What the care home does well

The Avenue provides residents with a lifestyle that allows for independence, choice and personal development. Residents said, "We make decisions and have control over our daily life". Another resident said, "I get up at 6am, I don`t have to, it is my choice, I can go out when I want, all I have to do is let the staff know". A resident also said, "we are kept fully aware of all events of the home, for example when we are getting a new washing machine or the electrician is visiting". Residents` records are well written and they are fully consulted about their assessed needs and care plans, which they sign and agree to. Healthcare needs, both physical and psychiatric needs are well met by the home, resident said, "if you are poorly, this is recognised and responded to appropriately". There is a good range of social and recreational needs, which are very much individualised. Residents are fully consulted about life and decisions of the home.

Details of the financial viability were received and confirmed that the situation was fine.

What the care home could do better:

The garden is in need of further development to ensure that it is safe and accessible for all residents. One of the hot water boilers is in need of repair or renewal so that resident can have a hot bath whenever they want to.

It has been operating for approximately 22 years and is a converted Victorian property close to the centre of Linthorpe in Middlesbrough. There are nine single bedrooms and two double bedrooms none of which have ensuite facilities, however the rooms do meet the size requirements. This inspection was an unannounced inspection, which took place over one inspection day, three and a half hours of inspection time in total. The inspector had discussion with the manager during the inspection process. A small number of records were examined; including, two sets of resident’s records and minutes from residents and staff meetings and there was also informal discussion and indirect observations. Ten of the thirteen residents were spoken to and were very happy to share their views of the home and the impact on their lives with the inspector. This was a very positive inspection and the inspector was warmly welcomed to the home by all. What the service does well: What has improved since the last inspection? JUDGEMENT – we looked at outcomes for standard(s) 2 It is evident that assessment of resident needs and aspirations are paramount to any admission to The Avenue. EVIDENCE: There had been no new residents admitted in the last three years, however reflecting on previous evidence and through discussion it was reaffirmed that individual needs are assessed prior to admission and thorough the trial basis and throughout life with The Avenue. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6, 7, 8, 9 Residents are well aware of their assessed needs and care plans, they are active in decision making both on an individual basis and in aspects of life in The Avenue, as a result their independence is promoted. EVIDENCE: Two sets of residents’ records were examined during the inspection and they continue to be extremely well written and contained detailed information about the individual residents, their assessed needs and lifestyles. They contained evidence of resident involvement with their records and they had signed the assessment and care plan documentation. The assessments and care plans examined had been reviewed and updated and there was evidence of regular evaluation of care. They were truly reflective of the individuals’ current situation and needs. There was discussion about residents meeting and also weekly menu meetings, which residents found extremely useful. Resident meeting minutes were made available for examination and included details of expenditure decisions for the home, such as the purchase of a new central heating boiler. It also included details of activities and holidays and it was evident that there was much consultations. Residents records also contained details of choice and decision making and recording in one residents file was, “he has chosen to share room with other resident”. Risk assessments were also contained within resident’s records and residents confirmed through discussion that they were able to take risk, which increased their independence. This included going out of the home, assisting in the kitchen, taking medication and doing their own washing and ironing. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11, 12, 13 , 14, 15, 16, 17 The lifestyle of residents within The Avenue is very much individualised and is underpinned by appropriate social and recreational activities as well as the opportunities for personal development. Residents’ benefit from maintaining personal relationships and have their right respected. Meals are provided to a good standard within a suitable environment. EVIDENCE: An examination of resident’s records demonstrated that recreation and social activity plans were in place, which had been signed up to and agreed by the residents. One resident spoke enthusiastically about the computer course they were completing and said, “I am really enjoying the computer course.” During discussion about involvement with the local community, residents did not see that their home was any different from any other home in the local community and spoke very positively of neighbourly relationships. One resident said, “there is no stigma at all, this home just blends into the community”. A number of resident were attending day care facilities, which they said they enjoyed. They also accessed the local churches and shops. Residents spoke enthusiastically about their holidays, a number of residents are going to Llandudno and one resident is going to Southern Ireland. The residents had particularly enjoyed a trip in Limousines and a glass of champagne. Residents spoke about maintaining contact with friends and family and said that this was positively encouraged. One resident said, “I go and stay with my daughter every few weeks for the weekend”. Other residents spoke of visits and maintaining contact via the phone. Residents firmly believed their rights were respected and confirmed that they had voted at the most recent elections. Residents confirmed that they were actively involved in the menu planning and said they meals were very good. They confirmed that there was fresh fruit and vegetables and also said that they could access the kitchen and make snacks if they wanted to. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18, 19, Healthcare needs and personal care needs are well met at The Avenue. EVIDENCE: Residents confirmed that there physical, emotional and psychiatric needs are met within the home. They discussed visits to the GP, Consultant Psychiatrist appointment, CPA meetings and other physical checks such as mammograms. One resident said, “if you are poorly, this is recognised and responded to appropriately”. Residents confirmed that they received care in the way they wanted and preferred it. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22, 23 The views of residents are paramount at The Avenue and are acted upon. Residents are protected from abuse, neglect and self-harm by the systems in place. EVIDENCE: Residents firmly believed they had a voice in the home and that they were listened to and appropriate action taken. Examples of this included the weekly menu meeting and regular residents meeting. During discussion with residents, they were well aware of the topic of abuse and gave examples of this to the inspector. They were clear about what action they would take if they observed or heard anything of this nature. They were confident in reporting any such incidents to the manager and were confident that the correct action would be taken. They said that there had never been any incidents of this nature in all the time they had resided at The Avenue. At previous inspections the policy and procedure were in place for Protection of Vulnerable Adults and staff were fully aware of this. No changes have been made to this since the last inspection and there have been no new staff appointed. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 The current hot water situation did not always allow for residents to have a hot bath. The garden needs to be developed further to ensure a safe and accessible environment for all residents. EVIDENCE: During discussion with the manager and residents, they stated that there had been some problem with the upstairs boiler and there had been occasion when there was no hot water for baths, although they could still have showers. It was confirmed that several quotes had been obtained for the repair or renewal and that the work would be completed in the near future. The garden was very pleasant, however there was a mound of garden rubbish in the centre of the lawn and had been there for some time.
