Conditions of registration: Minimum Staffing Notice. Date of last inspection 30th November 2004 Brief Description of the Service: Beis Pinchos Nursing Home provides specialist care services for older people from the Orthodox Jewish Community. The complex of Schonfeld Square is owned by Agudas Israel Housing Association and is a purpose built property. Beis Pinchos is a dually registered care home for 43 older people including 8 places for people with dementia. The home is based in the London Borough of Hackney on the border of Stoke Newington and Stamford Hill. It is in the midst of the largest Orthodox Jewish Community in Europe. Schonfeld Square is easily accessible by public transport and is near kosher shops and several parks. Beis Pinchos offers a wide range of religious and culturally appropriate services and activities to both service users and the well established Orthodox Jewish Community in the immediate area. The home takes referrals from throughout the UK and abroad and is currently operating the waiting list. This is an overview of what the inspector found during the inspection. This inspection took place over two days. On the first day the inspector (Mr Tim Weller, Regulation Manager) visited the home to speak to service users, the manager, proprietor and staff. He also reviewed documentation kept in the home relating to complaints conducted a brief tour of the premises and randomly reviewed the medication system. The second visit was carried out by Mr Robert Sobotka (Lead Inspector). The inspector spoke to a number of service users, some relatives, staff working in the home, the Registered Manager and the Proprietor. In addition he conducted the tour of the premises and reviewed a number of documents maintained by the home. A number of people living in the home, members of staff working there, as well as other health/care professionals have been asked to complete pre-inspection questionnaires, comments from which have been included in this report. What the service does well:
The home provides an exemplary quality of care and service to those living at Beis Pinchos. The inspectors received numerous praises about the home from the service users, their relatives, as well as healthcare professionals. Those who used the service were treated with respect and dignity and were offered a very wide range of activities both within the home and externally. All staff working in the home were very committed and enthusiastic about their jobs. The home had a very high number of staff on each shift, as well as forty four volunteers, a number of whom were visiting the home each day. All staff working in the home were fully aware of Jewish customs and religious practices. Staff have also received good training to ensure that the needs of those living in the home were fully met. Service users were provided with very varied meals and all requests were promptly met. Despite the size of the home, there was a homely feel and all those who lived in the home received adequate support. The healthcare needs of those who lived in the home were fully met by a dedicated team of nurses and healthcare assistants. All appropriate health and safety checks were in place.

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 2, 3, 4. Those who lived in the home received all the necessary information to make an informed choice about moving into the home. The home had a good assessment system in place. The needs of those living in the home were being met. EVIDENCE: The home had an exemplary Statement of Purpose and the Service User’s Guide in place. It contained all necessary information and in addition the brochure was attractively designed and included photographs of members of staff, some of the service users and visitors engaging in culturally appropriate activities and religious observances, as well as comments from residents, their relatives and members of pubic. Each person had an individual contract, which included fees payable and outlined service provided and terms and conditions. There have been some new admissions to the home since the last inspection. The home manager appropriately assessed each person, so that the needs of each person could be met.

At the time of this inspection, the home did not provide intermediate care. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9, 10, 11. The home was appropriately meeting the healthcare needs of its service users. Existing medication systems needed to be improved to address inaccuracies identified. EVIDENCE: Each service user accommodated in the home had a written care plans. The inspector viewed four care plans, which were chosen at random. Care plans viewed were well written and they were reviewed on a regular basis. Documents included actions needed by care staff to ensure that all healthcare, social and personal needs of those who lived in the home were taken into account. As part of this inspection, questionnaires were sent out to the health and care professionals. Fourteen comment cards were returned; all but one showed that the respondents were satisfied with the overall care provided to service users within the home. Comments on the cards included: - “Brilliantly run”, - “Excellent care provided”, - “Excellent service”, - “I think that the nurses at Schonfeld Square provide an excellent standard of care for their clients. They are caring, efficient and communicate very well with

