Conditions of registration: The conditions previously applicable to the service have been reviewed as part of this inspection, and it has been agreed with the proprietor that they will now be removed Date of last inspection 1st February 2005 Brief Description of the Service: Askham Place is a purpose built single storey home providing nursing and personal care for up to 15 service users aged between 18 and 65 with a physical disability. The home was first registered in September 2001, and is close to the small Cambridgeshire village of Doddington near March. The home is in the grounds of Askham House, a service for older people under the same ownership as Askham Place and shares kitchen and laundry facilities. Since the last inspection, the a new registered manager has been appointed and approved by the CSCI Askham Place I53 I03 S24299 ASKHAM PLACE V223222 030505 STAGE 4.doc Version 1.30 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. This was an unannounced inspection carried by the allocated inspector, and Cathryn Bramham, regulation manager. It was the first inspection of the current year, and concentrated on reviewing the conditions applying previously to the homes registration, as well as looking at the requirements from the last inspection. The inspection lasted about two hours, and during this time the inspectors reviewed some of the homes paperwork, and spoke to 2 of the staff working that day. The new manager was not at the home on the day of the inspection due to other commitments, but the homes administrator and the manager from Askham Place were available and provided the information required. Service user and relatives comment cards were left with the service to be completed and at the time of writing the report, comments had been received from most of the service users and from one relative. What the service does well: What has improved since the last inspection?
Since the last inspection, the new manager has made a number of changes to the way that the home is run, and new staff have been recruited. The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2, and 5 Prospective service users receive detailed information which enables them to make decisions where to live. They are fully informed of terms and conditions that apply. EVIDENCE: The administrator confirmed that the statement of purpose had been revised since the last inspection, and a copy was on the wall in the entrance. A copy of this will be sent to the CSCI for their information. Individual service users files were examined for two service users, and these contained details of the terms and conditions applicable as well as detailed pre admission assessment information. The intended outcomes for Standards 6 – 10 are: 6. 7. 8. 9. 10. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6,7 and 9 Service users all have detailed care plans which provide information to staff on their care needs. EVIDENCE: Care plans were examined for two of the service users living in the home, and these were detailed, with evidence of regular reviews. The care plans showed that the service users and their families where appropriate are involved in the care plans, and the care plans provide detailed information. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15 and 16 Service users are able to take part in a variety of varied activities, and be part of the local community. A varied menu is available, and service users can choose from at least two options where this is appropriate. EVIDENCE: During the inspection, the home staff commented on the wide range of activities available to the service users, and this was recorded in the care plans looked at during the inspection. Service users take part in activities including swimming, bowling, shopping, visits to the cinema and meals out as well as activities and board games in the home. An activities worker works four days a week, sometimes at weekends with support from the care assistants who often accompany the service users on the activities. Askham Place I53 I03 S24299 ASKHAM PLACE V223222 030505 STAGE 4.doc Version 1.30 Page 11 Activities normally take place either individually or in small groups, with occasional larger group activity, and the home has its own transport available, although it was suggested an additional vehicle would allow even more variety in the outside activities arranged. During the inspection, the chef indicated the range of foods available, and the choices given to the service users, although several service users require peg feeding, and are not therefore able to make this choice. The kitchens also provide for the residential home next door, owned by the same company. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18 and 19 Support and care is provided in an individual way to meet the health, physical and emotional needs of the service users. EVIDENCE: From the care plans examined and discussions with staff, it was clear that service users receive individual support, and their healthcare needs are met together with emotional and physical needs. The inspectors discussed the detailed care received by one of the service users, and the support she receives in maintaining contact with her partner and children. The service users living in this home have some very complex health and physical needs, but these are meet. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22 Procedures are in place to ensure service users and their representatives know how to comment or complain, and that their views will be listened to. EVIDENCE: The home has a detailed complaints policy, but has received no complaints since the last inspection. A log of complaints is kept, and all complaints would be recorded in the log and notified to the CSCI. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24,29 and 30 The home offers a safe and comfortable environment to live in, with suitable aids and adaptations where required. EVIDENCE: The home is purpose built, and offers and extremely comfortable setting for the service users living there, with large communal facilities. All the bedrooms have ensuite shower rooms and ceiling tracking linking the bedroom and bathroom, ensuring moving and handling is done safely and with as much dignity as possible. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33, 35 and 36 The staff team are well trained and appropriately supervised to ensure the needs of service users are met. EVIDENCE: Since the appointment of a new manager there have been some changes to the staff team, and from discussions with two members of staff these have had a very positive effect. All staff are now receiving supervision on a regular basis, carried out by either the manager or for domestic staff the housekeeper. The staff spoken to indicated that regular training is received, and in the last few weeks, they have received training in moving and handling, first aid, infection control and health and safety, and a session on peg feeding is due shortly. Service users benefit from competent and accountable management of the service. The Commission considers Standards 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 39 and 42 Service users receive a specialist service from the home, with care taken to ensure all health and safety issues are met. EVIDENCE: The home appears well run, particularly since the appointment of the new manager, and considerable efforts are being made to ensure that the views of service users and staff are listened to. Health and safety issues are suitably dealt with and relevant training received. A more detailed look at the records held by the home will take place at the next inspection.
