The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home has high standards and provides a homely environment in peaceful surroundings, for residents to enjoy. To ensure that residents needs can be met each resident is assessed by qualified staff before moving into the home. Residents say that staff are caring and that they are well looked after. Care plans are clear and provide staff with the information, which they need to meet the needs of the residents. Meals are varied, nicely presented and offer choice. The manager takes all complaints seriously and acts upon them to ensure that residents do not continue to be dissatisfied with any particular aspect of the service. Staff, residents and a visitor spoken with all had confidence in the managers approach to running the home.

What has improved since the last inspection?

There is an ongoing programme of redecoration of the home, which ensures that the home is maintained to a high standard. The en-suite facilities in one bedroom has been refurbished and enlarged to provide easier accessibility for the resident. Self-closing devices have been fitted to the majority of bedroom doors to allow residents to have their bedroom doors open during the daytime if they wish.

What the care home could do better:

Accident records should be completed for all accidents sustained by residents and should be available at for inspection. Staff records should all contain evidence of the member of staffs identity even though they have obtained Criminal Record Bureau (CRB) and Protection of Vulnerable Adult (POVA) clearance.

Two of the twenty eight residents may also have a physical disability. The home is situated in a rural area of Fishbourne on the outskirts of Chichester. The accomadation is on two floors, the upper floor having passenger lift access. Three bedrooms on part of the upper floor can only be accessed by steps. Communal accomadation consists of a lounge and a conservatory which open out onto well maintained, private gardens. Private accomadation consisits of twenty single bedrooms and four double bedrooms. The double bedrooms have en-suite facilities and are currently being used for single accomadation. This announced inspection took place over five and three quarter hours on the 14th July 2005. The inspector arrived at 10 am and was welcomed by the manager of the home. A full tour of the home took place and the majority of the twenty residents living at the home were met. Five residents were spoken with in depth to find out if they felt that their needs were being met. During the visit the inspector spoke with one visitor, who confirmed that they were regular a regular visitor to Beggars Roost and was satisfied with the level of care provided for their relative living at the home. The care records of four residents were inspected during the visit along with other records, which showed how care needs are to be met. The records of two staff were also inspected and two staff were spoken with informally. The manager and a director of Colville Care Limited were present to assist the inspector. There were no service user comment cards returned prior to the visit to Beggars Roost. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3,4,5 and 6 Residents are assessed prior to moving into the home to make sure that the home can provide a care plan which residents or their families know will meet their needs. Prospective residents, their relatives and friends have an opportunity to visit the home to look at the facilities available and assess its suitability before moving in. EVIDENCE: Three of the residents spoken with explained how they had come to live at beggars Roost. All three residents said that their relatives had visited the home on their behalf because they had not been well enough to visit themselves. The manager was able to give examples of visiting residents in hospital or in their own home to carry out assessments. Care records viewed at this visit showed that an initial assessment is carried out prior to moving into the home and then reviewed after moving into the home, to ensure that the residents needs can be met. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,10 Systems for care planning give clear information to assist with all aspects of health, personal and social care needs. Resident’s heath care needs are monitored to make sure that any signs of residents becoming unwell are recognised. Procedures for recording accidents are not always adhered to, which could result in residents fall risk assessments not being reviewed if not all falls are recorded. Medication at the home is well managed and ensures that residents are protected by the homes procedures for dealing with medication. Residents are treated with respect and their rights to privacy are recognised. EVIDENCE: Care plans examined had been developed from an initial assessment carried out prior to residents moving into the home. The plans give clear information to staff to enable them to meet the needs of the residents in all aspects of heath, personal and social care. Care records showed that the physical health of residents is regularly monitored, including weight and dietary needs. Residents spoken with confirmed that they have access to the health care professionals. One resident said that they had recently had a visit from the doctor. Another residents had recently been into hospital for an operation and had then returned to the home to be cared for by the nursing staff. Whilst examining one residents care records it was observed that the residents fall risk assessment had been reviewed following a fall. However when examining the accident records it was noted that no accident report had been completed for the incident. Other accident records viewed were in good order and had been audited to analyse trends. Medication procedures are in place to protect residents from the mishandling of medicines. Records viewed were well kept and up to date. Residents commented that if they want to stay in the privacy of their bedroom they could. During the visit staff were observed to knock on doors before entering bedrooms and were heard to speak to residents in a respectful manner. