This inspection was carried out on 5th April 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home has good assessment and admission procedures which involve residents and their families, and visitors are encouraged. There is a sound staff recruitment process and staff are well trained and able to meet residents` needs. The standard of care is good and the home has a happy and lively atmosphere. Residents enjoy the homely atmosphere and environment.

What has improved since the last inspection?

The refitting of the kitchen and provision of an improved level of equipment has enabled a wider choice of meals to be offered to residents. The manager also explained that staff now spent less time undertaking domestic duties and had more time to spend directly with residents.

What the care home could do better:

The manager was asked to ensure that medicines are securely stored at all times. Whilst the redecoration of the office is taking place the temporaryarrangements for storing confidential records need to provide sufficient security.

It provides residential care for up to 20 elderly residents, some of whom may have dementia. The home is on ground and first floors and there is a chairlift between these. There are a variety of aids and adaptations to allow residents to move about more independently. Eighteen of the bedrooms are single, two of these having an en suite toilet. There are three communal toilets and a bathroom on the ground floor, and three toilets and two bathrooms on the first floor. There is a garden to the rear of the building. Ashley Arnewood Manor Version 1.10 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. This inspection took place over two days. On the second day four hours were spent visiting the home. The opportunity was taken to look around the home, view records and policies and to talk to three staff and several residents, three of whom were spoken with at length in private. There were several visitors to the home at the time of the inspection including a district nurse. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3, 4 and 6. The admission process is well managed with an assessment of needs being done to ensure these can be met. Residents are given clear information regarding the service. EVIDENCE: A resident recently admitted for intermediate care said that she knew which room she was to have when entering the home, and was hoping to return to her own home in the future. Three care management assessments, made before the residents concerned were admitted to the home, were examined and contained a good level of detail. Residents have the opportunity to read and sign these if they wish. The service users guide given to residents at the time of admission includes a quality assurance form. Ashley Arnewood Manor Version 1.10 Page 9 Health and Personal Care The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9 and 10. Residents are looked after well in respect of their health and personal care needs. Further attention needs to be given to the security and storage of medicines. EVIDENCE: Care plans contain a good level of detail, and are signed by residents if they wish. They include information regarding access to specialist health care staff, such as a dentist and an optician, and are reviewed every month. During the inspection a district nurse was attending to a resident’s health care needs in private. The manager took immediate action when a lack of security of storage of medicines was pointed out. An item that had been placed on top of the drugs cupboard was put in it. Observation during the inspection showed that staff have a good awareness of how to protect residents’ privacy and dignity. They were seen to knock on Ashley Arnewood Manor Version 1.10 Page 10 doors and await a response before entering and spoke to residents in a respectful way. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) Outcomes for this group of standards were not inspected on this occasion. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 22, 23, 24, 25, and 26. A comfortable and safe standard of accommodation is provided for residents. EVIDENCE: The home is well maintained and suited to residents’ needs. It is decorated and furnished to a standard that creates a comfortable and homely ambience and there is a programme of redecoration and refurbishment in place. There is a chairlift between ground and first floors. There is a lounge with a dining area on the ground floor, another lounge with T.V., and further small sitting room which may be used to entertain visitors. The home’s kitchen has been subject to a complete refurbishment and a dishwasher has been provided. Staff spend less time on domestic duties and as a result a wider choice of meals for residents is now provided. One resident said ‘ the new handrail is useful’ and another said ‘the radiator covers are handy for putting pictures on’. Residents’ bedrooms looked homely, Ashley Arnewood Manor Version 1.10 Page 14 as they had been able to bring items of their own furniture and possessions with them. All areas of the home smelled pleasant and were cleaned to a high standard. Ashley Arnewood Manor Version 1.10 Page 15 Staffing The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 and 29, Staff at the home are well trained and supported and employed in sufficient numbers to meet residents’ needs. EVIDENCE: One resident said ‘there’s always someone to help you’ and another said ‘the staff are nice’. Staff were observed during the inspection to be providing assistance to residents in a calm and unhurried manner. The staff rota indicated there to be four carers on duty and a staff member said that generally there were enough staff on duty. Three staff records examined indicated a sound recruitment process which included the obtaining of references. New staff undertake induction training, and there is a training record sheet for each staff member. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 37. Residents’ best interests are safeguarded but attention needs to be given to ensuring confidentiality of records. EVIDENCE: There is a Quality Assurance policy together with a comprehensive Health and Safety policy. During the renovation of the office records had been moved to a less than secure area of the home.
