What the care home does well

The residents who spoke with the inspector said they were happy living at The Avenue and made comments such as, `they try to make it a home` and, `you`d have to go a long way to do any better`. Another resident said they were happy at the home. All residents have their needs assessed before they move in, and their needs set out in a detailed, person centered care plan. Staff are trained in administering medication and can support residents who wish to keep their own. Residents enjoy the range of activities, particularly trips out, and told the inspector that they could get up when they liked. The manager continues to maintain the environment to a high standard.

What has improved since the last inspection?

There were not any areas for improvement identified at the last inspection.

What the care home could do better:

The manager and staff need to have a clearer understanding of the role of Social Services with regard to the Protection of Vulnerable Adults, and what they should do if there was an allegation or suspicion of abuse.

Date of last inspection 18.11.04 Brief Description of the Service: The Avenue Rest Home provides residential care for up to 24 older persons, including those with dementia and mental disorder. The home is near to the town of Fareham, with a bus stop outside. The building is a detached property with grounds. There are two stairlifts and a passenger lift. There is a car park at the front of the house and there are well-maintained gardens at the rear and front of the house. Maintenance and decoration of the home is of a high standard. The inspection was the first unannounced one of the year. The inspector spoke individually with three residents, and the two care staff, together. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 Standard 6 does not apply to The Avenue. The manager ensures that no one moves into the home without an assessment of their needs. EVIDENCE: The inspector viewed two recent pre-admission assessments, and found them to be thorough and containing the necessary information. The manager will undertake assessment where it is most appropriate, and the two new residents had visited the home for their assessments. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 & 9 The manager ensures that all residents have their needs and wishes set out in a plan of care, and that medication is administered correctly. EVIDENCE: Residents have their needs and wishes set out in a plan of care. These plans were found to be detailed, stating, for example, the times residents liked to get up, go to bed, social interests, what sort of help was needed with personal hygiene, etc. Staff told the inspector that they used the care plans on a daily basis, particularly for recording any changes. The inspector looked at the medication trolley and checked the associated records, which appeared to be in order. The local pharmacist inspects the medication about twice a year and had recently visited. The inspector saw the report an there were no issues of concern noted. The majority of staff are currently undertaking a modular medication course which is assessed through Protocol Training. Staff explained the procedure they followed when administering medication, i.e. signing the records after tablets had been taken. Some residents are responsible for their own medication, and this is kept in a locked drawer in their rooms. GPs review medication every six months. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 The lifestyle of the home is relaxed and residents can decide what activities to join in with. EVIDENCE: Some of the residents recently went on a trip to the New Forest, to see deer and have afternoon tea at Rhinefield House. The trip was funded by the manager, and two people who spoke about the trip with the inspector thoroughly enjoyed it. The trip was described as, ‘well organised’ bringing a ‘touch of magic’. The inspector was also told about other trips to Lee on Solent and Gosport Pier. Other activities include facials and manicures, bingo, reminiscence games, ball games, visiting singers, exercise sessions, old films and craft sessions. Two residents who were asked, said that their mornings were relaxed, one said that they had breakfast in bed, and went downstairs just before lunch. Care plans showed what time people liked to get up and go to bed. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 & 18 The manager’s approach ensures that residents feel able to complain. The lack of understanding of adult protection procedures means that residents may not be fully protected in the event of an allegation or suspicion of abuse. EVIDENCE: The inspector spoke with three residents about the complaints procedure and they all felt that they could talk to the manager, and that he would do something about it. The home has a complaints policy which states that an initial response will be given within 48 hours. Both staff members said that they would tell the manager if a resident made a complaint. In the event of an allegation or suspicion of abuse at the home, staff said they would report this to the manager. When asked what they thought the manager would do about it, they said he would investigate further. The inspector asked the manager what he would do, and he said it would depend on whether the person had dementia or not (with regard to how reliable the accusation was). This issue was discussed at the inspection on 19/4/2004 and the procedure subsequently amended. The inspector advised the manager that that he must always refer allegations or suspicions to Social Services, who will make the decision whether to investigate or not, and what form the investigation will take. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 & 26 The manager ensures that residents live in a clean, safe environment, but the laundry may benefit from having a hot water supply. EVIDENCE: One of the residents said that there were, ‘comfortable, clean beds’ at the home, that they liked their bedroom, the communal rooms were, ‘comfortable’ and that the white paint had been redecorated. Another resident said they had a, ‘pleasant room’ and that the lounge was decorated for Christmas and birthdays. The manager told the inspector that one bedroom had been fitted with a new carpet and that he was ‘working through’ the white paintwork. The home is maintained to a high standard. There are three separate lounge/dining rooms, each of which appealing to people with different needs. The conservatory area is in the central lounge, and acts as a link between the house and garden. The home has well maintained gardens comprising of a lawned area, trees, rose beds, shrubs and seating. The manager has replanted the border in front of the house and placed some seats, which he says residents enjoy. Bedrooms are also decorated to a high standard. The sink in the laundry does not have a hot water supply and the manager believed that it was not necessary for there to be one. The inspector has referred this to Fareham Borough Council’s Environmental Health department for their consideration. JUDGEMENT – we looked at outcomes for standard(s) 28 The home’s commitment to NVQ training means that residents are in safe hands. EVIDENCE: The home has a high commitment to NVQ training and nine out of fourteen staff have achieved NVQ2, and a further four are due to start. This figure exceeds the standard of 50 of care staff needing NVQs and is seen as good practice.