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13, 14 and 15 The home provides a lifestyle that respects privacy, dignity and choice, so that residents are able to retain some control over their lives. Residents are encouraged to maintain contact with their family and friends so that they so that they can satisfy their social and emotional needs. Meals are well managed and offer choice so that residents have a variable diet and have meals, which they can look forward to. EVIDENCE: When the inspector arrived at Beggars Roost at 10am some residents were sitting in the lounge and conservatory. Others were being assisted to get washed and dressed in their bedrooms. There were a number of residents being nursed in bed during this visit to the home and it was observed that the care staff was monitoring them regularly. Residents spoken with during the visit commented about the flexibility of the home. One resident spoke about the fact that the although the staff were “enthusiastic with showers and bathing” they did feel able to say when they didn’t want a bath and was sure that they would be able to have one on an occasion chosen by themselves. An example of this was one resident’s interest in sailing. The resident had a model of a boat in the bedroom and a collection of paintings of sailing boats painted by his late wife; he was able to refer to these when talking about his experiences of sailing locally. Residents spoken with confirmed that they are able to keep in contact with relatives and friends. A visitor to the home confirmed that when they visit their relative they are always made welcome. The visitors book observed in the entrance hall records regular visitors to the home. The home arranges some entertainment, which is facilitated by outside organisations, these include: movement to music, various weekly activities and visits by a”PAT” dog for those residents who have had and miss a dog. The home organises seasonal activities such as parties. There is a new chef at Beggars Roost whom a resident described as “ a very good cook”. The same residents commented that each day one of the staff comes round and tells you what is on the menu for the day and then you can choose what you want. Another resident spoken with described the food as “fantastic”. On the day of the visit the main meal of the day was roast chicken, carrots, brussel sprouts and creamed potatoes. The chef spoke about alternatives provided for those residents on special diets and provided assurance that if a resident didn’t want something from the days menu he would go and speak to the resident to find out what could be provided instead. A visitor to the home confirmed that their relative living at the home has meals, which are made softer to make them easier to swallow. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 There is a clear complaints procedure, which enables those using the service to have the confidence that their complaint will be responded to within a maximum of 28 days. EVIDENCE: The home has a complaints procedure, which is included in the Service User Guide. There has been one complaint made since the last inspection at the home, which has been investigated through Beggars Roosts, own complaints procedure. There have been no complaints made directly to the Commission for Social care Inspection (CSCI) since the last inspection. Records of the complaint made to the home since the last visit were viewed at this visit and were observed to be well documented. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,22,23,24,25 and 26 Beggars Roost is well maintained and provides a safe environment for residents to move around in. Residents have access to a safe well-maintained secluded garden to enjoy during the summer months. The home is clean and residents have a comfortable, homely environment to live in. Comfortable bedrooms are provided which residents have personalised to make homely. EVIDENCE: The inspector visited all communal and private accommodation during the visit. The home was observed to be clean and well maintained. There is an ongoing programme of redecoration at the home, which ensures that residents have homely well-maintained bedrooms. Since the last visit to the home one bedroom has had the en suite facility completely replaced with a more modern Beggars Roost Nursing Home H60-H11 S24116 Beggars Roost V229825 130705 Stage 4.doc Version 1.30 Page 15 spacious facility. There is also plans to extend the home to provide a further seven ground floor bedrooms with en suite facilities. However the providers do not anticipate that this will increase the number of residents able to live at the home because further internal refurbishment of the present building will reduce the number of bedrooms but provide additional en suite facilities. The refurbishment will also include a new link corridor from the upper part of the first floor, which at present can only be accessed by a number of stairs. The residents spoken with during the visit all liked their bedrooms. One resident said that they had “ a lovely room” another resident who is confined to bed commented that they “felt very lucky to have a ground floor room which has a door out to the garden and a window which had such a lovely view of the garden”. The bathrooms and toilets seen by the inspector were clean and had grab rails to assist in the use of toilets. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28 and 29 The staffing numbers are set at level, which allows residents assessed needs to be met. Beggars Roost provides well-qualified staff so that residents are able to feel safe and well looked after. Recruitment procedures do not fully safeguard and protect residents at the home. EVIDENCE: The rotas provided prior to the date of this visit to the home showed that there is one trained nurse and five care assistants on duty between 8am and 2pm. Between 2pm and 8pm there is one trained nurse and three care assistants. At night from 8pm to 8am the next day there is one trained nurse and two carers on duty with a senior nurse on call. The care staff are supported by cleaners, a chef and an administrator. There are currently twelve care assistants working at the home, six of which are qualified to NVQ Level two or above, they support the qualified nurses to ensure that residents are in safe hands at all time. Residents spoken with during the visit to the home had positive comments to make about the staff and manager of the home. One resident said “ we’ve got good staff here” and another resident said, “the staff are all very kind and helpful”. Those records viewed showed that recruitment procedures are followed but only one of the two sets of records viewed had on file evidence of the member of staff’s identity. Such identity would have had to be provided for the member of staff to obtain evidence of Criminal Record Bureau (CRB) and Protection of Vulnerable Adult (POVA) clearance, which was available. This matter was discussed with a director of the company and an agreement reached. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32 ,33,37,38 The home is well managed providing leadership and guidance for staff. The homes record keeping does not fully protect and safeguard the residents living at the home. Beggars Roost provides an environment, which promotes the health, safety and welfare of residents and staff. EVIDENCE: All residents spoken with during the visit to the home spoke highly of the manager. A visitor commented that the “ manger is very good and is helpful and kind”. The majority for the records viewed during this visit to the home were in good order however: (a) accident records were not available for an accident sustained, by a resident, which the inspector requested to see. The home was observed to be a reasonably safe place to live, planned improvement to the environment will allow better access to bedrooms on the upper floors.
