The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home has a very friendly atmosphere with both staff and residents happy and chatty. Residents and visitors were very complimentary about staff and the home with comments such as "There`s really nothing to complain about", and "I`m comfortable". Food provision is good with favourable comments from staff and residents, such as, "Very nice food, plenty of it". The caring staff team attend sensitively to residents needs. The home has a very experienced and skilled manager, who cares deeply about the residents. There is a spacious and secluded garden for residents to access.

What has improved since the last inspection?

One bedroom carpet has been replaced and the hallways have been decorated. The toilets have new surrounds. All required and recommended actions identified at the last inspection have been carried out.

What the care home could do better:

Bedroom doors must not be wedged open. Sharps boxes need to be regularly cleared. Dripping taps need to be reviewed in some bedrooms. Recruitment records need to be in place for all staff. Annual appraisals to be started for all staff. Staff need to receive training on infection control.

There is a spacious garden surrounding the home and car parking space. The accommodation is arranged on two floors, comprising of two single bedrooms on the first floor and eighteen bedrooms on the ground floor. The first floor is accessible by stairs. Communal rooms comprise of a large lounge/dining room and a smaller lounge. The home is owned privately by Dr H and Dr C Condon and managed by Mrs E Davitt. This announced inspection was carried out by one inspector. The inspection lasted a total of six hours. The inspector talked to residents and carried out a tour of the home and gardens. The manager and staff on duty were talked to and records were looked at. All residents were happy to talk to the inspector. As preparation for the inspection, the previous inspection report was read and comment cards were sent out. Three comment cards were returned from residents, twelve from visitors/relatives, and one was returned by a social worker. All comments were extremely positive and included, “This is a lovely caring well run home”, “Matron and staff are friendly and professional, and go out of their way to ensure client needs are met”, “A very friendly home”. There have been no complaints or concerns raised to the Commission about this home since the last inspection. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 2, 3, 4, 5, 6 Prospective residents receive clear information to enable them to make a choice about whether or not they wish to live in the home. Residents receive a thorough assessment before moving in to the home, to ensure the home can meet their needs. EVIDENCE: The home’s Statement of Purpose and Service User Guide clearly state the facilities and care the home can provide to enable residents to make a choice as to whether the home can meet their needs. All residents receive a welcome pack before moving in to the home, which includes information relating to the home and a contract setting out terms and conditions. Those contracts read had been signed by the residents on arrival to the home. The documents are held on file for the duration of their stay. These assessments include resident’s likes and dislikes and health assessments. Visits to the home are encouraged prior to admission and a trail visit can be arranged. The last inspection report was accessible for any one to read. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9 Residents who live at April Cottage have their health and personal care needs met. Medication administration is sound. EVIDENCE: The inspector read six care plans. Care plans set out health, personal and social care needs ensuring staff deliver the correct support and care required by individual residents. Plans seen were very comprehensive and well maintained, being reviewed every month by the manager. Residents spoken with stated they felt they were well looked after and received the care they needed. Risk assessments were in place for individual residents, covering areas such as moving and handling, and for the building, e.g. kitchen and stairs. Records seen showed that community health care professionals support residents, as needed, e.g. district nurses, the wheelchair service, hospice, chiropodist and G.P’s. Other medication is stored appropriately and sound procedures are in place. The manager has just written guidelines to follow in case of a heat wave. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 13, 14, 15 Residents are provided with a variety of activities. Contact with family and friends is encouraged. Food provision is good. EVIDENCE: All residents spoken with stated their friends and family are able to visit at any time ensuring important contact is maintained. This was confirmed by a visiting relative, who said, “All of the family are made so welcome and I can visit any time I like”. On the day of the inspection the activities co-ordinator came to spend time with residents, offering them activities to participate in, if they wished. The food provision is of a good standard. On the day of the inspection lunch was braised beef, mashed potato, mixed vegetables and broccoli, followed by chocolate pudding and chocolate sauce. One resident chose to have bananas and ice cream for desert. One resident said, “The food’s very, very nice” The manager said that two residents choose to have early breakfast, but they are all offered a mid-morning snack. Residents are able to make choices about their lives. A visiting relative said, “No-one prohibits them from doing what they want to do, they have choices”. Another visiting relative said, “We do have a laugh at this home, it’s not regimented, I wouldn’t complain, it’s wonderful”. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, 18 Residents are protected from abuse. There are clear processes for complaints for residents and visitors to use if necessary. EVIDENCE: Residents stated they would feel happy to complain to staff if they had any concerns. No resident voiced any concern during the inspection. The home has an adult protection policy and records showed no incidents to be recorded. The Commission has not received any complaints or adult abuse enquiries since the last inspection. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 22, 23, 24, 25, 26 Residents live in a well-maintained environment, which is safe and clean and meets their needs. EVIDENCE: On arrival at the home, the inspector saw one resident enjoying a leisurely walk around the garden. The garden has a patio area, a covered pond with fountain and goldfish, lawned areas, flowerbeds and garden furniture. The garden is quiet and secluded. The inspector saw a wood pigeon and a blackbird come to drink from the fountain. Aids and adaptations are in place to assist residents getting around the home and accessing toilet and bathing facilities. The interior and exterior of the home are in good decorative order. The lounges were comfortably furnished and homely. All bedrooms were clean, tidy and comfortable. April Cottage H60 H11 S14364 April Cottage V225462 210605 Stage 4.doc Version 1.30 Page 16 The inspector saw two bedroom taps were dripping and this was brought to the manager’s attention. One resident said, “Every day my room’s cleaned”. One resident told the inspector “I’m not used to the (bedroom) door being shut, it’s usually wedged open, the wedge has been taken away today”. This was discussed with the manager, who confirmed that wedges are sometimes used. The manager must review this practice to ensure the health and safety of residents is uppermost. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 29, 30 Residents are cared for by a dedicated manager and staff. Staffing levels suit the needs of residents. EVIDENCE: The home was seen to running smoothly and efficiently. A staff rota showed staffing levels to be consistent with no gaps. No resident commented on a lack of staff at any time. The inspector noted very good communication between staff and residents during the inspection with residents feeling at ease with staff. Staff were observed to be very caring and sensitive and called residents by their preferred names. One resident said that a member of staff “really is tops” and about the other members of staff, “They do their best to keep us happy”. Staff records for six staff members were read. There were no recruitment records in place for one person. The manager needs to ensure that the recruitment procedure is followed for all staff, not only those staff that are employed in a caring role. All staff have attended training in moving and handling, first aid and medication. Some staff have received training in infection control and safety compliance. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 33, 36, 38 An experienced manager runs the home. The home is very resident focused. EVIDENCE: The manager is very experienced and dedicated, having been in post at April Cottage since August 1990. She has been a registered nurse since 1972, and undertaken subsequent relevant training. The manager makes a point of spending time with the residents and was noted to be very knowledgeable about their needs and routines. She was seen to interact extremely well with residents, staff and visitors. One resident commented, “Matron’s a dear”. No annual appraisals are held. The manager said this is because everything is covered in the regular supervision sessions. In addition to the supervision of staff, it is recommended that they receive annual appraisals. The manager needs to ensure that all staff receive training in infection control. The three sharps boxes were seen to be full and this was discussed with the manager, who confirmed that they were due to be dealt with by a new contractor. A quality assurance folder is held with questionnaires that had been completed by residents, visitors and relatives. The manager has collated all the information and made a pie chart for ease of reading.