If I had an elderly relative, I would be happy for them to be left in their care.” The home has recently purchased some new nursing beds. The registered manager advised that in due course all beds would be replaced with specialist equipment, which would benefit both service users and staff attending to them. Each person living in the home was registered with a General Practitioner. They were also able to access any specialist healthcare facilities which they may require. The home is also commended for the management of MRSA and ensuring that service users are protected from flu. During the inspection, the registered manager gave an example of the home participating during the winter months in the flu prevention programme, which proved to be a great success. All accidents and incidents were well recorded. The registered manager produced periodic evaluation to monitor falls management within the home. The home employs a high number of nurses. They are provided with additional training to further enhance and develop their knowledge of nursing practices. Only registered nurses are authorised to administer prescribed medication to service users. Medication stocks and administration records were examined during this inspection and minor inaccuracies in accounting and recording were identified. In order to evidence that medication is administered as prescribed accurate record keeping is essential. Staff working in the home were seen to treat service users with dignity and respect. Service users’ wishes in relation to personal care were also respected. The death of a service user is handled according to Jewish law. Staff have received training in palliative care to ensure that service users received appropriate attention and pain relief. Staff have also attended a lecture on cancer. Relatives are able to stay with a service user for as long as they wish. Since the last inspection, the home built a morgue at the rear of the garden. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14, 15. Those who use the service benefit from an excellent range of activities. Service users are helped and supported to take exercise choice and control over their lives. EVIDENCE: The home provides a very wide range of activities. There were a large number of volunteers from the local community who facilitated some of the activities. Each service user had a lifelong learning document in their file. The home had a minibus, which is used for outings. Activities programmes are given out each Sunday for the forthcoming week to each service user. Activities programmes are also displayed throughout the house. Activities usually start at 10.15am and finish at around 9pm. Those who spoke to the inspector said that they were very happy with what was on offer. There were also specialised activities for people diagnosed with dementia and Alzheimer’s disease. Maintenance of family involvement is very much part of the Jewish ethos of the home. There were no restrictions on visits and relatives and friends of the service users are given a copy of the home’s brochure. The inspector received seven comment cards from service users’ relatives. Everyone who responded stated that they were happy with the way the home is run and the quality of care offered to their relatives.
Beis Pinchos Nursing Home G56 G06 S7351 Beis Pinchos NH V221054 070605 Stage 4.doc Version 1.20 Page 14 Some of the comments from the relatives were as follows: - “The overall care is excellent”, - “Staff attitudes to residents and visitors are unfailingly positive”, - “Beis Pinchos appears to be run very well. My mother has been resident for over 2 years and the care is exemplary.”, - “The care in Schonfeld Square care home is outstanding. All staff are friendly and show both skills and compassion with residents. The atmosphere is that of a personal home, not institutional, and I am impressed on each visit.” Most service users’ finances are handled by their relatives, which is traditionally accepted. Service users are encouraged to bring their own possessions, as agreed on admission to the home. Those who lived in the home had access to their records, in accordance with the Data Protection Act. All meals provided by the home are Kosher and are cooked to meet traditional Jewish cuisine. The kitchen conforms to the Jewish laws. The inspector joined service users for a lunchtime meal, which was well presented, nutritious and tasty. All meat, fruits, vegetables and dairy products are delivered to the home on a daily basis. There is an alternative meal offered to the service users as well as a vegetarian option. Any other requests and special diets are also catered for. Menus are provided in large print. Kitchen staff are supplied with a list of service user’s birthdays. Mealtimes were unhurried and service users had an option of eating on their own or in company of others. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, 18. Service users are protected from abuse. Complaints are few however a more comprehensive system for responding to them is suggested. EVIDENCE: Staff spoken to at the time of the inspection were aware of adult protection issues and the home had appropriate policies and procedures to ensure that those living in the home were protected from abuse. The home receives very few complaints about the service it delivers. However, a complainant had put their complaint in writing on more than one occasion but had not received a response in writing. It was verbally evidenced by the home that action was taken in relation to the complaint, however it is reasonable to expect that correspondence is acknowledged. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 21, 22, 23, 24, 25, 26. Those who live in the home benefit from a homely, comfortable, clean and safe environment. EVIDENCE: The premises are located in the heart of Stoke Newington. The home is situated in Schonfeld Square, which is purpose built and has won some architectural awards. Service user bedrooms are located on three floors. All bedrooms have en-suite facilities and are similar in design. There are 3 quiet lounges and a larger lounge on the ground floor, where most of the activities take place. There is also a synagogue. With the exception of some corridor areas where minor aesthetic work was required to bring the wall finish up to standard, the premises were well maintained. The third floor accommodates 8 service users with dementia. All communal areas were spacious. The main lounge is used for a variety of specific social, cultural and religious activity and there are three lounges where service users can meet relatives. Lighting in communal areas is sufficiently bright and positioned to facilitate reading and other activities.

