patio area to the rear of the home. This unannounced random inspection was carried out over a period of three hours starting at 09:30 Monday 29th September 2008. The manager Ms Hodgson was on duty and she assisted with the inspection supplying information as necessary. This was an inspection as part of the commissions programme to visit a small number of services who have achieved a 2 star/ good rating within the normal plan of inspection. However, the commission has also received a concern regarding some general issues and although this has not been investigated as a complaint they have been included in the areas looked at. What the care home does well:
The care plans looked at were detailed and up to date. They are currently being improved to make them more person centered and involve the care staff more in their completion. The staff were working hard to assist the residents in their day to day lives and they were helping them with their personal care and meals. The residents were clean and appropriately dressed for the activities they were involved in. Staff were courteous and friendly and residents were positive in the way they reacted to this. Meal times were calm and well organised and the residents had appropriate crockery and cutlery. They have a choice of cups to have their beverages from and these included both mugs, tea cups and feeding cups depending upon the manual dexterity and choice of the individual residents. There is an activities co-ordinator employed on a full time basis. She has organised a number of opportunities for the residents including painting and crafts as well as visiting entertainers. Photographic evidence of which is available on the walls of the corridors. Improvements have been made to the garden area which is still ongoing. Raised beds have been built and these are to be used for growing vegetables and herbs. Fruit trees are to be planted in some of the areas. There has been a recent period of time, for approximately one week, when the access to the garden was restricted as the home admitted an individual resident who was at risk if given free access to it. This has now been resolved. The home continues to organise the café teas which are popular with the residents, on occasion these have needed to be canceled but this is not frequent. The staff were following practices to reduce the risk of cross infection by hand washing and using disposable towels which were available as necessary. Incontinence pads were being placed in bags prior to being disposed of in the clinical waste bags. This reduces the odour and reduces the risk of cross infection. There are chairs situated at various points along the corridors which are placed for the use of the residents. There has been some concerns expressed that they would present an obstruction in the event of a fire. Advice was being taken from the fire service regarding this and other issues. At the time of the visit the staff on duty were, the Manager, two qualified nurses, seven care staff, the administrator, handyman, activities co-ordinator, two domestic staff, two catering staff and a laundry assistant. Page 4 of 8 the morning. The staff were working well together and there was no suggestion of the residents being hurried or not being supported safely.
