Conditions of registration: Date of last inspection 5 August 2004 Brief Description of the Service: Barnham Manor is a Care Home registered to accommodate up to twenty-three service users in the category OP (Old age not falling into any other category) with N (Nursing). Barnham Manor is located on the outskirts of the village of Barnham. Local shops and transport services are near by. Accommodation is provided on two floors: the first floor can be accessed by a passenger lift.The responsible Individual on behalf of the organisation is Mr Gary Curpen. Mrs Sivagamee Curpen is the registered manager in charge of the day to day running of the establishment. This is an overview of what the inspector found during the inspection. This unannounced inspection took place over 5 hours and was carried out as part of the routine programme of inspections. Preparation for this inspection focused on a review of previous inspection reports and general correspondence. During the course of the inspection the inspector toured the home, spoke with residents either privately in their own bedrooms or within the communal areas of the home in order to gain a sense of how the home is being run and how they experienced living at the home. Two visitors were also spoken with in the company of their relatives. Three staff were spoken with in order to gain a sense of the support and training they receive in order to carry out their jobs and to gain insight into their knowledge of the aims and objectives of the homes philosophy of care. Since the last inspection the refurbishment work has been completed. The home appears warm and welcoming, and the standard of cleanliness throughout is of a good standard. One residents who has been living in the home whilst the work has been carried out said that they are pleased with the changes and that the home ‘is much cleaner now’. All residents spoken with said that staff are ‘kind, caring and helpful’. Both residents and relatives told the inspector that they are satisfied with the care and services they receive. The inspector also took the opportunity to observe the interaction between both residents and staff, it was noted that the atmosphere within the home was jovial and relaxed and that the staff carried out their duties in a respectful manner taking into account the dignity and privacy of residents. What the service does well:
Residents told the inspector that the standard of care they receive is very good and that staff make every effort to maintain their privacy and dignity. The majority of residents told the inspector that meals provided were of a good standard. The general cleanliness of the home is of a good standard. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 Prior to moving into the home each prospective residents needs are assessed in order to ensure that the home can meet their needs. EVIDENCE: Written pre admission assessments were available and demonstrated that resident’s health, personal and social needs are assessed. Residents spoken with who have recently been admitted confirmed that they were either visited in their own homes or visited Barnham Manor themselves for the day in order for the assessment to be carried out. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,10 Care plan clearly set out the health, personal and social care needs of residents, which enables staff to maintain continuity of care. All care needs are carried out in an appropriate manner ensuring that they dignity and privacy of residents are maintained. EVIDENCE: Four care plans were reviewed, they clearly identified the individual needs of service users. The majority of residents spoken with confirmed that they are aware that information regarding their care needs are kept and that staff refer to them in order o meet their varying needs. Care plans contained all relevant information, including risk assessments for manual handling and nutritional assessments. During the course of the inspection the inspector was able to observe staff maintaining resident privacy and dignity by knocking on doors prior to entering their rooms. Residents who were asked confirmed that staff ensure that they are appropriately covered when being taken to and from the bathroom. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,15 Residents are enabled to satisfy their social, religious and recreational interests, which assists them to live a fulfilled life with in the home setting. Residents are offered a well-balanced and varied diet. EVIDENCE: Service users spoken with confirmed that they have the opportunity to take part in the homes programme of activities. There are various activity programmes displayed around the home along with the list of Communion dates. Activities include sing-a longs, music and movement, reminiscence and quizzes. Residents confirmed that they were able to take part in the planned activities or pursue their own interests as they wish. Residents told the inspector that a hairdresser attends the home weekly. The majority of residents spoken with told the inspector that food provided was of a good standard, however one service users told the inspector “that the meat especially the beef, was tough on occasion”. A four weekly menu is followed, it was confirmed that the menus get updated at regular intervals. Menus demonstrate and residents confirmed that they could have a cooked breakfast each morning if they choose. Alternatives to the main meal are available, residents are informed of the menu at coffee time should they not like the

A new dishwasher has been installed. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,18 A complaints process is in place enabling residents and relatives to raise their concerns with the management. Policies and procedures are in place to ensure the safety and well being of residents. EVIDENCE: There is a clear complaints procedure, which is included in the Statement of Purpose and Service User Guide. The records of complaints were reviewed. Residents spoken with confirmed they felt able to approach Mrs Curpen or her care team with any concerns they have. All staff have recently undergone training in Adult Protection procedures. Staff spoken with were clear of the action they would take should they suspect that a resident is being abused. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,21, 24,26 The location and lay out of the establishment were suitable to the needs of the current service users EVIDENCE: From touring the building the inspector was able to determine that the standard of cleanliness through out the home was of a good standard. A refurbishment programme has recently been completed. Bathing and WC facilities are sufficient in number. The establishment is well maintained, clean and free from offensive odours. A number of aids and adaptations are in place to enable service users access to various parts of the building and garden areas. Residents who were asked confirmed that they are able to furnish their rooms with their own belongings giving them a personal and homely atmosphere. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,29, Staff were on duty in sufficient numbers and the skill mix of staff as appropriate. A recruitment procedure is in place however all staff must undergo a Criminal Records Bureau Enhanced check prior to commencing employment. EVIDENCE: Duty rotas demonstrated the numbers of staff on duty. One member of staff who has recently been re employed told the inspector that she is aware of the need to submit her CRB application but has not had the time to do so yet. Mr and Mrs Curpen also confirmed that this employee has not yet submitted the application. The inspector advised them that this must be carried out as a matter of urgency and that whilst the CRB check is being sought the employee must be supervised at all times. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,35 The home is run and managed in an appropriate manner. Service users financial interests are safeguarded. EVIDENCE: Mrs Curpen is an experienced nurse who has experience in managing services. She advised the inspector that she will be completing the Registered Managers award in September/October2005. Mr and Mrs Curpen advised the inspector that they do not manage resident personal finances, which was confirmed by residents. However, some money is held for resident on request so that they can pay for things such as hairdressing, small personal items and chiropody. Individual residents money is kept separate and records are kept of transactions made. Should a resident not wish to have money held in safe keeping there is a system in place whereby Mr Curpen can bill residents or their relatives each month.