All bedrooms have en-suite facilities and an additional sink unit housed behind the cupboard doors. There are 6 communal bathrooms, one with a Parker bath, which is on the first floor. All bathrooms were clean, tidy and hygienic. They were pleasantly decorated and the flooring was in good condition. The home had adequate provision of aids and adaptations in place to assist service users with physical disabilities. The home has a lift, grab rails in corridors and communal areas. There are call alarm systems in all rooms. Alarm system and the lift are serviced on regular basis. All rooms had a bed, a built-in wardrobe and a further wardrobe, a chest of drawers and a table and chair. All bedrooms were kept closed and were lockable. Rooms viewed by the inspector were clean and in good decorative order. The home had appropriate laundry facilities. Sluicing facilities were also in place. The home was clean and hygienic at the time of the inspection. Appropriate clinical waste disposal arrangements were in place. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 28, 29, 30. Those who live in the home continue to benefit from a dedicated and well trained staff team. Staff recruitment practices required improvement. EVIDENCE: The inspector was satisfied that the home had appropriate staffing levels to meet the needs of the current service user group. Staff spoken to and those who returned inspection comment cards to the inspector were very happy and proud to work in the home. Staff morale continues to be high. Since the last inspection, the home employs two nurses during each night shift. There is also an additional nurse on call at all times. The home has a large volunteer pool, who spend time with service users every day. As part of this inspection, staff personnel files were examined. One of the files for a new member of staff did not contain all information required by law. This was brought to the attention of the registered manager, who took appropriate action immediately. The home was able to evidence that all staff (nursing, care and domestic staff) receive very comprehensive training. The management have a commitment to ongoing training and development. All new staff undergo a comprehensive induction based on the TOPPS induction programme. The programme includes religious observances and orthodox Jewish laws and customs. Once completed the induction pack is signed by the registered manager. All members of staff spoken to stated that all training offered is of a very high quality and they felt valued due to the fact that the organisation considers staff training and development as a vital factor contributing to the excellent quality of care

Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 33, 36, 37, 38. The service is run by a competent manager who ensures that the needs of those living in the home are met. Appropriate health and safety checks were in place. She is a registered nurse and was competent. The inspector received positive comments from service users, staff and one Health and Social Care Professional (via pre-inspection comment card) about the way the manager runs the home. The registered manager, must however obtain the relevant qualification in management by the end of 2005. It was transparent that the management approach of the home (this includes the Responsible Individual) created an open, positive and inclusive atmosphere. This was reflected by the high staff morale, members’ of staff commitment to providing excellent quality of care to service user. The inspector viewed team-meeting minutes, which covered a wide range of topics discussed amongst the team. All staff received regular supervision. The home is commended for its quality assurance and quality monitoring systems. Monthly person in control visit reports viewed evidenced that the Responsible Individual is very committed to ensuring that the views of those who used the service are obtained. Views of relatives and staff are also sought. In addition relatives and residents meetings are held on a regular basis. Service user questionnaires are circulated to all living in the home on a quarterly basis, after which the comments are analysed by the home management and action taken. Supervision and appraisal systems were in place. Staff working in the home benefit from a wide variety of courses/lectures offered to enable them to carry out their job effectively and in a professional manner. Additional support is offered to those staff who do not use English as their first language. Staff were aware of their roles, responsibilities and lines of accountability. The home had appropriate policies and procedures in place. Health and safety records kept in the home were well maintained. The home had appropriate insurance cover in place.
