Conditions of registration: PD: Up to 2 persons with a physical disability may be accommodated commensurate with the homes Statement of Purpose. Date of last inspection 12th January 2005 Brief Description of the Service: Belmont Grange is a care home registered to provide care (including 24 hour nursing care) for persons aged 65 years or older. It is situated in the centre of Belmont and is within easy reach of the shops and local amenities. The main building is listed with a modern built extension added. It consists of a two-storey building. There are 25 single bedrooms and 4 double rooms, 1 of which is en suite. However double rooms are not routinely used unless specifically requested. There is a passenger lift available. The home and was purchased by the current owners Four Seasons Healthcare (England) Limited in 2000. The home is surrounded by which are generally well maintained. However on the day of inspection there was some unavoidable debris in the form of trees, which had been blown down during the very recent adverse weather conditions. The Registered Manager advised that arrangements were in place to address this issue.

This is an overview of what the inspector found during the inspection. CSCI has a statutory duty to inspect all care homes at least twice a year. This announced inspection was carried out in accordance with this duty. The inspection took place on the 28th June 2005 between the hours of 09:30am and 3pm. During the inspection of the home, time was spent talking to service users, staff and relatives. Records were examined and a tour of the building took place. In line with current CSCI policy on Proportionality, the inspection focused upon a number of key standard outcomes for service users. The key standard outcomes not inspected on this occasion will be raised during the next inspection of the home. What the service does well: What has improved since the last inspection?
Due to the acting manager only recently taking up post there has been limited progress since the last inspection. However both she and the Regional Manager demonstrated during the course of the inspection that plans are in place to move the service forward. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2,3,&4 Information is given to prospective service users to help them make an informed decision about moving into Belmont Grange. Appropriate assessment procedures are in place but these needs to include more information about peoples mental health, psychological and physical needs to ensure that the home can meet the needs of service users. EVIDENCE: The contract for service users is being reviewed by the Company and is not yet available. Assessments from Care Managers are obtained for all new admissions funded by the Local Authority. The home also carries out its own assessments on service users; some of the areas within these were blank, not all had a completed social assessment and risk assessments were not fully completed. Assessments form the basis from which the individual plan of care will be written and demonstrate how the home will meet an individuals needs. The information provided within assessments must cover all areas of physical, social, emotional and psychological needs. Where possible service users and/or their relatives should be involved in the assessment process.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 & 8 Individual care plans are in place, however these require further development to ensure they reflect all of the information obtained during the assessment process. EVIDENCE: Four care plans were reviewed during the inspection. The care records examined did not contain plans to meet all of the needs identified in the assessment. One service user who had been admitted on 2nd June 2005 had an incomplete assessment and no care plans available. Staff confirmed that the needs of these people were being met even though there was a lack of clear plans. This approach is dependant on informal communication systems. Residents are at risk of not having their needs met if these informal systems break down. Where appropriate, plans contained some evidence of input from other health care professionals. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 The range and choice of activities available within the home were extremely limited due to there being no dedicated activities organiser. EVIDENCE: Feedback from staff service users and relatives indicated that activities are limited with one service user stating that ‘there is nothing to all day, the staff attend well to our health needs but do not have time to even sit and talk never mind organising an activity or outing’. JUDGEMENT – we looked at outcomes for standard(s) 19,24&26 The home is safe and well maintained however some furniture and carpets are showing signs of wear and tear. EVIDENCE: The home is generally well maintained; a number of maintenance files were examined and found to be up to date. The inspector looked around the building, which was found to be clean, tidy and free from offensive odours. Service users could individualise their rooms with their own furniture and personal belongings. The home was found to be clean and free from unpleasant odours. EVIDENCE: Individual staff records were viewed which confirmed that robust recruitment procedures are in place. Individual staff training records were found to be up to date and applicable to the skills needed to deliver the care service to the people living in the home.
